Mycosis fungoides(MF) is a cutaneous T-cell lymphoma of low-grade malignancy characterized by the proliferation of small to medium-sized cerebriform lymphoid cells confined to the skin. It usually shows favorable prognosis, but morphologic transformation occurs in some cases and is often associated with a more aggressive clinical course. Herein we report a case of Ki-1 positive large cell lyrnphoma (Ki-1 LCL) which developed in the plaque stage of mycosis fungoides. Although the lesions responded well to low-dose methotrexate therapy and there was no evidence of local or systemic recurrence until now, continuous follow-up is needed because the prognosis of transformed MF is known to be poor compared with primary cutaneous Ki-1 LCL. We also discussed the differential diagnosis of primary cutaneous Ki-1 positive lymphoproliferative disorders.

We report the clinical evolution of two elderly male patients with extramammary Paget's disease who were treated with radiotherapy. Both cases primarily involved the scrotum and penile root, and were not associated with any underlying or visceral malignancy. The total doses administered were 6000 cGy and 3000 cGy, respectively. A good clinical response was achieved in both cases. No major side effects were observed. The patients are being followed for any signs of recurrence and have been completely asymptomatic to date

We report a case of alopecia neoplastica-a rare form of cutaneous metastasis-in a 43-year-old Korean woman, who presented with multiple red, bald patches of 1 month's duration on her scalp. About 2 years previously, she received a modified left radical mastectomy and adjuvant chemotherapy for breast cancer. Histopathological findings of the bald lesions revealed diffuse infiltration of hyperchmmatic tumor cells arranged singly and in strands and cords in the upper and lower dermis. We started chemotherapy with adriamycin, but she died 13 months later.

BACKGROUND: Although the culture of urethral discharge using a cutton swab is regarded as one of the most sensitive and reliable diagnostic methods of gonorrhea, it may give pain or diagnostic methods of gonorrhea, it may give pain or discomfort to patients, and its detection rate may be influenced by the examiner's technical ability. OBJECTIVE: The purpose of this study was to compare the positive culture rates between ure4thral discharge and urinary sediment for the diagnosing of Neisseria gonorrheae. METHODS: We compared this technique with the culture of urethral discharge using the specimens from 357 male patients with gonococcal infection at VD clinic of Jungku Pvblic Health center in Seoul from Jan 1991 to Jun 1993. RESULTS: The culture of urinary sediment was positive in 339 of 357(94.95), whereas the culture of urethral discharge was positive in 346 of 357(96.95). However, these data have no statistical significance(P<0.05). The obe observed sensitivity and specificity of culture of urinary sediment comjpared to culture of urethral discharge were 97.75 and 90.9%, respectively. The predictive value of positive and negative test compared to the culture of urethral discharge were 99.7% and 55.65, respectively. As our data showed low negative predictive value, a negative culture result of urinary sediment does not always imply a negative culture result of urethral discharge. CONCLUSION: Therefore, at present, the previous method of the culture of urethral discharge is more highly recommended and further studies for this diagnostic and/or new method of gonorrhea based on larger group of patients is encouraged.

The purpose of this study was to know the effect of short termed fasting on the usage of metabolic energy sources and the metabolic differences between non-athletic and athletic subjects. Subjects were divided into non-athletic and athletic group and exercise was loaded on both groups after feeding and fasting. Exercise was loaded by a treadmill running at the speed of 8 km/hour for 30 minutes in both groups. The experiment yielded following results. In the fed state, the level of plasma FFA increased markedly after 15 and 30 minutes of exercise compared with it's level of pre-exercise period in both groups. In the fated state, the level of plasma FFA in non-athletic group increased steadily according to the duration of exercise, while it's level in athletic group showed no changes. At pre-exercise period, the level of plasma FFA was higher in fasted state than fed state. Immediately before the exercise and 15 and 30 minutes after the exercise, blood for the determination of plasma free fatty acid (FFA), glucose, triglyceride (TG) and cholesterol was sampled from antecubital vein, and simultaneously heart rate was measured. In the fed state, the level of plasma glucose was increased mildly according to exercise, and in the fasted state it's level increased according to exercise in both groups also. In the fasted state, the level of plasma TG was lower than that in the fed state. The level of plasma TG and cholesterol in the fed state was no changed by the exercise from the pre-exercise period. The level of plasma cholesterol in athletic group had tendency to lower than that in non-athletic group. Heart rate increased markedly according to exercise in both groups, but the athletic group's increasing rate of heart rate was lower than the non-athletic group's heart rate increased according to exercise and athletic groups heart rate increased early period of exercise, but did not change during latest post-period of exercise.

The mechanism responsible for the formation of sunburn cells in mammalian skin is unknown. However it is suggested that the reactive free radicals and oxygen species generated by UV radiation are causing oxidative reactions in certain keratinocytes that are manifestated in the form of dyskeratotic cells and free radical scavengers such as a-tocopherol acetate or p-carotene were thought to be photoprotective against sunburn radiation. In the present study, we evaluated the effect of single and multiple applications of n-tocopherol acetate(a-TCA) and p-carotene on the formation of sunburn cells. We also determined the effect of these two antioxidants on the sunburn reartion by UVB irradiation. The results were as follows : 1. Both a-tocopheroJ acetate and p-carotene were photoprotective and prevented the formation of sunburn cells and sunburn reaction. 2. Topical p-carotene was less photoprotective than u-tocopherol acetate. 3. Topical use of a-tocopherol acetate and p-carotene in dose range exceeding 1,000 pgcm could provide photoprotective effect if the UVB exposure doses were less than 300 mJ/cm'( 3 MED)

A statistical observation ryas made on Genito-Urinary-Tract injuries of Korean Army Personnel in Vietnam War during the period of November, 1965 to November, 1966. and the following results were obtained. 1. The Genito-Urinary-Tract injuries were 15 cases(5.3 per cent) among 825 war causalities. 2. As for the causes of injuries, 25 cases (55.5 per cent) were caused by gunshot, 9(20.O per cent) by booby trap, 5(11. O per cent) by mortar, 3(6.6 per cent) each by crush and grenade. 3. Concerning the frequency of injuries in each organ, 12 cases (6. 6 per cent) each were observed in urethra, in scrotum and testis, 10 (22.2 per cent) in kidney, 8 (17.7 per cent) in bladder, and 3 {6. 6 per cent) in penis. 4.The incidence of Genito-Urinary-Tract injuries associated with the other organs was as follows. In kidney injuries, Z cases were associated with intestine, 4 with liver, 3 with spleen and one each with radial nerve and iliac vessel. In bladder injuries. 2 cases were associated with pelvic bone, and one with spleen. In urethra injuries, 6 cases were associated with pelvic bone and 2 with intestine. 5. As for the multiplicity of Genito-Urinary-injuries, 4 cases were observed in scrotum, testis and urethra, and 3 each in urethra and bladder, and scrotum and testis. 6. Syringomyelia was described by Etienne, Morgagni and Santorini in 1594 for the first time and was named by Olliver in 1824. In spite of hundreds of years study, it's cause is still not distinctly known and symptoms are very much variable because of its wide, irregular lesion in the spinal cord. The patient of 34 years old male had been suffered from sensory and temperature dissociation and muscle weakness of lower extremity including lower trunk for two years. In case of classical syringomyelia, sense of pain and temperature were diminished on the same area but this patient had complained disturbance of pain and light touch on the same area, but temperature loss does not coincides with, as figure shows. This report will review syringomyelia of lumbosacral type with a case history and references which is relatively rare in incidences.

Congenital pseudarthrosis of the tibia is a condition most often combined with cafe-au-lait spots and neurofibromatosis. It is notoriously difficult to attain a sound bony union. Compression plating and screw fixation with massive bone graft resulted in successful union in our case.

Congenital fibrous bands may be found at the gluteus maximus, the gluteus medius,thequadriceqs,the triceps and the deltoideus muscles. Congenital fibrous bands of both gluteus maximus muscles are rare and cause extension contracture of the hip joints. We experienced a case of congenital fibrous bands of both gluteus maximus muscles associated with severe limited flexion of both hips. Almost full flexion of both hips was obtained by excision of the congenital fibrous bands.
