1.A Case of Hutch's Diverticulum Associated with Severe Bilateral Vesicoureteral Reflux.

We experienced a case of primary hepatic actinomycosis which was initially diagnosed by means of fine needle aspiration. The patient was a 31-year-old emaciated man with a 2-month history of 10 kg weight loss, right upper quadrant pain and flank pain. The liver was palpable and tender on physical examination. Computerized tomography scan of the liver showed two ill-defined hypodense masses in gallbladder fossa and inferior pole of right lobe. Hepatocellular carcinoma was clinically suspected. A CT-guided fine needle aspiration was performed. Microscopically, smears showed numerous radiating clusters of filamentous bacteria with many neutrophils and monocytes in necrotic background. The symptoms were improved by incision and drainage and massive administration of penicillin.

4.Study on Effect of Conjugated Equine Estrogen and Progestogen on Serum Lipid Profiles and Bone Mineral Density in Postmenopausal Women.

Hormone replacement therapy combined with progestogens induces changes in effect of estrogen on serum lipid levels and it has been known that the changes depend on a type and dosage of progestogen. It is also known that progestational agent induces positive ch-ange in bone mineral density. To study the effects of progestogen on lipoprotein and bone metabolism, we administ- ered conjugated equine estrogen 0.625 mg alone to 50 postmenopausal women, in combinat- ion with medroxy- progesterone acetate 5 mg to 40 postmenopausal women. The data demonstrated a beneficial effect in lipoprotein profiles in both groups. Total cholesterol in two groups decreased from the baseline values, LDL-cholesterol decreased significantly by 4.8 % in group I and 16.2 % in group II(p < 0.05), HDL-cholesterol increa- sed significantly by 11.3 % in group I and 14.7 % in group II(p < 0.05), triglyceride incre- ased slightly in both groups. Bone mineral density of femur was maintained and BMD of vertebrae increased by 1.1 % in group I and 2.0 % in group II, but it is not statistically significant. The differences of changes between two groups were not statistically significa- nt. Our results suggest that medroxyprogesterone acetate have no adverse effect on HDL -cholesterol and have no additive effect on bone mineral density in hormone replacement therapy.

5.Treatment of Distal Femoral Fractures with a Retrograde Supracondylar Intramedullary Nail assisted with Arthroscopy.

Since Green et al has introduced a new technique of retrograde intramedullary nail, the use of retrograde interlocking intramedullary nails has been recommended as one of the treatment options for the distal femoral fracture. However there are some disadvantages that an arthrotomy is required for insertion and the knee joint could often be violated. Authors present a simple, arthroscopically assisted method using the retrograde intramedullary nailing for distal femoral fractures with minimal invasiveness to the knee. From March 1995 to March 1997, the retrograde intramedullary nail was used to treat 9 distal femoral fractures. Five of 9 patients were fractured at the distal shaft of the femur and others were fractured at the supracondylar region of the femur. Only one of the fractures was open injury(Gustilo-Anderson grade II). Significant concomitant knee joint injuries were revealed through the arthroscopy in 3 patients. Eight of 9 fractures healed by 5 months, but one fracture was not healed and required bone grafting. Average knee range of motion was 130. Complications included 1 nonunion and 1 hardware failure. There were no patellofemoral problems and no posttraumatic arthritis of the knee joint. This arthroscope-assisted method have some potential benefits that include decreased risk of damage to the knee joint, early evaluation and treatment of the associated knee joint injuries, and accurate placement of the nail.

6.The Effect of Ginkgo Biloba Extract (Ginexin(R)) on Experimental Central Retinal Vein Occlusion.

PURPOSE: To investigate the beneficial effect of ginkgo biloba (Ginexin(R)) on ischemic retina model by retinal vein occlusion after photodynamic injury. METHODS: Ginkgo biloba 20 mg/kg/day, 100 mg/kg/day or carboxymethyl cellulose as a control group was administrated orally from 1week before the retinal vein occlusion to 2weeks after. After rose bengal was injected through vein of tail, retinal vein was occluded with argon laser. And then venous occlusion was confirmed by fluorescein angiogram. Rats were examined by using an indirect ophthalmoscopy and fluorescein angiography to detect reperfusion and new vessels. Eyeballs were enucleated for histological examination, retinal ganglion cell count and intravitreal glutamate level. RESULTS: The reperfusion of occluded vein was faster in the ginkgo biloba-treated group than the control group. In the ginkgo biloba-treated group, damage of the inner retina and TUNEL-positive cells were smaller than the control group on histologic examination. The concentration of retinal ganglion cells in the control group was lower than in the ginkgo biloba-treated group. CONCLUSIONS: Ginkgo biloba has protective effects in experimental ischemic retina after vein occlusion.

7.Relapse of Mantle Cell Lymphoma and This Was Diagnosed by Skin Metastasis on the Face.

Mantle cell lymphoma (MCL) is an uncommon and aggressive form of non-Hodgkin lymphoma and this typically involves lymph nodes. Extranodal involvement is frequent and especially in the bone marrow, spleen, gastrointestinal tract and Waldeyer's ring. MCL is not well represented in the dermatology literature because the skin is rarely involved. We describe here a case of relapse of MCL that was diagnosed by skin metastasis of the face. A 71 year-old male patient presented with erythematous nodules that had been on the Rt. forehead for 6 months. The patient was initially diagnosed as having MCL and he had undergone chemotherapy. A year after complete remission, multiple nodules developed on the forehead. Biopsy of the nodule revealed skin metastasis of MCL.

8.Medical students' satisfaction with clinical clerkship and its relationship with professional self-concept

PURPOSE:
Medical students construct their identity as a student physician through clinical clerkship. However, there is a lack of research on the effect of clinical clerkship on professional self-concept formation. The aim of this study is to analyze and ascertain the relationship between medical students' satisfaction with clinical clerkship and professional self-concept.
METHODS:
This investigation studied 84 third- and fourth-year medical students enrolled in the Ajou University School of Medicine and Graduate School of Medicine. Study measures tools included satisfaction with clinical clerkship and professional self-concept measurement. For data analyses, a descriptive analysis of the research variable characteristics was applied, gender differences in variables by years of medical school were analyzed with t-tests, and correlation analysis was used to check for relationships between variables.
RESULTS:
We found no statistically significant differences between satisfaction with clinical clerkship with respect to medical school year and gender. While professional self-concept did not show significant differences by year of medical school, we observed statistically significant differences by gender with respect to the subscales of professional practice and communication factor. In addition, satisfaction with clinical clerkship and professional self-concept demonstrated statistically significant positive correlation. The present research was able to confirm that there exists a correlation between medical students' clinical clerkship experience and professional self-concept formation.
CONCLUSION
Our study outcomes shows that provision of positive assistance as a measure to enhance satisfaction with clinical clerkship via the curriculum and environmental improvement is envisaged to lead to medical students' professional self-concept formation.
9.A Needs Analysis Oral Health Education Contents for Teachers and Parents Using Borich Priority Formula and The Locus for Focus Model.

The purpose of this study was to analyze the priorities of oral health education contents for preschool children by targeting teachers and parents using the Borich priority formula and The Locus for Focus Model. The survey was conducted in 212 teachers and 215 parents from December 26, 2017 to January 21, 2018. The priorities of oral health education contents were based on a 3-step analysis method, including the paired sample t-test, Borich priority formula, and The Locus for Focus Model. As a result of this study, the number of items about oral health education for preschool children that were prioritized by teachers was 7, while that by parents was 9. The top priorities that teachers and parents had in common were the following 5 items; “The progression of dental caries,”“Symptoms of dental caries,”“How to prevent dental caries,”“Eruption sequence of permanent teeth,” and “Method for emergency management of avulsed teeth.” The teachers' priorities of the oral health education contents were the same between teachers and parents; “Eruption sequence of deciduous teeth” and “The function of the permanent teeth” were added. The parents' priorities of the oral health education contents were the same between teachers and parents; “The effect of fluoride application,”“The number of permanent teeth,”“How to prevent malocclusion,” and “The appropriate timing of malocclusion treatment” were added. Based on the results of this study, when developing oral health education programs for teachers and parents, oral health education for teachers should include 7 items and oral health education for parents should include 9 items.

10.Effects of a mobile healthcare service provided by public health centers on practicing of health behaviors and health risk factors

BACKGROUND/OBJECTIVES: This study evaluated whether a mobile health (mHealth) application can instigate healthy behavioral changes and improvements in metabolic disorders in individuals with metabolic abnormalities. SUBJECTS/METHODS: Participants were divided into an mHealth intervention group (IG), which used a mobile app for 24 weeks, and a conventional IG. All mobile apps featured activity monitors, with blood pressure and glucose monitors, and body-composition measuring devices. The two groups were compared after 24 weeks in terms of health-behavior practice rate and changes in the proportion of people with health risks, and health behaviors performed by the IG that contributed to reductions in more than one health risk factor were analyzed using multiple logistic regression. RESULTS: Preference for low-sodium diet, reading nutritional facts, having breakfast, and performing moderate physical activity significantly increased in the mHealth IG. Furthermore, the mHealth IG showed a significant increase of eight items in the mini-dietary assessment; particularly, the items “I eat at least two types of vegetables of various colors at every meal” and “I consume dairies, such as milk, yogurt, and cheese, every day.” The proportion of people with health risks, with the exception of fasting glucose, significantly decreased in the mHealth IG, while only the proportion of people with at-risk triglycerides and waist circumference of females significantly decreased in the control group. Finally, compared to those who did not show improvements of health risks, those who showed improvements of health risks in the mHealth IG had an odds ratio of 1.61 for moderate to vigorous physical activity, 1.65 for “I do not add more salt or soy sauce in my food,” and 1.77 for “I remove fat in my meat before eating.” CONCLUSIONS: The findings suggest that the additional use of a community-based mHealth service through a mobile application is effective for improving health behaviors and lowering metabolic risks in Koreans.
