6.An Ipsilateral Crossed Pinning Technique to Fix Supracondylar Fractures of the Humerus in Children: A Report on the Technique to Escape form Ulnar Nerve Injuries

Of several possible configurations of pin fixation of a displaced supracondylar fracture of the humerus in children, the medial and lateral crossed pinning technique has been known to provide the greatest resistance to gross rotational displacement. A new technique with ipsilateral two lateral crossed pins was devised so as to avoid the ulnar nerve injury with good stability for fracture site. During the period from 1992 to 1994, 18 children with displaced supracondylar fracture of the humerus were treated by closed reduction and ipsilateral two lateral crossed pinning. Eighty nine percents of the final results were satisfactory. There were no ulnar nerve injuries and fixation loss in all cases from the treatment. This is a safe, simple and reliable technique for providing the good stability of fracture site as well as avoiding the ulnar nerve injury.

7.A Case of Subretinal Cysticercosis Associated with Subcutaneous Nodules.

A new case of the recently experienced intraocular cysticercosis associated with multiple skin lesions (subcutaneous nodules) is presented. Clinical picture of this case was characteristic with bizarre shape retinal hemorrhage, and clinical diagnosis was possible by the fluorescein angiography of fundus. Trans-scleral extraction of the subretinal cysticercus was performed successfully without rupture of a cyst-capsule through the infero-temporal quadrant of sclera near to the equator of right eye. Diagnosis of ocular cysticercosis was confirmed by histo-parasitological examination on the extracted cyst of 5 x 7 mm in size. Oral medication is generally not amenable treatment for the intraocular cysticercosis, and less traumatic surgical removal is the only effective method for the improvement of visual function or preservation of the eye itself. The immense review of literatures on the ocular cysticercosis including the subretinal invas ion was attempted.

8.MECHANICAL PROPERTIES OF TITANIUM CONNECTORS TREATED BY VARIOUS WELDING TECHNIQUES.

The use of pure titanium and titanium alloys have been increased recently in fixed, removable prosthodontics and implant fields as a framework. But when they were used for superstructures of implant of metal framework of removable prosthesis, welding is necessary to reconnect the fracture site to control the casting distortions. To overcome the difficulties in soldering the titanium due to high oxidation property, much effort have been devoted. In this study, some of mechanical properties were compared between pure titanium and Ti-6Al-4V alloy by usingl aser welding, electron beam welding technique and tungsten arc welding. Mechanical properties such as tensile strength, yield strength, elongation and microhardness were measured. And, in order to compare the effect of welding site and surrounding metal tissue according to the welding condition, SEM photographs were taken an element distribution was observed by Wave Dispersion Spectroscopy. Through analyses of the data, following results were obtained; 1. In items such as tensile strength, yield strength and elongation according to the welding techniques of pure titanium, only tungsten arc welded group showed significant lower value than other groups(P<0.05). 2. In items such as tensile strength and yield strength according to the welding techniques of Ti-6Al-4V alloy, control group and tungsten arc welded group showed significant difference among all the groups(P<0.05). 3. Ti-6Al-4V alloy exhibited significantly greater elongation than control group when the laser welding method and electron beam welding method were used, and elongation showed increasing tendency. 4. Pure titanium specimens exhibited increasing tendency of microhardness regardless of the welding technique applied, and especially tungsten arc welded group demonstrated a great increase of microhardness than parent metal. 5. There was no hardness change in laser welded group and electron bean welded group of Ti-6Al-4V ally, but in tungsten arc welded group, hardness changed greatly from parent metal to weld seam. 6. Through the metallographic examination and scanning electron microscopy, laser welding caused central fusion and recristallizations were formed and tungsten arc welding caused localized fusion to 0.3-0.7mm from the surface.

9.Telomerase Activity of Endometrium Related to the Effects of the Sex Steroid Hormone and Endometrial Cancer.

During the reproductive period, human endometrium undergoes a pattern of cyclic change. Human endometrium undergoes a complex pattern of proliferation, secretory activity, and menstruation over an approximately 28 days period. Proliferative activity is highest during late proliferative phase under influence of estrogen, and minimal activity in the late secretory and menstrual phase. To study a possible change of telomerase activity during menstrual cycle, telomerase activities in normal and hormone treated endometrium were tested using telomerase repeat amplification protocol(TRAP) assay. Telomerase activities were detected in 9 of 10 proliferative endometrium(90%), and maximal activity was shown in late proliferative phase. Only 3 of 10 secretory endometrium(30%) revealed weak activity. However telomerase activity was not detected in menstrual phase endometrium(N 2) and senile endometrium(N=3). Four of tamoxifen treated endometrium(N 4) and 1 of provera treated endometrium(N 3) Levels of telomerase activity of treated endometrium(N 4) and late proliferative endometrium(N 6) were as high as them of detected in endometrial cancer and hyperplasia. Above findings reveal that telomerase activity of endometrium is changed according to menstrual cycle, And the level of telomerase activity is related to proliferative activity of endometrium that is dependent on the status of female sex steroid hormone and tamoxifen treatment.
