During ideal physiologic occlusion in the intercuspal position, all molars are occluded simultaneously and each occlusal contact evenly transmits similar pressure. The purpose of this study is to analyze the occlusion in the mandibular bone fracture patients treated by open reduction. This patients were selected according to the following criteria: 1) complete dentition without any missing teeth, 2) relatively normal occlusion without original TMJ disorder. Occlusal analysis using T-scan system were performed in 22 patients and 20 normal subjects. Our results were followed: 1. The number of occlusal contacts and the magnitude or occlusal force were increased with time. (p<0.05). 2. In the unilateral mandibular fracture patients, occlusal force and occlusal contacts were increased with time on the both sides. 3. There was significant difference in the number of occlusal contacts and occlusal force between postoperative 1 month and normal group(P<0.05), but there was no significant difference in the number of occlusal contacts and occlusal force between postoperative 6 month and normal group(p>0.05).

This report is a part of the health survey, which was carried out in the summer of 1973 on all adults older than 15 years of age, inhabiting in the sampled area. Thirteen villages out of 31 natural villages in Shindong Myon, Chunseong Gun, Kangwon Province, a typical rural area of Korea were randomly sampled. A little over 70 percent of the population sampled, or 1198 persons were examined. The health survey included laboratory screening tests, history taking, physical examination and consultation to specialists for selected cases. Diseases of circulatory system were classified by WHO ICD A-List (150). Hypertensive heart disease was defined by the criteria that was used in U.S. Health Survey, and hypertension by WHO criteria for the purpose of comparison. Results obtained were as followings: 1. The overall prevalence of the diseases of circulatory system per 100 adults examined was 8.6 for male and 9.9 for female. 2. Age specific prevalence of the diseases showed gradually increasing pattern as age advanced for both sexes. For example, the prevalence for 15~19 years old age group was 2.4/100 adults which increased to 24.8 when age reached over 60 years. 3. The most prevalent disease category, 3.6/100 adults, was A-88: other diseases of circulatory system that was consisted mainly of varicose vein and hemorrhoids; the second one was hypetensive heart disease, 2.4. 4. Female had higher prevalence than male in A-84: other form of heart diseases (4 times, arrhythmia, beriberi heart disease, and heart disease secondary to anemia), in chronic rheumatic heart disease (4.5 times), and in hypertensive heart disease (1.4 times); in other hand, cerebrovascular diseases (4.5 times) and diseases of arterioles and capillaries (1.5 times) occurred more frequently among males than females. 5. The proportion of hypertensive heart disease among persons with definite hypertension was 22.0% for male, 25.8% for female, and it increased gradually as age increased. Hypertensive heart disease among persons with borderline hypertension was 2.2%. Only 27% of hypertensive heart disease patients had previous treatment for hypertension. 6. The prevalence of definite hypertension per 100 adults examined (double blood pressure check with interval of 30~40 minutes) was 9.5 for male, 9.4 for female, with increasing pattern by advancing ages; marked increase occurred after age of 45 years for male and 35 years for female. The prevalence of hypertension in this study was much lower than that of the study on employees of Korea Electricity Comparny when compared with the same age groups. 7. Electrocardiographic findings on 105 heart disease suspects revealed left ventricular hyperspecific change in 4 persons. This study was supported by China Medical Board of New York Inc.

The purpose of this study was to explore and describe the lived experience of the family caregivers with the demented elderly. For investigate purposes of this study, data collection was done from May 9th to October 16th 1997, by means of in-depth interviews with 6 individuals in caregiving families. The research question was "What are the lived experiences of being a family caregiver with the demented elderly? "All interviews were tape-recorded and transcribed for the analysis using Colaizzi's method. The main results of this study were as follows : 1) Family caregivers ascribed the cause of the dementia to 'environmental change' and the 'introverted personality of the elderly'. 2) Family caregivers let the demented elderly alone initially and then they restrained the elderly who was in a fit of dementia. 3) The coping response of family caregivers varied. The coping reponses were (1) having their moods go up and down, (2) enduring and praying, (3) avoiding the elderly, (4) hoping for release from responsibility, (5) enduring their conditions(obligations), (6) accepting the elderly, (7) taking an objective view, and (8) taking safety measures. 4) The long ordeal of coping with a demented elderly person resulted in the (1) loss of physical and psychological well-being, (2) ethical conflicts, (3) family conflicts, (4) become desperate, (5) rejection of the aging process, (6) sympathy and understanding for the elderly, (7) awareness that the support system is important and (8) hope of sharing their responsibility. The results of this study may help nurses to understand the lived experiences of the family caregivers with the demented elderly better, in order to provide more basic data for the development of educational programs for dementia family caregivers. It may help to make the coping process easier and more successful for the family members of the demented elderly.

Nineteen children found to have gallstones at Seoul National University of Children's Hospital from Jun. 1982 to Jun. 1992 were reviewed. The results were as follows: 1) The mean age was 10 years, with 9 boys and 10girls. Most of the cases(63%) were over 10 years of age. 2) Clinical manifestations were abdominal pain(14 cases), nausea and/or vomiting (5 cases), fever (2 cases), jaundice (2 cases) and fatty food intolerance (1 case). Five cases were asymptomatic. Of 14 cases with abdominal pain, only 5 cases showed typical biliary colic which suggests cholelithiasis. 3) Associated conditions in cholelithiasis were Wilson disease (5 cases), congenital hereditary spherocytosis (2 cases), choledochal cyst (2 cases), total parenteral nutrition following ileal resection (1 case). And the other conditions which had association with cholelithiasis are not certain, and they were nephrotic syndrome, histiocytic cytophathic panniculitis, meningococcal meningitis, portal vein thrombosis and cardiac cirrhosis. The remaining 4 cases showed no associated conditions or diseases. 4) Eleven of 19 patients with cholelithiasis (58%) showed cholecystitis. 5) Cholecystectomy was performed in 4 cases with frequently recurred abdominal pain or associated diseases which needed operative correction. Symptoms completely subsided after operation. In conclusion, cholelithiasis in children has a variety of associated conditions and nonspecific abdominal symptoms were nore common than classic biliary colic. Although cholelithiasis in pediatric population are uncommon problems, the possibility of gallstones should be considered in the evaluation of abdominal pain in children with associated conditions.

Fifty five children with endoscopically proven esophageal varices from Jul. 1987 to Dec. 1992 were analyzed for endoscopic and clinical characteristics. The results were as follows: 1) According to etiological classification of portal hypertension in 55 cases, 35 cases showed intrahepatic causes 964%) and 20 cases had extrahepatic causes (36%). The most common clinical manifestation on admission was hematemesis (42%) and abdominal mass (36%). 2) Portal vein thrombosis was the most common cause of portal hypertension. Patients with portal vein thrombosis showed more frequent bleeding than with intrahepatic portal hypertension. Most of them had the first bleeding episode before 7 years old of age. Progression of portal hypertension in portal vein thrombosis was more rapid and bleeding episode was earlier than that in most of intrahepatic causes of the portal hypertension. 3) Twenty patients showed gastritis (10 cases), duodenitis (4 cases), gastric ulcer (4 cases) and esophagitis (2 cases) besides varix on the endoscopic examination. In 4 cases, the cause of upper GI bleeding was found as duodenal ulcer (3 cases) or hemorrhagic errosive gastritis (1 case) with no variceal bleeding. 4) Among 36 cases with variceal bleeding, 20 cases had predisposing factors for bleeding such as medication for URI including Aspirin. 5) More than one episode of bleeding were noted in 36 cases (65%) of the patients with varix. Thirty-one cases were treated conservatively with success. The remaining 5 cases with severe bleeding were managed with IV pitressin, SB-tube insertion and emergency shunt operation. High mortality rate (40%) was found in these patients group. In conclusion, in patients with clinically suspected portal hypertension, regular endoscopic examination might be recommended for the earlier recognition and effective prevention of variceal bleeding. for bleeding varices, in addition to conservative management, active treatment such as pitressin, SB-tube, sclerotherapy, endoscopic variceal ligation and emergency shunt operation should be considered with the emergency endoscopy which can identify bleeding focus and predict the chance of rebleeding.

Identification of virus is very importnant in aspects of preventive surveillance system of viral infection. The first isolation of echovirus serotype 25 from hand, foot and mouth disease was accomplished in Pusan, 1998. The authors determined some properties of this virus. Two cases of outbreak were confirmed from the stools of a 3 year-old girl and one 4 year-old boy suffering from hand, foot and mouth disease. Both occurred in April. The isolated viruses showed strong cytopathic effect on RD cells, also on HEp-2, and Vero cell lines after 3 days at 34'C, CO incubation. Isolated virus was identified as echovirus serotype 25 by neutralizing antibody test. Electron micrograph of negative-stained echovirus serotype 25 showed non-enveloped, isometric particle and about 30 nm in diameter.

Diagnosis of the disorders following menlscectomy is occasionally difficult, as ther has frequently been involved many causes other than the torn menisci. The retained fragments of the menisci have been blamed to be responsible for postoperative knee disorders. Arthroscopic examination was carried out in 40 knees after meniscectomy and the frequency of retained posterior horn in relation to the postoperative trouble was observed. 1. In 11 out of total 40 knees, the retained posterior horn could be diagnosed, consisting of 27.5% of all. 2. Only flve of these 11 knees showed some kinds of postoperative disorders,while the reat had no trouble. 3. Those knees in which the excision of the posterior horn was incomplete, were often associated with Irregular healing pattern of the local tissue, which might be closely related to the occurrence of the secondary Intraarticular change.
