1.An Experimental Study of the Effect of Kirschner-wire and Screw insertion upon the Epiphyseal Plate of the Distal Femur in the Growing Rabbit

The effect of surgical traumata upon the epiphyseal plate of the growing bone has been controversial among many authors. The resultant deformity subsequent to the epiphyseal plate injury-yields numerous different pattens and any of the explarations of the pathogenesis is also not accepted for all. Therefore, the author made an experiment to elucidate more clearly the histological changes of the epiphyseal plate of immature rabbits following insertion of Kirschner-wire and screw. Thirty rabbits weighing 700–1200gm were divided into 2 groups, as following snd the direction of longitudinal insertion from articular surface of the epiphysis of the distal femur. Group 1: lnsertion of Kirschner-wire Group 2: Insertion of screw The animals were sacrificed at 1, 2, 4, 8, and 10 weeks after the operative procedure and the histological examination of the injured epiphyseal plate of distal femur were made. The summary of the histological findings were follows: 1. The margin of the epiphyseal plate adjoining the Kirschner wire showed a new osseous tissue which formed a bony bridge running from the epiphysis to the metaphyseal bony trabeculae. 2. The margin of the damaged epiphyseal plate adjoining the screw revealed new bone formation and the amounts of the reactive new bone is less than Kirschner-wire. 3. Histological findings of the epiphyseal plate showed to be uniform and normal except in the adjacent area of the Kirschner-wire and screw.

2.A Study on 1 , 302 Cases of Skin Tumor.

A study was made on 1,302 cases of skin tumors which were obtained mainly from surgical pathology specimen during the period of 14 years from 1973 to 1986 at Department of Dermatology in Korea University Hospital. The results were summarized as follows : 1. Of the 1,302 cases of skin tumors, 1,143 cases(87.8%) were benigh tumor, 159 cases(12.2%) of malignant tumor and male to female ratio was 1.1: l. 2. Among the 1,143 cases of benign tumors, 301 cases of epidermal cyst, 183 cases of lipoma, 173 cases of pigmented nevi and 88 cases of hemangioma were noted. 3. Among the 159 cases of malignant tumors, 46 cases of squamous cell carcinoma, 27 cases of basal cell carcinoma, 26 cases of malignant melanoma, and 17 cases of metastatic carcinoma were observed. 4. The sites of predilection of the epidermal cysts were trunk (21.9%), face (20.9%), and neck (13.9%) and lipoma was trunk (48.6%). The sites of predilection of the suamous cell carcinomas were penis (11.1%), sclap (12.9%), face (18.5%), and leg (11.1%), basal cell carcinoma was face (55.6%) and malignant melanoma was sole (26.9%). 5. Among the 17 cases of cutaneous metastatic cancer, primary sites were breast (4 cases), pancreas (1 cases), lung (2 cases), stomach (1 cases) and unkown sites (9 cases).

3.Clinical Observation of Grenz Ray Therapy.

Supersoft roentgen rays called grenz rays, generated in a tube at low voltages and having wave lengths of 1 to 3 angstroms(H.V.L. 0. 01 to 0. 035mm. Al. ), are employed in the treatment of superficial derrnatoses patients (112 cases) in the Department of Dermatology, Cho Sun University hospital from March 1, 1972 to April 30, 1974. Results of these Series are as Follows: 1) Among the ll2 patients, including 28 patients of neurodermatitis, 14 patients of nummular eczema and. 10 patients of atopic dermatitis showed therapeutic success. 2) Grenz ray irradiations were remarkably effective for 6 cases of capillary hemangioma, 2 cases of basal celI epithelioma and 3 cases of verruca plana juvenilis, and less effective for psoriasis vulgaris, tinea pedis, acne vulgaris, nevus verrucosus and alopecia areata. 3) Complications included 21 cases of hyperpigmentation and burning sensations in neurodermatitis and nummular eczema.

4.A case of hypereosinophilic dermatitis.

Hypereosinophilir. dermatitis, which was first described in 1981, is a clinically distinet disease within the spectrum of the hypereosinophilic syndrome. It is characterized by a generalized polymorphous pruritic eruptions and blood eosinophilia with minimal or no extracutaneous involvement known cause hypereosinophilia, namely parasites, allergies and drug association are absent in these patients. We present a patient with hypereosinophilic dermatitis who did not show evidence of systemic disease. The patient was a 18-year old male who had pruritic erythematous brownish elevated plaque on both lower extremities for 5 months. A)though the patients skin lesions improved after treatment with steroids and dapsone, after discontinuing medication, hypereosinophilia recurred.

6.Cutaneous Resurfacing of the Pitted Acne Scars with the Erbium:YAG laser in 100 Patients.

BACKGROUND: Laser resurfacing of cutaneous scars, rhytides, and photodamaged skin has become very popular. Pulsed erbium:YAG laser resurfacing has recently come into favor for the treatment of pitted acne scars. OBJECTIVE: This study was designed to investigate the efficacy and safety of pulsed erbium:YAG laser in cutaneous resurfacing of pitted acne scars. METHODS: 100 patients with pitted acne scars were enrolled in this study. All patients were treated by means of a pulsed erbium:YAG laser with a 2 mm handpiece at the setting of 500 to 1000mJ/pulse and fluences used varied between 5-15J/cm2. Photographs of the face were obtained at baseline and 2 week postoperatively. Two weeks after treatment, postoperative care such as applying hydroquinone 4% and tretinoin 0.05% was recommended for 2 to 4 weeks. RESULTS: Out of 100 patients, 20% of patients showed an excellent response, 50% a good response, 21% a fair response and 9% a poor response. After 6 months, erythema had developed in two patients and and three patients had developed postinflammatory hyperpigmentation, hypopigmentation and delayed contact dermatitis. CONCLUSION: Treatment with pulsed erbium:YAG laser shows high efficacy and low morbidity in pitted acne scar resurfacing.

8.A Case of Eosinophilic Pustular Folliculitis Associated with Plantar Lesion.

We report a case of eosinophilic pustular folliculitis (EPF) with associated plantar lesions in a 45-year-old man. Exarnination of the skin disclosed an erythematous patch, approximstely 3 x 5cm, on the left cheek, which contained discrete and confluent pspulopustules and scaly circinated vesicular patch on the right foot. Histologic examination of a biopsy specirnen from the left cheek revealed the hair follicles and the sebaceous glands are infiltrated mainly by eosinophils and by some mononuclear and neutrophils. Histologic examination of a biopsy specimen from the right foot revealed a vesicle extending from the subcorneal portion to the lower epidermis, which was filled with neutrophils and a small number of eosinophils.

9.A Case of Leukemia Cutis in Myelodysplastic Syndrome Evolving into An Atypical Chronic Myeloid Leukemia.

We report a patient who had been initially diagnosed as a myelodysplastic syndrome in 1998 presenting purpuric patches on the left arm that started to develop about a year prior. The purpuric lesions were diagnosed as leukemia cutis by skin biopsy. Her subsequent bone marrow biopsy showed progression into an atypical chronic myeloid leukemia with increased numbers of leukocytes in the peripheral blood. Leukemia cutis typically is regarded as a sign of progression of disease or a manifestation of recurrent disease in treated patients with an established diagnosis of leukemia. We suggest that the skin lesion in this patient could have been a sign of con-version into atypical chronic myeloid leukemia.

10.A Study of Minimal Inhibitory Concentrations of Several Kind of Drugs against Pityrosporum orbiculare.

The bacteria as well as the fungi can build up resistance against different kinds of drugs or can decrease in sensitivity to these drugs. Therefore it is necessary that we use the MIC test for acertain period of time to determine the right concentration of drug to use in therapy. The culturing of P. orbiculare was successful in 1951. Up until that time, there were few drugs available to use in the MIC test. So, treatment for Tinea versicolor was dependent upon ones previous experience in determining the drug's effective concentration. Therefore, we performed the MIC test of clotrimazole, sodium thiosulfate, selenium sulfide, propylene glycol against P. orbiculare. The diagnos is for Tinea versicolor in the patient was confirmed by using KOH smear and by the clinical manifeststion. The fungi were then isolated in a culture from an olive oil rich media Mixture of isolated fungus and olive oil was applied to the lateral side of a healthy male's right upper extremity. After one week, P. orbiculare was confirmed by wood lamp examination, KOH smear and culture. After confirmation of P. orbiculare, a variety of concentrated drugs were used. Then media, containing propylene glycol, sodium thiosulfate, selenium sulfide and clotrimazole with a small amount of olive oil was used in inoculations of 10 viable unit/ml of P.orbiculare in attempt to estblish the MIC test. Test results are as follows: l) Propylene glycol MIC Range; 12. 5mg/ml~7. 5mg/ml. 2) Sodium thiosulfate MIC Range; 500mg/ml~7. 5mg/ml. 3) Selenium sulfide MIC Range; l. 25mg/ml~0. 04mg/ml. 4) Clotrimazole MIC Range; 0. 5mg/ml~ 0. 0005mg/ml.
