4.Clinical Evaluation of Complications of Interlocking Nail for Femoral Shaft Fracture

The intramedullary nailing is one of the most available method in treatment of femoral shaft fracture. Since the development of Kuntscher naukl in 1940, new design such as Grosse-Kempf IM nailing became widely used by more applicable level of femoral shaft fracture. Therefore, complications of intramedullary nailing was rarely seen. The authors analysed complications in 7 cases of intramedullary nailing for treatment of femoral shaft fracture in Koryo General Hospital from Jan. 1991 to Jan. 1993., and the results were as follows; 1. The average age was 40.5 years and all patient's sex were male. 2. Among 7 cases, complications were delayed union in 4 cases, metallic failure in 2 cases and femoral neck fracture in 1 case. 3. Among 4 cases of delayed union, 3 cases treated by weight-bearing after dynamization and 1 case treated by bone graft after dynamization. 4. Metallic failure occurred in 2 cases, 1 case developed metallic corrosion in medullary canal, the other case developed metal failure at fracture. 5. Fomoral neck fracture in 1 case was treated by multiple Knowles' pins.

5.The Treatment of intertrochanteric Fractures of the Femur using Compression Hip Screw

The intertrochanteric fracture of the femur occurs frequently in the elderly who has osteoporosis. These fractures had been treated with many fixation devices. Recently dynamic hip screws been commomly used. But, some complications were reported by this fixation the patients who were osteoporotic and/or who had unstable type fracture. We have analysed the roentgenograms and the medical records to study impaction degree and change of neck-shaft angle according to fracture type and degree of osteoporosis in the 35 patiensts wit minimum 1 year follow-up who have intertrochanteric fracture of the femur treated with dynamic hip screws, from Jhn. 1989 to Jun. 1993, retrospectively. The results obtained were as follows; 1. The most common fracture type by Bord-Griffin classification was type I, 19 cases(57%), and the most common osteoporotic by Sight's index was grade II, 21 cases(60%). 2. There were no significant differences of impaction degree and neck-shaft angle according to the fracture types or osteoporosis degrees. 3. The mean impaction degree of fracture site was 9.3mm and the mean neck-shaft angle change was 0.8。 of varus angulation.

6.5-Fluorouracil, Leucovorin, Ifosfamide and Cisplatin (FLIP) Combination Chemotherapy for Adevanced Non-Small Cell Lung Cancer.

PURPOSE: To evaluate the response rate and toxicity of combination chemotherapy including 5-fluorouracil (F), leucovorin (L), ifosfamide (I) and cisplatin (P) for the previously untreated patients with unresectable stage IIIB or IV non-small cell lung cancer. MATERIALS AND METHOD: The doses of FLIP were 5-fluorouracil 800 mg/m2 CI days 1-5, leucovorin 20 mg/m2 IV days 1-5, ifosfamide 1000 mg/m2 CI days 1-3, cisplatin 100 mg/m2 IV day 1 respectively. Cycles were repeated every 3 weeks until disease progression. Seventy-three previously untreated patients were enrolled. Age ranged from 30 to 73 (median 56 years); 43 were male, 30 female. Fifty-three patients had performance status (ECOG) 0-1 and 19 performance status 2. Twenty-two patients had stage IIIB and 51 stage IV. Follow-up ranged from 7+ to 160weeks (median 57 weeks). RESULTS: The overall response rate was 46.7% for 62 evaluable patients. (CR 1 patient, PR 28 patients) Median response duration was 24 weeks (range 1+ to 36+ weeks). Toxicity > Grade II (WHO) included: granulocytopenia 19.8%, anemia 13.5%, nausea and vomiting 31.5% stomatitis 46.5%, neuropathy 24.6%. CONCLUSION: FLIP chemotherapy was comparable to other combination chemotherapy for advanced non-small cell lung cancer with moderate toxicities.
