1.Periumbilical Perforating Pseudoxanthoma Elasticum.

Periumbilical perforating pseudoxanthoma elasticum (PPPXE) is a localized acquired disorder found most frequently in obese, multiparous, middle-aged women. It is characterized clinically by yellowish, lax, well-circumscribed, reticulated or cobblestoned patches or plaques in the periumbilical region. Multiparity, obesity, massive ascites, and abdominal surgery are thought to the initiating factors. There is controversy about the etiology of PPPXE. Some authors have classified it as a separate entity from hereditary pseudoxanthoma elasticum (PXE), while others speculate that this condition merely represents a variable expression of PXE in which systemic associations are likely. We report a case of periumbilical perforating pseudoxanthoma elasticum associated with a clinical PXE lesion on the anterior neck.

2.A Case of Hair Structure Abnormality Associated with Iron Deficiency Anaemia.

Iron deficiency has been discussed as an etiologic factor in diffuse alopecia. Although the mechanism of alopecia related to iron deficiency remains unknown, the diagnosis is confirmed if hair loss ceases with iron administration. However, hair structure abnormalities due to iron deficiency are less known. We report a case of hair structure abnormality associated with iron deficiency anaemia. The hair structure abnormality was a fusiform and expanded node of the hair shaft. The hair loss ceased and the abnormality was corrected with iron administration.

3.A Case of Porokeratotic Eccrine Ostial and Dermal Duct Nevus.

We report a case of porokeratotic eccrine ostial and dermal duct nevus in a 28-year old man. The skin lesions, present since birth, were multiple keratotic papules and punctate pits on the palms, soles and heels bilaterally. Histopathologically, the lesion was characterized by dilatation and hyperplasia of the eccrine sweat ducts with parakeratotic plugs related to sweat gland pores. This entity should be considered the differential diagnosis of other types of porokeratosis and comedo-like keratosis on palms and soles.

5.A Case of Multiple Xanthogranuloma in an Adult.

We report a case of multiple xanthogranuloma occurring ori the scalp in a 33 years old wornan. Multiple yellowish papules were developed on her scalp 3 months ago. Histopathologic findings showed massive granulomatous infiltrations of foamy histiocytes Touton giant cells in the dermis, and these cells were positive for lipid stain. This case was considered to be multiple xanthogranuloma in an adult, and treated by sur- gical excision.

8.A Study of Allergen Prick Test Reactivity and Peripheral Blood Eosinophil and Total Serum IgE Levels in Patients with Atopic Dermatitis and Urticaria.

Common environmental allergens play an important role in the pathogenesis of atopic dermstitis and urticaria via type I hypersensitivity reaction. The present study was undertaken to observe the prick test reactivity to common environmental allergens and peripheral blood eosinophil and total serum IgE levels in 20 patients of atopic dermatitis, 26 patients of chronic urticaria and 27 patients of artificial urticaria The results were as follows : 1, The psitive reactivities of allergens in patients with atopic dermatitis, chronie urticaria and artificial urticaria were 75.0%, 84.6% and 66.7%, respectively. 2. The positive reactivities of the inhalant allergens were more common than those of the food allergens, but statistically significant difference was found only in patients with chronic urticaria. 3. The most frequent allergen in prick test reactivity was D. farinae. 4. In patients with atopir, dermatitis, chrcnic urticaria and artificial urticaria, peripheral blood eosinophilias were encountered in 31.3%, 30.4%, and 25.0%, respectively. 9.Diagnostic Importance of Computed Tomography for the Gastric Carcinoma ;in Terms of the Staging System

The authors analysed the CT findings in 75 cases diagnosed as gastric carcinoma by U.G. I.S. or endoscopy in a 3)1, year period. The comparative analysis betw~en CT findings and surgical pathologic findings was done in 55 cases who underwent surgery. 20 cases could not be operated upon because of distant metastasis. The gastric carcinoma was devided into 4′ stages according to the staging system of the American joint Committee on Cancer. The CT findings were then compared and the following results noted : 1. The relationship with gastric wall thickening and penetratioil degree of tumori no thickening (Iesser than 1.0 cm) of 3 cases showed all of T1, 11 of localized thickening (1. 0.1 .5cm) showed 7 of T2, 38 of localized thickening (more than 1.5 cm) showed 37 of T3 and 3 of diffuse thickening showed all of T4a. 2. The accuracy of Iymph node detection by CT scan, 1) Regionaf tymph node showed 86% in operative f in ding and 73% in pathologic. 2) Extragastric Iymph node showed 87% in operative finding and 89% in pathologic. 3) Retro-peritoneallymph node showed 96% in operative finding and 95% in pathologic. ic. 3. In operated cases, there was one case of false positive distant metastasis and three false negative cases. 20 cases of non-operated cases showed distant metastasis such as liver, lung or kidney. 4. 55 cases were classified as stage 1 in 4 cases, 11 in 18, 111 in 23 and IV in 10 by pathologic findings, and 1 in 3, 11 in 24, 111 in 19 and IV in 9 by CT. The overall accuracy by CT stagi ng is 75%. 5. Among a total of 75 cases, there were 46 operable cases by CT finding an d 43 operabl e cases by pathologic staging. There we re 29 inoperable cases by CT finding and 27 cases by pathologic staging. Therefore, there was a 93% overall accuracy by CT stagi ng in terms of determination of operability. 6. The accuracy of adhesion into adjacent structure by CT finding was 69%. compared with operable finding.

10.The Effects of Ondansetron on the Analgesic and Side Effects of Intrathecal Morphine.

BACKGROUND: Intrathecal morphine provides good pain relief after anorectal surgery, but often associated with unpleasant side effects. Ondansetron, a selective 5-hydroxytryptamine3 (5-HT3) receptor antagonist, have been introduced for the prevention and treatment of emesis after chemotherapy in cancer patients and after general anesthesia. METHODS: Thus we studied the effect of ondansetron on the postoperative analgesic and side effects of spinal morphine in 60 patiens. The patients were given subarachnoid injection of 0.5% tetracaine 5 mg mixed with morphine 0.3 mg and positioned to jack-knife after fixation of anesthetic level. Either simple 5% dextrose solution 1000 ml or dextrose solution 1000 ml mixed with ondansetron 8 mg was injected intravenously in a rate of 100 ml/hr. The visual analog scale (VAS) of pain and incidence and severity of postoperative nausea, vomiting, pruritus and urinary retention were evaluated at 12 hour, 24 hour and 48 hour after injection of spinal morphine. RESULTS: The number of patients who became nauseated or vomited did not differ significantly between groups. Also, the VAS and the incidence and severity of other side effects such as pruritus and urinary retention did not differ significantly between groups. CONCLUSION: Ondansetron administered intravenously, did not prevent side effects of intrathecal morphine.
