1.Resection of Lateral Prominence in Malunited Calcaneus: Report of 7 Cases

Pain snd disability often fractures of the cslcaneus even though the original injury was skillfully treated. The causes are traumstic arthritis of the subtalar joint, abnormalities of the peroneal tendons or weakness of Gastrocnemius, etc. We have experienced the resection of lateral prominence of calcaneus in 7 patients who complained of pain by entrapment of peroneal tendons between lateral malleolus and lateral prominence of malunited calcaneus. The follow up time after operation was 3 years and 4 months to 12 months. In all patients the pain was markedly improved between postoperative 5 weeks and 10 weeks. When the last follow up, 6 patients among 7 patients except one case of subtalar arthritis were astisfaetory.

4.A clinical review of congenital gastrointestinal anomalies.

A clinical review was done on 343 infants and children diagnosed and operated at the Department of Pediatric Surgery in Keimyung University, Dong San Medical Center for congenital gastrointestinal anomalies from January, 1988 to December, 1991. The results are summarized as follows; The most prevalent age group of congenital gastrointestinal anomaly was within first week after birth, and infants within 3months occupied 70% of total, and male to female ratio was 2:1. 2) The moat common lesion of congenital gastrointestinal anomalies was stomach with 87cases (25.4%), followed by anus with 80 cases (23.3%), colon with 63 cases (18.4%) and biliary tract with 38 cases(11.1%). 3) The most common congenital anomaly was congenital hypertophic pyloric stenosis with 87 cases (25.4%), followed by imperforate anus with 70 cases (20.4%), congenital megacolon with 63 cases (18.4%), and prevalent age was 2 week-3month, first week after birth, 1month-3year. 4) The incidence of common congenital gastrointestinal anomalies were mostly higher in male than in female but choledochal cyst was higher in female than in male. 5) Associated anomalies were observed in 14 cases (4%) of total cases, duodenal atresia was seen the highest rate of the associated anomalies and the most common associated anomaly was annular pancreas, followed by Down syndrome, congenital diaphragmatic hernia.

8.Degranulation of human eosinophils induced by Paragonimus westermani-secreted protease.

Eosinophil degranulation is considered to be a key effector function for the killing of helminthic worms and tissue inflammation at worm-infected lesion sites. However, relatively little data are available with regard to eosinophil response after stimulation with worm-secreted products which contain a large quantity of cysteine proteases. In this study, we attempted to determine whether the degranulation of human eosinophils could be induced by the direct stimulation of the excretory-secretory products (ESP) of Paragonimus westermani, which causes pulmonary paragonimiasis in human beings. Incubation of eosinophils for 3 hr with Paragonimus-secreted products resulted in marked degranulation, as evidenced by the release of eosinophil-derived neurotoxin (EDN) in the culture supernatants. Moreover, superoxide anion was produced by eosinophils after stimulation of the ESP. The ESP-induced EDN release was found to be significantly inhibited when the ESP was pretreated with protease inhibitor cocktail or the cysteine protease inhibitor, E-64. These findings suggest that human eosinophils become degranulated in response to P. westermani-secreted proteases, which may contribute to in vivo tissue inflammation around the worms.

9.A Seroepidemiologic Study on Rubella Antibody Positivity: Primary School Students in Gyenggi-Do.

PURPOSE: The purpose of this seroepidmiologic study was to examine the immunization and infection history of rubella, positivity of rubella specific antibody, seroconversion rate of two rubella vaccines and to develop an adequate immunization program of rubella. Research Methods : Subjects of the study were 2.071 students of 8 priamary schools in Gyenggi-Do. Questionnaire and blood sampling were done. Test method of Rubella specific antibody was ELISA (Abbott IMx autoanalyzer). The anayltic methods for the study were chi-square-test, t-test and ANOVA. RESULTS: The results of the study were as follows : (1)The number of subjects who had infection history of rubella were only 28 students(1.5%). (2)The immunization rate of rubella was the highest in 6 year-old group(65%), it decrease as subjects grow older. (3)The positivity of rubella antibody was 76. 6%, it is the highest in 6 year old group and the lowest in 7 year-old group and it increase again as subjects grow older. (4)Average antibody titer was 53.3 IU(SD 79.2), the trend of antibody titer by age was similar to that of positivity. (5)The seroconversion rates of both vaccines, vaccine A,vaccine B, were 99.5% and 99.6%, the ratio of increase of antibody titer after immunization was higer than 400. it means that both vaccines are very efficacious. CONCLUSION: Results of the study suggest that it is necessary to increase the coverage of rubella immunization, and further studies are necessary to get more information for rubella.
