1.A Study on Clinical Effects and Serum Concentration of Digoxin.

From mechanocardiography and echocardiography, the systolic time intervals and the ejection phase indices were measured with determination of serum digoxin concentration(SDC) to elucidate the value of oral maintenance digoxin therapy on patients with heart failure in sinus rhythm. The drug interactions of digoxin with quinidine in heart failure, with verapamil in atrial fibrillation, and with aluminium hydroxide gel in healthy volunteers were observed with concomitant changes of SDCs. The results obtained are as follows. 1. After 10 days of treatment with digoxin 0.25 mg/day in 21 patients with heart failure there was a significant decrease in electromechanical systole(QS2), pre-ejection period(PEP) and PEP/left ventricular ejection time(LVET) ratio. There was also and equivocal decrease in LVET and an equivocal increase in mean velociy of circumferential fiber shortening(Vcf). However there was no significant change in ejection fraction(EF) and heart rate. The steadystate SDC was 1.20+/-0.12(S.E.M.)ng/ml. 2. Excellent correlation of the systolic time interval sand ejection phase indices measured from mechanocardiography and those determined from echocardiography was demonstrated. 3. SDCs were measured before and following quinidine therapy in 20 patients receiving maintenance digoxin for heart failure and who require quinidine for suppression of ventricular premature beats. Steady-state SDC following quinidine(Y) could be estimated form steady-state SDC before quinidine(X) as expressed by regression equation, Y=-0.394+2.309 X with correlation coeffcient, r=0.927(p<0.01). 4. In 12 patients with atrial fibrillation receving maintenance digoxin 0.25 mg/day, SDC before and following coadministration fo first 160 mg/day and later 240 mg/day of verapamil for 7days on each occasion was 0.85+/-0.07(S.E.M.) ng/ml, 1.00+/-0.09(S.E.M.)ng/ml and 1.33+/-0.13(S.E.M.)ng/ml, respectively. The difference of SDC between at control and under 240mg/day of verapamil was significant statistically(p<0.05). 5. Digoxin 0.75mg single-dose studies of bioavailability in 11 healthy volunteers showed a statistically significant difference(p<0.05) of the area under the 8-hour SDC curve between the digoxin only group and the digoxin plus aluminium hydroxide gel group. The area under the curve was 680+/-25(S.E.M.) min*ng/ml and 509+/-29(S.E.M.) min*ng/ml, respectively.

3.Diagnostic Value of an Electrocardiogram for Hyperkalemia.

BACKGROUND: Hyperkalemia is a common and potentially life-threatening metabolic disorder. The electrocardiogram(ECG) is known to be a relatively sensitive diagnostic tool hyperkalemia. However many exceptions, in which patients showed normal ECG findings even though hyperkalemic, have been reported. The purpose of this study was to determine the extent of correlation between the ECG findings and hyperkalemia and to determine when the ECG has value for diagnosing hyperkalemia. METHODS: Patients who had been diagnosed as having hyperkalemia at two university hospitals during three years were enrolled in this study. We reviewed the medical records of the patients and evaluated the following 6 ECG abnormalities: tall T waves, narrow T waves, QRS widening, atrioventricular block, loss of P waves, and sine waves. We defined tall T waves and narrow T waves as 20 percentiles of heights and widths of the T waves from the 100 patients with normokalemia. RESULTS: During the study period, there were 100 hyperkalemic patients, and we analyzed 69 available electrocardiograms. Abnormal ECG findings were revealed in 67% of 69 patients. The higher the serum potassium level, the more abnormal ECG findings. The common ECG abnormalities were tall T waves and loss of P waves. The patients with normal ECGs even though hyperkalemic had relatively low potassium levels. And whether chronic renal disease was not correlated to the ECG abnormality. CONCLUSION: The electrocardiogram is a good diagnostic tool for hyperkalemia if it is used with accurate diagnostic criteria. Thus, hyperkalemia should be considered when the ECG shows tall T waves or loss of P waves.

4.Changes of Splenocyte Proliferative Capacity and Subpopulation of Peripheral Lymphocytes Related to the Hemorrhage Amount in Rats.

BACKGROUND: Hemorrhage itself has been shown to produce abnormalities in immunity, particularly depression of the lymphocyte function. In order to better examine the amount of hemorrhage required to suppress the lymphocyte function, we determined the effect of graded fixed-volume hemorrhage on splenocyte proliferation and the lymphocyte subpopulation. METHODS: Male Sprague-Dawley rats(weight, 350~400g) were anesthetized, subjected to hemorrhages of 7.5ml/kg, 15ml/kg, and 22.5ml/kg by percutaneous cardiac puncture with 26G needles. After 1, 2, 4, and 7 days, animals were killed to obtain the blood and spleen. The splenocyte proliferative capacity was measured by using the tritiated thymidine incorporation technique, and the peripheral lymphocyte subpopulation was determined using flow cytometry with the following monoclonal antibodies: T cell(CD3+), T helper cell(CD4+), T cytotoxic cell(CD8+), and B cell(CD45RA+). RESULTS: Hemorrhage of 7.5ml/kg did not induce depression of splenocyte proliferation. However, for hemorrhage greater than 15ml/kg, the splenocyte proliferative capacity was significantly depressed at 2 days after hemorrhage and recovered at 4 days. Hemorrhage induced no changes in the relative percentage of lymphocyte subpopulations and in the number of each cell in peripheral blood. CONCLUSION: This study suggests that cellular immunity is depressed at 48 hrs after a hemorrhage greater than 15ml/kg without any change in the peripheral lymphocyte subpopulation.

6.A Clinical Study on Fracture of the Hand

The hand is a part of the human body, which has a complex structure, various functions and is one of the parts of the body most susceptible to trauma. With the increasing number of the traffic and occupational accidents proportionate to the rapld development of modern culture, fracture of the hand have become one of the commonest fracture of the human body. The object of treatment of fracture of the hand is to restore function the greatest degree possible with exact diagnosis and proper treatment based on the knowledge of anatomical structure and function. The authors have reviewed 228 cases of fractures of the hand (fracture with severe soft tissue injury were excluded) which were treated in the department of orthopedic surgery, Severance hospital from 1969 to 1978. The following results were obtained; 1. Fractures occured more commonly In man (80.3%) than woman. 2. Fractures occured more commonly in the second decade and the group between 10 and 30 years of age accounted for 73.7% of all fractures. 3. There was no significant differnnce between the left and right side. (105:123) 4. The commonest cause of the fracture was traffic accident (38.6%), machinary injury was the second (21.1%). 5. Most of the patients were brought to our hospital within 12 hours after injury (61.8%). 6. The most frequently fractured bone was the metacarpal (34.4%), then the proximal phalanx (28.1%), distal phalanx (16.9%), middle phalanx (10.5%) in decreasing frequency. 7. The average time for clinical union was 4.2 weeks in the case of the distal phalanx, 6.8 weeks in middle phalanx, 7.3 weeks in proximal phalanx and 6.5 weeks in metacarpal. 8. Complications were found in 8.3% of all cases and angulation deformlty was the most frequent.

7.A Clinical Study of Replantation of Amputated Limbs and Digits

In 1960 Jacobson and Suarez introduced the operating microscope to vascular surgery and demonstrated the superiority of small vessel repair using magnification. The first clinical replantation of an arm in the western world was performed by Malt in 1962, and Komatsu and Tamai reported the successful replantation of a completely amputated thumb in 1965. To perform a digit or hand replantation, microsurgical anastomosis of blood vessels is absolutely essential to revascularize the severed part. Without accurate repair of bone, nerves and tendons in addition to vascular anastomosis, however, it is impossible to gain functional success when replanting a digit or hand. This is a difficult task because all structures must be reconstructed at the same level and time. Even if the repair of some structures has been postponed, the secondary reconstruction may also be troublesome due to circular scarring at the replantation level. Since replantation surgery has been popularized throughout the world in recent years, the replantation sucess rate has increased and the surgeons interest in functional reconstruction of replanted digits or hands has been stimulated. The authors have seen and studied 6 cases of upper Jimb and 20 cases of finger replantation from May, 1980 through January, 1982 at the department of orthopedic surgery, Yonsei University College of Medicine. The results were as follows: I. The average age was 20 years and the male to female sex ratio was 4.5:1. 2, The causes of injury are detailed as follows: electric saw(6); cutting machine(5); roller(4}; presser(2); stone(2); others(3). 3. The level of amputation in 26 replantations was as follows: thumb, 3 cases; index finger, 4 cases; middle finger, 9 cases; ring finger, 4 cases; palm, I case, wrist, 3 cases; forearm, 1 case; upper arm, 1 case. 4. The maximum ischemic times for successtul results were 16 hours in limb replantation and 22 hours in finger replantation. 5. Six limbs, with four complete and two incomplete amputations, have been replanted and all six limbs survived: Twenty digits, with nine complete and eleven incomplete amputations, have also been replanted and 6.5 cases of nine complete amputations and 10 cases ot eleven incomplete amputations survived. A total of 22.5 cases(86.5%) of replanted limbs and digits survived. 6. The results of joint motion, two point discrimination, sensory recovery and status of sweating due to recovery ot sympathetic nervous function following replantation were satisfactory. 7. In the early stage of our series, postoperative systemic heparinization was used in some cases, but recently we have achieved good recults without it. 8. Main causes of reattachment failure were tissue crushing and secondary thrombosis of the anstomosed vessels.

9.Left Ventricular Diastolic Filling in Dilated Cardiomyopathy: Pulsed Doppler Echocardiographic Study.

Abnormal left ventricular diastolic properties have been reported in dilated cardiomyopathy (DC). Characteristics of transmitral flow were analysed in 37 patients with DC and 29 age matched normal subjects by pulsed Doppler echocardiography. Peak flow velocity of early diastole(PFVE, E), atrial systole (PFVA, A), E/A and deceleration rate of early diastolic flow (DEF) were measured from mitral Doppler spectrum. The extent of mitral regurgitation (MR) was determined by mapping method in the left atrium. Significant mitral regurgitation was founded in 27 out of 37 patients. Three distinct transmitral flow velocity patterns were demonstrated. Ten Patients without significant MR(27%, group 1), PFVE(58+/-17 cm/s), PFVA(73+/-17 cm/s) and E/A (0.94+/-0.4) were significant different from normal subjects (73+/-11 cm/s, 61+/-11 cm/s, 1.22+/-0.26, P<0.025, P<0.005, P<0.05, respectively). In contrast 17 patients with significant MR(46%, group 2) showed higher E (89+/-24 cm/s), lower A(52+/-19 cm/s), higher E/A (1.83+/-0.6) and DEF (596+/-149 cm/s2) than group 1 patients. Remained 10 cases (27%, group 3) had higher single peak flow (104+/-25 cm/s) with higher DEF and significant MR. In conclusion, abnormalities of left ventricular filling are detected in dilated cardiomyopathy without MR but not in DC with MR by Doppler echocardiography. The presence of MR, which augments early diastolic filling, may mask abnormal diastolic filling properties of DC.
