2.A Case of Kerion Celsi Caused by Trichophyton verrucosum and Its Epidemiologic Study.

Trichophyton(T.) verrucosum had not been reported until now in Korea. We report herein a case of kerion celsi caused by T. verrucosum considered the first case in Korea and its epidemiologic enviroment. She was a 10 years old girl living in cattle country. Three weeks ago, three small erythematous nodules were developed and rapidly progressed to be large boggy mass like tumefactions. Histopathologically, fungal hyphae and spores were found in hair follicles in H & E and PAS stain. In fungal study, characteristic features of T. verrucosum were found as chains of chlamidospores et als. With above results, we could diagnose these lesions as kerion celsi caused by T. verrucosum which were transmitted from a cattle. After 4 weeks-administration of ketoconazole, lesions were healed completely.

3.The Association between Blood Selenium Level and the Gastric Diseases.

To reveal the association between blood selenium level and the gastric diseases, 180 persons received the gastrofiberscopic examination at the outpatients department of the two university hospitals from July to September 1987, after the exclusion of the persons having the esophageal varix, were randomly selected. Their general characteristics such as age, sex and educational level and so on, were investigated. Five microliter venous blood was collected from each subjects and stored at 0 degrees C in heparinized vaccum tube. The blood selenium level was measured by the flameless atomic absorption spectrophotometry. In the procedure of data analysis, five subjects having benign tumor and anomaly of the stomach, were also excluded. The mean blood selenium levels of the 155.5 microgram/l among gastritis cases, the 154.8 microgram/l gastric ulcer and the 133.0 microgram/l gastric malignancy were significantly lower(p<0.05) than that of the 173.3 microgram/l among normal controls. In men the mean blood selenium levels among gastritis, gastric ulcer and gastric malignancy cases were significantly lower(p<0.05) than that among normal controls. In females, the mean blood selenium levels among gastritis and gastric maligancy cases were significantly lower(P<0.05) than that among normal controls(169.7 microgram/l), buy that among gastric ulcer cases(177.7 microgram/l) was not significantly higher. In the logistic analysis, coefficient of the blood selenium level was -0.0436(p<0.05 ; odds ratio 0.957) for gastritis, -0.0197(p=0.17 ; 0.981) for gastric ulcer, -0.4876(p<0.05 ; 0.614) for gastric malignancy and -0.0411(p<0.05 ; 0.960) for gastric diseases including the gastritis, the gastric ulcer and the gastric malignancy. These data support the hypothesis that the gastric diseases are to be associated with the low selenium level but, for the gastric ulcer, the further research is recommended.

4.Relationship between the blood flow patterns of left atrial appendage(LAA) with spontaneous contrast(SC) echogenecity and thrombi in LAA.

BACKGROUND: Patients with sinus rhythm, the left atrial appendage(LAA) appeared as a vigorously contracting structure and the blood flow patterns of LAA showed biphasic configuration. However, patients with AF rhythm the blood flow showed irregular or no configuration in doppler echocardiographic study. The purpose of this study is to evaluate relationship between the blood flow patterns of LAA with the presence of spontaneous contrast(SC) and thrombi in LAA. METHODS: We performed TEE and TTE simultaneously to 58 consecutive patients who had native mitral valve disease or mitral prosthesis. Spontaneous contrast(SC) and the presence and location of LAA thrombi were evaluated during TEE and left atrial dimension(LAD) & ejection fraction(EF) were evaluated during TTE. We divided the whole subjects into three groups according to the blood flow patterns ; regular pattern(Group 1), irregular pattern(Group 2), no flow pattern(Group 3). RESULTS: 1) Blood flow within the left atrial appendage is divided with three groups among of them, atrial fibrillation have two blood flow patterns. 2) Peak prositive & negative flow velocity within LAA in AF rhythm groups were slower than normal sinus rhythm group.(p<0.01) 3) Left atrial dimension in group 3 is significantly larger than others groups.(p<0.01) 4) Spontaneous contrast(SC) with LAA could be detected in 2(6.8%) of the 29 patients of group 1, 6(31.5%) of the 19 patients of group 2, 10(100%) of the 10 patients of group 3. Incidence of SC within LAA in group 3 is significantly higher than others groups. 5) LAA thrombi could be detected in 1(3.4%) of the 29 patients of group 1, 1(5.2%) of the 19 patients of group 2, 3(30%) of the 10 patients of group 3. Incidence of LAA thrombi is significangly higher than other groups. CONCLUSIONS: We can suggest that blood flow within LAA is divided into three groups according to the blood flow patterns, and spontaneous contrast and thrombi in LAA were closely related with the blood flow pattern.

5.Juvenile Hyaline Fibromatosis in an Adult.

Juvenile hyaline fibromatosis is a rare disorder probably inherited as an autosomal recessive trait. It is characterized by multiple slowly growing subcutaneous nodules, hypertrophy of gingiva, flexion contracture, and radiolucent bone destruction. The histological features of the tumor-like lesions are characterized by the deposition of amorphous hyaline material in which spindle shaped cells are embedded. We report a case of juvenile hyaline fibromatosis in a 26 year-old-woman. She had multiple subcutaneous nodules in scalp, ear, forearms, right knee, and back. Surgical excision of the tumors in the scalp and ear was done. The largest one measured 13 9 6 cm, and had homogeneous, grayish yellow cut surface with calcification. Light microscopic examination showed abundant eosinophilic hyaline material with extensive calcification and metaplastic bone formation. Spindle cells were rarely observed at the periphery of the tumor. Hyaline matrix was PAS positive, diastase resistant, and alcian blue negative. Scattered spindle cells were positive for vimentin but negative for S-100 protein and smooth muscle actin. There were many reports regarding early lesions of juvenile hyaline fibromatosis; however in this patient, tumor existed for more than 20 years and the histology was somewhat different from the early lesions reported in the literature.

7.Minimally Invasive Dynamic Hip Screw for stable Pertrochanteric Fracture.

Purpose: To report the surgical skills needed, and the clinical results of, minimally invasive hip surgery with dynamic hip screws and the comparison with the classical technique in stable pertrochanteric fractures of the femur. Materials and Methods: Thirty-two patients with pertrochanteric fractures of the femur who were treated with dynamic compression hip screws between April 1999 and March 2004, and were evaluated retrospectively, and were followed up for more than 12 months. 16 cases were treated with a classical dynamic hip screw technique and 16 cases with a minimally invasive technique in random order. The mean age was 73.2 years, and there were 11 males and 21 females. The operative times, total hemovac bleeding loss, functional scores, and average hemoglobin decrease ratios were evaluated. The clinical assessments were performed with the Harris Hip Score (HSS) and bony union was evaluated with serial follow-up plain radiographs. Results: The average operative time was 63.9 minutes with the classical technique and 42.5 minutes with the minimally invasive technique. The total average hemovac blood loss was 640 cc with the classical technique and 143 cc with the minimally invasive technique. Clinically, the HHS was an average of 85.9 and 89.2 in groups 1 and 2, respectively, by the last follow-up and there were no statistically significant differences between the two groups (P <0.05) The hemoglobin decrease ratio was the same in both groups and there was no nonunion. Conclusion: Minimally invasive dynamic hip screw insertion was useful in stable pertrochanteric fractures, because it decreased blood loss and operative times; and there were no changes in fracture healing and functional outcomes.

8.The Preference Survey for Long-Acting Injectable Antipsychotics of Community-Dwelling Patients with Schizophrenia and their Caregivers.

OBJECTIVE: The prescription rates of long-acting injectable (LAI) antipsychotics are very low around 1% in Korea. This study was aimed to explore the preference of LAIs in patients with schizophrenia, who are currently living in community, and their caregivers. METHODS: The patients, diagnosed with schizophrenia by DSM-IV TR and were registered in the 31 mental health centers of Gyeonggi province and their caregivers were inquired the knowledge of the LAIs. The questionnaires contained information such as demographic characteristics, history of psychiatric treatment, and so on. RESULTS: About 5,318 were registered in 31 community mental health centers of Gyeonggi province in February 2012. The questionnaires of 614 patients and 365 caregivers were gathered from 20 community mental health centers. The mean ages (+/-SD) of patients and caregivers were 41.9 (+/-10.2) and 62.2 (+/-13.4) years old, respectively. 272 patients (44.6%) had experienced the discontinuation of medications without doctor's consent. 217 patients (35.9%) and 97 caregivers (27.1%) knew about the LAIs. The preference rates for LAIs were 35.2% and 46.8% for the patients and caregivers, respectively. CONCLUSION: There is still huge discrepancy between the preference and the real prescriptions of LAIs in community-dwelling patients and their caregivers, much higher than in those of hospital settings. This study suggests that both patients and caregivers registered in the community mental health centers have a strong commitment to live in the community. The obstacles against the benefits of LAIs need to be resolved.

9.3-Day adolescent smoking cessation program in a tertiary level hospital.

BACKGROUND: Despite the fact that adolescent smokers are increasing in Korea, an effective smoking cessation program is not widely available. The purpose of the present study was to develop and evaluate a short term smoking cessation program. METHODS: The subjects involved in the smoking cessation program were 89 students from 9 different middle and high schools located in Seoul. They were referred to our program by their school teacher as a penalty for smoking in school. The duration of the program was a total of 270 minutes and the program was carried out in 3 sessions during a period of one week. The program included cigarette related experiments and life skills enhancement training, such as self-assertiveness, cigarette refusal skills and self esteem enhancement. The students considered to be smoke free at the end of the program were those who declared themselves to be smoke free and achieved an expiratory CO level of less than 8 ppm. The stage of smoking cessation was also assessed at the beginning to later evaluate if any progress in stage had occurred. Certification was given to those who succeeded in meeting the smoke free criteria. The students who were not able to meet the smoke free criteria were given the certification only under the condition that they achieve a level of expiratory CO measurement close th that of a nonsmoker's. RESULTS: Eighty seven students out of the 89 received the certification and 90.7% had attempted smoking cessation during the program. 39.1% Among, met the smoke free criteria used in this study. There was a significant difference between those who declared to be smoke free (54.1%) on the questionnaire and those who actually met the smoke free criteria. There was an average of a 0.75 stage advancement in terms of the stage of smoking cessation (P<0.005). CONCLUSIONS: The program yielded high smoking cessation attempt and cessation rate in spite of the short duration of the program.

10.Clinical Observations on 12 Children with Alport Syndreome.

Pilomatricoma is a well defind and characteristic entity histologically, but this tumor may be encountered during aspiration biopsy of subcutaneous mass, and is, occasionally, confused with other neoplasms, particularly squamous cell carcinoma, basal cell carcinoma, and epidermal inclusion cyst. Recently, we have experienced a case of pilomatricoma diagnosed by aspiration biosy. The smear of the aspirate revealed anucleated "ghost" squamous cells, basaloid or basophilic squamous cells and nucleated squamous cells, which lack nuclear features of malignacy. These findings are consistent with pilomatricoma.
