1.THE CHANGE IN TRANSVERSE STRENGTH OF DENTURE BASE AFTER RELINE PROCEDURE.

The purpose of this study was to evaluate the effect of proportional thickness of various reline materials on the transverse strength of denture base. The denture base resin used in this study was Vertex RS (Dentimex Zeist., Holland). The reline resins used were Tokuso rebase normal set (Tokuyama Corp., Japan), Rebaron (GC Corp., Japan), KoolinerTM(GC INC., U.S.A). New trulinerTM(Harry J. Bosworth Co., U.S.A). The bulk specimens with 2.5mm thickness of denture base were prepared as the control group. Group 1 was fabricated with 2.0mm thickness of denture base and 0.5mm reline material, group 2 with 1.5:1.0mm. group 3 with 1.0:1.5mm, group 4 with 0.5:2.0mm composition. Measurements of transverse strength were taken for each specimens. The results were as follows: 1. Regardless of the reline resin type, the transverse strength of denture base was decreased after reline procedure. 2. The transverse strength according to the reline resin type was decreased in the following order : Rebaron, Tokuso rebase, Kooliner, and then New truliner and there was a significant difference among the reline materials (P<0.05). 3. The strength of the relined denture base generally decreased as the proportional thickness of the denture reline material increased. These results suggest that increasing the proportional thickness of the reline material progressively decreased the strength of the relined denture base. Thus, the denture base should not be unnecessarily altered during the reline procedure.

2.AllogeneicLymphocyte Stimulating Capacity of Contact Sensitized Epidermal Cells in Mouse.

Allergic contact dermatitis is a prototype of delayed hypersensitivity reaction. Langerhans cells, keratinocytes and T lyrnphocytes play major roles in the pathogenesis of allergic contact dermatitis. We observed Langerhans cells and keratinocytes in contact sensitized epidermal sheets of mice and performed mixed epidermal cell lymphocytes reaction with normal epidermal cells and contact sensitized epidermal cells to determine the lymphocyte stimulating capacity of contact sensitized epidermal cell. We obtained following results : 1. The Langerhans cells were decreased in number and morphologically damaged in contact sensitized epidermis. 2. Ia antigen expression on keratinocytes was detected in almost all contact sensitized epidermal sheets. 3. The allogeneic lymphocyte stimulating capacity of contact sensitized epidermal cells was greater than that of normal epidermal cells. 4. The allogeneic lymphocyte stimulating capacity of contact sensitized epiderrnal cells was lost after treatment of epidermal cells with anti Ia antibody and complement. From these results, it is conceivable that the contact sensitized epidermal cells can amplify the immune reactions by stimulating the lynphocytes which are infiltrated in contact sensitized epidermis.

3.Phylogenetic Relationships of the Polyporaceae Based on Gene Sequences of Nuclear Small Subunit Ribosomal RNAs.

The Polyporaceae is a chaotic mass of genera having poroid hymenophores in the Aphyllophorales. To classify the Polyporaceae into more natural groups, phylogenetic analyses were performed using nuclear small subunit ribosomal DNA sequences. Thirty-six species from the families of the Polyporaceae, the Hymenochaetaceae, the Ganodermataceae, the Corticiaceae, the Bondarzewiaceae, the Meruliaceae, the Steccherinaceae and the Lentinaceae were phylogenetically compared. By performing maximum parsimony analysis, seven phylogenetically meaningful groups were identified and discussed. The hyphal system, presence or absence of clamps, and the type of rot were found as important characters in defining the groups. Each group was phylogenetically significant enough to be a core member of each family when the Polyporaceae was split into smaller and more natural families.

5.Two Cases of Pigmented Bowen's Disease.

Pigmented Bowen's disease (PBD) is a rare variant of Bowen's disease(BD). Most of the reported cases showed pigmented patches or thin plaques. Thus its clinical manifestations may simulated other various pigmented skin lesions. We experienced 2 cases of PBD in patients with multiple BD developed after taking Korean proprietary pills (KPP, "Hwan-Yak"), which were suspected to contain certain amount of arsenics. Both patients also showed arsenical keratosis on their palms and soles. The darker pigmentation of the PBL led us to differentiated them from melanoma.

6.A Case of Malignant Atrophic Papulosis.

A 15-year-old female patient had malignant atrophic papulosis(Degos' disease) that affected the skin and probably intestinal tract. She had multiple erythematous papules with porcelain-white atrophic centers on the trunk and extremities. Also she suffered from intermittent nausea, vomiting and abdominal pain. She received appendectomy for abdominal pain 2 months ago. Histopathologic studies showed atrophic epidermis and necrobiotic dermis. There were endothelial thickening, fibrinoid necrosis of bload vessels, perivascular lymphocytic infiltation in the lower dermis. She was treatd with acetyl salicylic acid and dipyridamole, but there was no improvement.

9.Predictive Value of Clinical Examination, Computed Tomography and Magnetic Resonance Imaging in the Clinical Staging of the Cervical Carcinoma.

A total of 162 patients with invasive cervical cancer was investigated retrospectively with a view to elucidate the efficacy of pretreatment staging procedures of application with intravenous pyelography, cystoscopy, rectosigmoidoscopy. All 72 intravenous pyelograms, 155 sigmoidoscopies, 158 cystoscopies were normal. Of 111 patients with stage Ib who had paraaortic lymph node biopsies, two patients had a positive node(0.02%). Of 51 patients with stage II who had paraaortic lymph node biopsies, five patients had positive paraaortic lymph node(10%). And the accuracy of computed tomography and magnetic resonance imaging in the evaluation of invasive cervical cancer was assessed. In evaluating stage of cervical cancer, clinical staging had an accuracy of 78.9%, compared with 65.7% for CT and 58.9% for MRI. And for detection of parametrial invasion, clincal staging had an accuracy of 85.9%, compared with 85.1% for CT and 78.9% for MRI. Both modalities were comparable in evaluating lymph node metastasis ( 76.1% for CT, 74.4% for MRI ). IVP, cystoscopy and sigmoidoscopy for staging procedure evaluated in this study are unnecessary and should no longer be performed in patients with early stage cervical cancer. In addition, paraaortic lymph node biopsies in the absence of clinically suspicious nodes are not warranted. Compared with CT and MRI, pelvic examination offered improved evaluation of cervical cancer staging, parametrial invasion, but CT and MRI was useful in detecting the pelvic lymph node metastasis which was nearly impossible by clinical evaluation.

10.Psychiatric Consultation for 5 Years in a University Hospital.

OBJECTIVE: This study aimed to investigate the pattern of psychiatric consultation-liaison in a new general hospital over a 5-year period. METHOD: Retrospective chart review was conducted for psychiatric consultations performed from May 2004 to December 2008 in a new general hospital in Hwasun, Korea. RESULTS: A total of 1,852 patients who were referred to the Department of Psychiatry for consultation were included in the analysis. The main reasons for psychiatric consultation were changes in mental status(20.5%), depression(16.8%), insomnia(12.8%), and anxiety(7.9%). Psychiatric consultations were conducted with diagnoses of delirium(39.7%), depressive disorder(28.2%), adjustment disorder(7.9%), and anxiety disorder(4.1%). Patients with delirium were significantly older than were those with other psychiatric diagnoses(p<0.001). Delirium was more common in male patients than in female patients(47.1% vs. 29.9%, respectively), and depression was more common in female patients than in male patients(48.3% vs. 33.9%, respectively)(p<0.001). Delirium was more common in patients who underwent surgical operation than those who did not(p=0.010). CONCLUSION: Delirium was the most common diagnosis for psychiatric consultation followed by depression in a university hospital. Delirium was more prevalent in men than in women, while depression was more common in women.
