2.Surgical treatment of hypothalamic hamartoma

Hypothalamic hamartomas are often associated with early onset gelastic seizures, precocious puberty,
behavioral problem and suboptimal response to antiepiletptic drugs. Until now, four surgical options
have been reported to reduce seizure by >50%. Surgical excision have good seizure outcome but
postoperative complications were not infrequent, whether by pterional or transcallosal interforniceal
approach. Radiosurgery is noninvasive alternative to resective surgery but the effect usually does not
appear until several months later. Radiofrequency ablation is less invasive than surgical resection and
its effect is immediate, but lacks long term follow-up data. It also requires three dimensional analysis of
the lesion to enhance effi cacy and safety. As hypothalamic hamartoma is intrinsically epileptogenic and
epileptogenic discharges spread from the lesion, blocking the seizure propagation through endoscopic
disconnection is regarded as an effective and safer option. Surgical choice for a particular patient
should take into account the hamartoma’s size, location, surgeon’s preferences, possible complication
as well as the effect and risk of the various surgical methods. In the present review, open surgery,
endoscopic disconnection, radiosurgery and radiofrequency ablation are discussed.

4.Expression of Green Fluorescent Protein in Both Spodoptera frugiperda Cells and Bombyx mori Larvae by Ac-Bm Hybrid Virus.

We have expressed GFP in Sf9 and Bm5 cells or Bombyx by larvae by using Ac-Bm hybrid virus capable of replicating in both Bm5 and Sf9 cells. Genomic DNA of Ac-Bm hybrid virus expressing P-galactosidase was cotransfected with baculovirus transfer vector containing GFP gene, pBacPAK-GFP in Sf9 cells. The Ac-Bm hybrid virus harboring GFP was named as Ac-Bm hybrid virus-GFP. The Ac-Bm hybrid virus-GFP-infected insect cells were easily selected by detecting the emission of GFP from each well of cell culture dish on the UV illuminator. GFP produced by Ac-Bm hybrid virus-GFP in Sf9 and Bm5 cells or B. mori larvae was confirmed by SDS-PAGE and Western blot analysis using GFP antibody. In addition, B. mori larvae infected with Ac-Bm hybrid virus-GFP was apparently appeared fluorescence from the whole body at 5 days postinoculation. The fluorescence of GFP from the hemolymph and fat body of B. mori larvae infected with Ac-Bm hybrid virus-GFP was also observed by fluorescence microscope. In conclusion, our results demonstrated that in baculovirus expression vector system, use of Ac-Bm hybrid virus have an additional advantage of expanded host range for producing recombinant proteins.

5.Correction of Dark Coloration of the Lower Eyelid Skin with Nanofat Grafting.

BACKGROUND: A number of factors can cause dark circles around the eyes including excessive pigmentation, thin and translucent lower eyelid skin overlying the orbicularis oculi muscle, and shadowing due to skin laxity and tear trough. Autologous fat graft is an effective method for the treatment of lower lid dark circles, but irregularities caused by leaving visible lumps of the fat can occur. Tonnard et al. suggested 'nanofat' grafting and introduced its characteristics and clinical applications. The authors used their nanofat grafting to correct lower eyelid dark circles. METHODS: Nanofat grafting was performed in 19 patients for dark lower eyelids. The grafts were injected into subdermal layer using blunt cannula. Microfat grafting with nasojugal fold was performed to all the patients. Among them, 18 patients received transconjunctival fat removal at the same time. RESULTS: All the patients showed much improvement from preoperative dark coloration. There were no visible lumps of fat, contour irregularities, or fat necrosis. Postoperative edema and ecchymosis were minimal. CONCLUSIONS: Nanofat grafting methods provide a good alternative for correcting dark circles by augmenting thin skin with lower complications. This simple, cost effective procedure is suitable for correction of dark circles and various skin rejuvenation purposes.

6.A Feasibility Study of an Exercise Intervention Based on Self-efficacy Theory among Overweight and Obese College Students in Korea.

PURPOSE: This study explored the feasibility of a 12 week self-efficacy based exercise intervention for overweight and obese college students. METHOD: A repeated measure, 4-group randomized controlled design was used. Students were recruited from two universities, in Seoul, Korea using: (1) flyers either on the campus bulletin boards or in campus restrooms; (2) advertisement on campus e-board; and (3) self-referral. Inclusion criteria were: (1) Korean college students aged 18 to 29; (2) BMI >23; (3) viscerally obese; and (4) accessible to a phone and a computer. Participants completed three self-reported questionnaires: socio-demographic questionnaire (at the baseline), Exercise Self-Efficacy Scale (at the baseline and 12th week), and Physical Fitness sub-subscale (at the baseline and 12th week). Additionally, physiological data (height, weight, blood pressure) were collected at the baseline and 12th week. Descriptive statistics and a two-way mixed ANOVA were performed using SPSS12.0. RESULTS: No significant group difference was observed. However, students with increased exercise self-efficacy during the program showed better physical fitness in the 12th week. When students' physical fitness was enhanced, the physiological factors were improved. CONCLUSIONS: Self-efficacy based exercise intervention may be applicable and acceptable to the college students. It is necessary to conduct a replicated study with a larger sample and an elongated intervention period.

7.A Case of Uterine Prolapse in Pregnancy.

Uterine prolapse with pregnancy is rare condition. The overall incidence is 1/10000- 15000 deliveries. The complications from uterine prolapse range from minor cervical ulceration and infection to fetal death or uterine rupture. The fetal mortality was as high as 22% mainly due to prematurity, respiratory infection. The management of this condition is focused on preventing late occurrence of prolapse during pregnancy and continued reduction. We present a case of uterine prolapse in pregnancy with a brief review of the literature.

8.Dovetail Cheiloplasty.

The widely prevailing Millard's rotation-advancement flap method is characterizes with the upper lip scar on a philtral column and that it is less conspicuous than LeMesurier's or Tennison's metheds. Nowadays, straight line closure methods are employed together with the Millard's. However there are still some problems which are a straight line closure that goes against the principle of plastic surgery, a noticeable scar, tenting of the peak of Cupid's bow, a short lip tendency, and depression of the lip when the muscle contracts. In this respect, we designed two or three small trapezoid skin flaps on the cleft side and the same number of releasing incision lines on the non-cleft side and then let them interdigitate one another. We called it dovetail cheiloplasty. The muscle work was done by suturing one third of the cleft side muscle to the dissected dermis of the non-cleft side skin flap just under the philtral dimple. Our patients had a primary incomplete, a microform type cleft lip or a secondary cleft lip deformity. The result of employing this method showed an inconspicuous scar, a shorter lip, and a natural formation of the philtrum. We believe this method induces the improvement of straight line closure with respect to the quality of scars and the morphology of an upper lip.

9.Correlation between Spontaneous Nystagmus and Vestibular Function Test Parameters according to Gender and Age in Vestibular Neuritis .

BACKGROUND AND OBJECTIVES: There are caloric test, SCEP, DP in Step velocity and VOR asymmetry in SHA test as parameters for evaluation in unilateral peripheral vestibulopathy in parameter of vestibular function test. The aim of this study is to assess the correlation between these parameters and intensity of spontaneous nystagmus in vestibular neuritis. MATERIALS AND METHOD: We evaluated 75 patients who were diagnosed as vestibular neuritis with 3 degree spontaneous nystagmus at vestibular function test lab from January 2000 to July 2005 in Dankook university hospital. The correlations between direction and intensity of spontaneous nystagmus and monothermal caloric test, SCEP and DP in step velocity, VOR asymmetry (0.01, 0.04, 0.16 Hz) in SHA test were analysed. And correlations according to sex, age were also analysed. RESULTS: The correlation coefficient between intensity of spontaneous nystagmus and monothermal caloric test was 0.60. The correlation coefficient between intensity of spontaneous nystagmus and SCEP was 0.35. The correlation coefficient between intensity of spontaneous nystagmus and Tc DP was 0.36 and it showed no significant correlation. The correlation coefficient between intensity of spontaneous nystagmus and VOR asymmetry (0.01, 0.04, 0.16) was 0.57, 0.46, 0.51, respectively and there was higher correlation in man, age under 60. CONCLUSION: Intensity of spontaneous nystagmus showed close relations to monothermal caloric test, VOR asymmetry of SHA, Tc DP, SCEP DP in sequence. In addition, young male patients tends to be closely related to intensity of spontaneous nystagmus.

10.Local Production of IgE in Nasal Polyp.

Among the several possible etiologic factors for the development of nasal polyp, localized nasal allergy had been insisted to be one of the major factors. This study aims to explore the existence of local production of IgE within nasal polyp, which can be the indirect evidence of localized nasal allergy. Fifty-two patients, who underwent nasal polypectomies between April 1993 and December 1995, were selected. The levels of total IgE and specific IgE of serum and polyp fluids were assessed. By using Donovan's equation, the percentage of local production of IgE in nasal polyp were calculated. Local production of total IgE was demonstrated in 18 cases of 28 polyp patient group (64.3%). Local production of specific IgE for Dermatophagoides pteronyssinus and Dermatophagoides farinae were demonstrated in 6 cases of 24 polyp patient group (25%). These results suggest the existence of local production of IgE in nasal polyp.
