1.A case of Eosinophilic Ascites noted in Eosinophilic Gastroenteritis.

The observation of eosinophilic ascites is uncommon. They can be noted in parasitic disease, malignant condition, vasculitis, idiopathic hypereosinophilic syndrome or allergic disorders including eosinophilic gastroenteritis, which is a rare disease of unknown etiology characterized by massive tissue infiltration of eosinophils in the layers of any area of gastrointestinal[GI] tract. Clinical manifestations are related to the level of the histologic infiltration in the wall, and the segment of the GI tract involved. Mucosal involvement may result in abdominal pain, nausea, vomiting, diarrhea and weight loss. Muscle layer involvements have obstructive symptoms. Subserosal eosinophilic infiltration may result in the development of eosinophilic ascites. We experienced a case of eosinophilic ascites as manifestation of eosinophilic gastroenteritis in a 43-year old man who also had jejunal obstruction. High proportion of eosinophil count was noted in the ascites, however peripheral blood eosinophilia was not noted. Parasitologic studies were negative. Histologic examination of segment of jejunum showed heavy transmural infiltration of eosinophils which were extended to subserosal layer. Eosinophilic ascites noted in eosinophilic gastroenteritis, though not a common disease entity, had not been described in the laboratory medicine related papers in Korea. Therefore we report this case as an example of eosinophilic ascites.

2.Squamous Cell Carcinoma and Struma Ovarii Arising in Benign Cystic Teratoma.

Malignant tumor is found in 1-2% of ovarian benign cystic teratomas. Among these malignant neoplasms, squamous cell carcinoma is by far the most common malignancy, whereas the incidence of struma ovarii is less than 5% in mature teratoma. As far as concerned the struma ovarii, a very small percentage is associated with carcinoid, mucinous or serous cystadenoma, or Brenner tumor. However, any reports of struma ovarii associated with squamous cell carcinoma in the same ovary could not be found in English literature. Recently we have experienced a case of squamous cell carcinoma and struma ovarii arising in an ovarian benign cystic teratoma in 72 year old female patient.

3.Unusual Morphology of Eosinophil Nucleus: Presented with Double Chromatin Filament Bridge: Report of Three Cases.

We found an unusual morphology of eosinophil nucleus having longer chromatin filament in addition to a single narrow chromatin bridge. The nucleus having two chromatin filament bridge looked like two legged eosinophil, instead of usual glasses shape. As the physiologic function of the nucleus of granulocyte segmentation and the mechanism by which the lobes are formed during differention is still unknown, we could not know the definite nature and significance of these double chromatin filament. However we could suggest that they may be a reactive change of eosinophilia. This not uncommon morphology has not been described as yet. Here we report three cases of unusual morphology of eosinophil nucleus presenting double chromatin filament bridge, one case with a band form nucleus looked like ring shape, with brief review of literatures.

4.Clinical Study on NeonataI Meningitis.

A clinical study was made on 68 cases of neonatal meningitis occuring under the age of 1 month at the department of Pediatrics at Severance Hospital, Yonsei University College of Medicine from 1st Jan. 1965 to 31th Dec. 1981. The sex ratio of male to female was 1.8:1 approximately. Neonatal predisposing factors significantly associated with neonatal meningitis were omphalitis(14 cases), skin infection (13 cases), birth injury (9 cases) and pneumonia (8 cases) etc. The most common maternal predisposing factor was difficult labor (13 cases). In 27 out of the 68 cultured CSF, the most common organisms were E. coli (29.6%), Staphylococcus coagulase(+) (22.2%) and Beta meholytic streptococcus (22.2%). Gram negative organisms were found in 12 cases (44.4%). The most common presenting symptoms were non-specific in nature -an elevated or subnormal body temperature, convulsion, poor feeding, irritability, jaundice and vomiting in that order of frequency. The presence of a poor Moro reflex, neck stiffness, unconsciousness or convulsion correlated with the high mortality rate significantly. Complications and sequelae included convulsion(11 cases), subdural effusion (8 cases), candida infection (8 cases), hydrocephalus (2 cases) and cerebral hemorrhage (2 cases) in that order of frequency. In the 68 cases, there were 29 mortalities or 42.6%.

5.Two Cases of Cri Du Chat(5p-) Syndrome.

We have experienced two cases of the Cri Du Chat Syndrome. The first case, 1 and 11/12 year old female, was admitted to evaluated profound mental retardation and failure to thrive. The other, 3 month old female, visit this dept for cat-like cry. They had the typical clinical characteristics of the Cri Du Chat Syndrome and showed showed the classical chromosome abnormality (46, XX 5p-), revealed by cytogenetic study.

6.Ginger and Its Pungent Constituents Non-Competitively Inhibit Serotonin Currents on Visceral Afferent Neurons.

Nausea and emesis are a major side effect and obstacle for chemotherapy in cancer patients. Employ of antiemetic drugs help to suppress chemotherapy-induced emesis in some patients but not all patients. Ginger, an herbal medicine, has been traditionally used to treat various kinds of diseases including gastrointestinal symptoms. Ginger is effective in alleviating nausea and emesis, particularly, for cytotoxic chemotherapy drug-induced emesis. Ginger-mediated antiemetic effect has been attributed to its pungent constituents-mediated inhibition of serotonin (5-HT) receptor activity but its cellular mechanism of action is still unclear. Emetogenic chemotherapy drugs increase 5-HT concentration and activate visceral vagal afferent nerve activity. Thus, 5-HT mediated vagal afferent activation is essential to provoke emesis during chemotherapy. In this experiment, water extract of ginger and its three major pungent constituent's effect on 5-HT-evoked responses were tested on acutely dispersed visceral afferent neurons with patch-clamp methods. The ginger extract has similar effects to antiemetic drug ondansetron by blocking 5-HT-evoked responses. Pungent constituents of the ginger, [6]-shogaol, [6]-gingerol, and zingerone inhibited 5-HT responses in a dose dependent manner. The order of inhibitory potency for these compounds were [6]-shogaol>[6]-gingerol>zingerone. Unlike well-known competitive 5-HT3 receptor antagonist ondansetron, all tested ginger constituents acted as non-competitive antagonist. Our results imply that ginger and its pungent constituents exert antiemetic effects by blocking 5-HT-induced emetic signal transmission in vagal afferent neurons.

7.Genetic populations of Bacillus anthracis isolates from Korea.

Bacillus (B.) anthracis is the pathogen that causes fatal anthrax. Strain-specific detection of this bacterium using molecular approaches has enhanced our knowledge of microbial population genetics. In the present study, we employed molecular approaches including multiple-locus variable-number tandem repeat analysis (MLVA) and canonical single-nucleotide polymorphism (canSNP) analysis to perform molecular typing of B. anthracis strains isolated in Korea. According to the MLVA, 17 B. anthracis isolates were classified into A3a, A3b, and B1 clusters. The canSNP analyses subdivided the B. anthracis isolates into two of the three previously recognized major lineages (A and B). B. anthracis isolates from Korea were found to belong to four canSNP sub-groups (B.Br.001/2, A.Br.005/006, A.Br.001/002, and A.Br.Ames). The A.Br.001/002 and A.Br.Ames sub-lineages are closely related genotypes frequently found in central Asia and most isolates were. On the other hand, B. anthracis CH isolates were analyzed that belonged to the B.Br.001/002 sub-group which found in southern Africa, Europe and California (USA). B.Br.001/002 genotype is new lineage of B. anthracis in Korea that was not found before. This discovery will be helpful for the creation of marker systems and might be the result of human activity through the development of agriculture and increased international trade in Korea.

Regulation for new standardized rescue record from of the 119 EMS system was established by law from February 1, 1998. We have recently evaluated the effectiveness of the record from. Records of 192 patients who visited the Emergency department of Sangggye Paik Hospital. Inje University by the 119 EMS system from February 1 to February 28, 1998 were analyzed statistically according to the contents of record form. In conclusion, the new standardized rescue record form of the 119 EMS system was effective in giving information of patients' history and analysing of the 119 EMS system. But, Detailed modification of the record form about disease entities is needed for proper initial evaluation of emergency patient.

9.A comparative analysis of patients with mesiodenses: a clinical and radiological study.

OBJECTIVES: A mesiodens appears most commonly as a supernumerary tooth impacted in the anterior maxilla. The purpose of this study is analyze mesiodens clinically. MATERIALS AND METHODS: Gender, crown form, direction of impaction, relation to permanent incisors, and chief complaints of patients with extracted mesiodens were analyzed. RESULTS: Patients were analyzed for motivation to visit the hospital; 85.4% of the patients were referred from other hospitals. Mesiodens was more common in males than in females (3.7:1), and 70.1% of patients had only one mesiodens, while 29.6% had two mesiodenses. Of the mesiodenses, 61.4% were of the aconical form, and the most common direction was upward (62.4%), followed by the normal position (26.0%) and the horizontal position (11.6%). The mesiodenses caused orthodontic problems with the permanent incisors in 46.3% of cases. Mesiodens associated with dentigerous cyst was rarely observed in our patient group. CONCLUSION: Mesiodens is more common in males than in females and often affects the permanent incisors. Thus, careful clinical and radiological evaluations of mesiodenses are important.

10.A Case of Primary Sjogren's Syndrome Presenting as Adie's Syndrome.

Sjogren's syndrome is a hererogenous autoimmune disease characterized by progressive destruction of the exocrine glands and accompanied by a variety of autoimmune phenomena. Sjogren's syndrome patients can develop symptoms of ocular and oral dryness as well as extraglandular complications including central and peripheral nervous system disease. Sometimes neuropathy precedes the diagnosis of Sjogren's syndrome. Adiets syndrome is characterized by tonic pupil and the absence of tendon reflex. Sweating abnormality and chronic peripheral polyneuropathy can also be present. We report a case of primary Sjogren's syndrome preceded by Adie's syndrome with peripheral neuropathy. A 26-year-old woman was admitted for photophobia and paresthesia. On examination, her pupils were anisocoric and did not react to light but constricted promptly to pilocarpin. Sensation decreased on her left side of body and deep' tendon reflexes were absent. Biopsy of minor salivary gland demonstrated infiltration by lymphocyte consistent with Sjogren's syndrome, but Schirmer test was negative. So she was diagnosed as Adie's syndrome with peripheral neuropathy. Five month later she complained of dry eye and dizziness. Rose bengal staining was positive. Sjogren's syndrome was diagnosed and she was discharged with local therapy for the sicca symptoms.
