The purpose of this study is to investigate the effect of the ball-cone contact interface variation on the ceramic ball stress which is the dominant factor of the design of the moldular artificial hip joint. A contact stress analysis problem is formulated using the optimization principle and finite element analysis technique on the ball-cone system of three dimensional axi-symmetrical model, Numerical procedure is employed to get the solution for varying angles of cones and angular mismatches between the ball and cone interfaces. There is a fair amount of friction in the interface and included in the analysis model.

3.The palato-gingival groove - anatomical anomaly occurred in maxillary lateral incisors: case reports.

This report describes clinical cases of a palato-gingival groove on a maxillary lateral incisor with associated localized periodontal disease and pulp necrosis. The tooth of the first case was extracted because of severe bone destruction. The palato-gingival groove of the second case was eliminated using a round bur, and the resulting defect was filled with synthetic graft and covered by an absorbable membrane. Both diagnosis and treatment of palato-gingival groove were very difficult and usually extraction of the involved tooth is the treatment of choice, but combined endodontic-periodontic treatment allowed the tooth to be saved.

5.The changes of Corneal Endothelial Morphology after Phacoemulsification by using Healon GV of Viscoat.

Corneal endothelial cell damage from phacoemulsification has been attributed to mechanical injury caused by surgical manipulation and oxygen free radicals produced by ultrasound. We evaluated role of protective effect in Healon GV and Viscoat on corneal endothelial cell damage during phacoemulsificatin Seventy five eyes underwent phacoemulsigication hrough scleral tunnel incision with posterior chamber lens implantation.: Healon GV, 32 eyes and Viscoat, 43 eyes. We analyzed the corneal endothelial morphology using non-contact specualr microscope and analysis program.The percent loss of corneal endothelial density at 3 months postoperative period was greater in Viscoat(5.87%) than in Healon GV(2.47%),although it was not statistically significant(p>0.1). Coefficient of variation in cell size and hexagonality of the corneal endothelial cells also showed no significant difference between the two groups(p>0.1). In conclusion, Healon GV and Viscoat have similar protective effect on the corneal endothelial cell damage induced by ultrasound during phacoemulsification.

6.Assessment of the Reproducibility of Corneal Endothelial Cell Analysis with Non-Contact Specular Nicroscope.

The noncontact autofocus specular microscope with incorporated semiautomated image analyzing program provide a rapid morphometric endothelial analysis. We evaluated the reproducibility of endothelial cell analysis with Konan noncon Robo-ca sp 8000 noncontact specular microscope. Specular microscopic examinations were performed three times each in twenty eyes by two examinations were performed three times one examiner. The difference of mean coefficient of variation of cell density evaluated by two examiners were not statistically significant(4.83% and 3.81, p=0.16). But, mean coefficient of variation of CV(coefficient variation of cell size, poluyimegathim) and hexagonality(pleomorphism) were statistically significantly different between two examiners(12.46%, 17.90%, p=0.04 and 12.30%, 8.40%, p=0.01, respectively). Repeatability evaluated by one examiner in two eyes showed small coefficient of variation of cell density(3.67% & 3.69%) and large coefficient of variation of CV(11.41% & 15.00%) and hexagonality(7.79% & 10.24%). In conclusion, our study has shown that Konan noncon Robo-ca sp 8000 noncontact specular microscope allows accurate estimation of endothelial cell density, But reproducibility of coefficient of variation of cell size and hexagonality analysis were lower than that of cell density analysis.

8.Aging Effect on Intraocular Pressure and Associated Factors.

Background : There is a substantial body of evidence that intraocular pressure increase with age among western population. However, Japanese study showed that intraocular pressure was inversely related with age. So we investigated whether intraocular pressure is related with age for Koreans. Methods : Between March, 1996 and February, 1997, 6201 subjects underwent comprehensive health examinationinthe Kyung Hee medical screening center in Seoul, Korea. The intraocular pressure was measured with pneumatonograpyh. At the same time blood pressure, body weight, height, serum osmolarity, fasting blood glucose, T3, T4, TSH(thyroid stimulating hormone), hematocrit measurements were performed. We selected 250 case randomly and investigated whether the distribution of C/D ration(cup/disk ratio), smoking status, alcohol drinking amount are uniform by aged group. So we measured C/D ratio by fundusphotograph and investigated smoking and drinking habits by questionnaires. Results : In the randomly sampled 250 cases, the amounts of smoking and alcohol consumption were lower in the elderly group. But, the C/D ration didn't show any difference between young and elderly group. In a multivariable analysis, both right and left intraocular pressure showed significantly inverse correlations with aged and visual acuity. Male had significantly higher intraocular pressure than female. Both right and left intraocular pressure showed significant positive correlations with body mass index, fasting blood glucose, hematocrit and systolic blood pressure. Diastolic blood pressure, serum osmolarity, T3 and T4 were not associated with intraocular pressure. TSH had significant positive correlation with right intraocular pressure, but no correlation with left intraocular pressure. Conclusion : Age was independently and inversely related with intraocular pressure for Koreans.

9.Effect of Sclerotherapy after Percutaneous Aspiration of the Simple Renal Cyst.

PURPOSE: Recently, percutaneous aspiration with sclerotherapy has become widely used as a treatment of the symptomatic, large, and simple renal cysts, due to its minimal invasiveness and high effectiveness. However, the reported recurrence rates of simple aspiration are high, in the range 30 to 70%. Therefore, the effect of percutaneous aspiration with sclerotherapy of simple renal cysts was analyzed. MATERIALS AND METHODS: The results of the treatment in 35 simple renal cysts, between June 1994 and July 2003, were evaluated. The symptoms associated with simple renal cysts were; none (40%), flank pain (31.4%), indigestion (17.1%), hematuria (8.6%) and a palpable mass (2.9%). The simple renal cysts were treated with percutaneous aspiration in only 7 cases (group I), single percutaneous aspiration with sclerotherapy, using 99% ethanol, in 18 (group II) and a repeated percutaneous aspiration with sclerotherapy, using 99% ethanol, in 10 (group III). All patient were followed up by ultrasound or CT scan for 12 to 80 months (mean 28.1 months). RESULTS: Complete and partial collapses and recurrences of the renal cysts occurred in 1 (14.2%), 3 (42.9%) and 3 (42.9%) in group I, 7 (39%), 10 (55.5%) and 1 (5.5%) in group II, and 6 (60%), 3 (30%) and 1 (10%) in group III. The overall efficacies were 92% in all 3 groups. Furthermore, all recurrences occurred within the first year of the follow up period in all groups. CONCLUSIONS: For a simple renal cyst, percutaneous aspiration with sclerotherapy, using 99% ethanol, either singly or repeatedly, appears to be effective in the prevention of re-accumulation of cystic fluid; it also proved to have a positive long term result in terms of a relapse of a simple renal cyst.

10.Retroprosthetic Membrane Formation after Implantation of Experimental Keratoprothesis in Rabbit.

The studies about factors influencing the retroprostheic membrane formation, one of major complications after keratoprosthesis implantation, and about histological characteristics of the retroprosthetic membrane were performed. The methods to inhibit formation and proliferation of membrane were also tried. Formation of the membrane according to corneal trephination size and the effects of lens removal to membrane formation were studied. The influence by the design of keratoprosthesis was also studied. Corticosteroid and SIMP (synthetic inhibitor of metalloproteinase) were applied topically to see the inhibitory effects of drugs to membrane. To evaluate the postoperative intraocular inflammation. Interleukin-1beta was assessed in the aqueous humor at postoperative 3 weeks. Regardless of trephination size, all trephination sites were replaced with opaque fibrous membranes. Histologically, in small trephination group (under 6mm) the membrane was regeneration of cornea showing corneal stromal fibroblast migration and proliferation and mature collagen. In large trephination group (over 7mm), the periphery of the membrane was compatible with corneal regeneration, but central portion was filled with granulation tissue. The removal of lens didn`t influenced the formation of membrane formation. In disc type keratoprosthesis implantation group, all the implants were extruded within 1 month due to retroprosthetic membrane formation. In cylinder type keratoprosthesis implantation group, the physical barrier of cylinder stopped the retroprosthetic membrane proliferation and spared the posterior surface of optic portion. The implants were not extruded for average 10 weeks. Corticosteroid and SIMP showed the effects of decreasing mature collagen formation in the membrane. The level of Interleukin-1beta at postoperative 3 weeks showed no difference according to different drugs and the highest level was checked in cylinder type keratoprosthesis implantation group.
