1.Studies on Restoration of Functional and Morphological Damage of the Kidney Induced by Ureteral Ligation Histopathological Observation on Experimental Hydronephrosis.

Experimental hydronephrosis was induced in rabbit by ureteral ligation, then deligation followed after certain experimental period as designed. Histopathological changes of the kidney and restoration of hydronephrosis were studied and the following results were obtained: 1. Ligation of the ureter for 3 days produced mild to moderate degrees of tubular dilatation. irregular proliferation and dilatation of small arterioles. Deligation initiates regeneration of the tubules at 1 week and almost normal histology returns by 3 weeks. 2. Ligation of the ureter for 5,7 and 14 days causes marked tubular dilatation, atrophy of the .cortex, and partial obliteration of the glomerulus. By 3 weeks after deligation, the tubules seem to partially regenerate but no significant histological changes were observed. 3. Ligation of the ureter for 21 to 60 days causes formation of scar, marked obliteration of the glomerulus and compensatory hypertrophy of the remaining glomerulus. No morphological restoration was observed by 3 weeks after deligation. 4. It seems that histological restoration starts at periglomerular tubules located closer to the cortex toward the medulla.

2.The effects of compound madecassol on the wound healing.

This study was designed to assess the effects of compound madecassol(madecassic acid with neomycin sulfate and hydrocortisone acetate) on the wound healing. Madecassol is a titrated extract of centella asiatica and the clinical effects of madecassol are stated to the enhancement of wound healing and the prevention and relief of excessive scar formation. Compound madecassol is composed of madecassol and neomycin sulfate and hydrocortisone acetate. Neomycin sulfate has antibiotic effect and hydrocortisone acetate has antiinflammatory and antiallergic effects. So, compound madecassol has a synergistic effect of madecassol as well as neomycin sulfate and hydrocortisone acetate. Using 54 rats, we compared the effect of compound madecassol on wound healing at 3rd, 7th, 14th, and 21th postoperative days. This study examined the histologic findings and the gross findings which were wound size, epithelization and quality of granulation tissue. In this study, compound madecassol showed lower degree of inflammatory infiltration, shorter inflammatory phases and less wound contraction. The number of the myofibroblast in the group of compound madecassol were fewer than other groups. Granulation tissue of the compound madecassol was relatively healthier than others. There were no significant difference of re-epithelization between compound madecassol and other groups. In conclusion, compound madecassol can reduce excessive wound contraction and promotes wound healing process.

4.6 cases of trichostasis spinulosa.

Trichost.asis Spinulosa consists of several fasciles of reta ned vellus hairs in the hyperkeratotic sebaceous follicles, resulting in a dark slightly raied horny follicular spines, resembling comedones on the trunk and face. The condition is relatively common, particulary among the age, but is usually overlooked on routine skin examination as the patient is rarely aware of it.. We report 6 cases of trichostasis spinulosa, which showed numerous asymptomatic tiny black comedo-like dots on their backs. We obtained the follicular material by tweezer, which consised of several vellus hairs surrounded by a keratinous plug. Histologic examination was done in 3 patients, one of whom showed a rarely reproted double follicle and double papilla. With scanning electron microscopy, we observed multiple vellu hairs embedded in a keratotic plug, emerging from a hair follicle opening.

5.Surgical treatment of Quadrant Hemivertebra in Congenital Kyphoscoliosis.

STUDY DESIGN: Sixteen patients with congenital kyphoscoliosis were evaluated the clinical and radiological results according to the treatment method. OBJECTIVES: To provide a guideline for surgical treatment of congenital kyphoscoliosis SUMMARY OF LITERATURE REVIEW: Congenital kyphoscoliosis is an abnormal coronal and sagittal curvature of spine that is caused by the presence of vertebral anomalies, which causes a significant deformity and disability. Correction of the deformity is challenging and accompanied by high risk of complications. Many methods of operative treatment were recommended with various results. MATERIALS & METHODS: We reviewed 16 patients of congenital kyphoscoliosis who were operated from 1984 to 1997, and follow-up periods were over 12 months in all. The average age of surgery was 12 years (ranged from 4 to 20 years). Nine cases were operated by excision of quadrant hemivertebra through combined anterior & posterior approach and 6 cases were treated only by posterior instrumentation and one underwent posterior on bloc spondylectomy. RESULTS: The type of curves were thoracic in 5, thoracolumbar in 9, lumbar area in 2 patients. Average angle of kyphosis was 55 degree preoperatively, 25 degree postoperatively, and 39 degree at the follow up study. Average angle of scoliosis was 48 degree, 22 degree, and 27 degree, respectively. In 9 cases corrected by excision of quadrant hemivertebra; kyphotic angle was changed from 62degrees preoperatively, to 22degrees postoperatively and to 36 degree at the end of follow up study, while scoliotic angle was changed from 47degrees, to 12degrees and to 21degrees, respectively. In 6 cases corrected by posterior instrumentation kyphotic angle was changed from 52degrees preoperatively, to 33degrees postoperatively and to 48degrees degree at the follow up study, while scoliotic angle was changed from 53degrees, to 33degrees and to 37degrees, respectively. CONCLUSION: Excision of quadrant hemivertebra through combined anterior and posterior approach or posterior instrumentation were resulted in significant loss of correction during the remaining growth period due to the secondary change of adjacent vertebrae. So, to obtain better correction and to reduce the recurrence of deformity, more aggressive approach including the concept of three dimentional osteotomy involving the upper and lower adjacent curve may be considered.

6.A case of Traumatic Asphyxia Associated with Ophthalmic Manifestation.

PURPOSE: Traumatic asphyxia is a rare clinical syndrome characterized by cervicofacial cyanosis, edema, and multiple petechiae after a severe chest crush injury, and ophthalmic involvement includes violaceous discoloration of lid, lid edema, bulbar subconjunctival hemorrhage, exophthalmos and retinal hemorrhage. The authors experienced a case of traumatic asphyxia with typical ophthalmic manifestations after a crush chest injury, and we report this case with literature review. METHODS: A-54-year old man was consulted for bilateral proptosis after a crush chest injury. Ophthal-mologice valuation and intervention were performed. RESULT: Right eye was not checked due to previous corneal opacity, but visual acuity was 0.8 and IOP was 45 mmHg in left eye. There were bilateral proptosis and severe bulbar subconjunctival hemorrhage, and orbital computed tomography showed orbit fat interposed between globe and medial orbital wall. IOP was controlled with medications and other clinical signs including proptosis showed progressive improvement.

7.Ki-I Lymphoma In a Young Adult.

Ki-1 lymphoma is a rare, large cell anaplastic non Hodgkin's lymphoma. It expresses the CD30 antigen and is recognized by the antibodies Ki-1. This Ki-1 positive anaplastic lymphoma was first described in 1985 as a new histological subtype and was added to the updated Kiel classification in 1988. Morphological and immunological features of this lymphoma have been well described, but clinical studies have been limited and follow up has been short. The authors report a case of Ki-1 lymphoma of the skin without systemic involvement in a young adult based on clinical, pathological and immunological features.

8.Relation between the Efficacy of Amiodarone on Chronic Atrial Fibrillation and Left Atrial Size and Fibrillatory Wave Form.

To assess the efficacy of amiodarone on chronic atral fibrillation(AF) and to evaluate the relation between the ability to convert AF to sinus rhythm (SR) with amiodarone therapy and left atrial(LA) size and atrial fibrillatory wave forms, 22 patients with AF, aged 40 to 60 years(mean 47.5 years), were studied. Nine patients(40.9%) had mitral valvaular heart disease, 6(27.3%) hypertension, 5(18.2%) lone AF and 2 (9.1%) cadiomyopathy. Amiodarone therapy with either 600mg for 1 week, 200mg for 4 weeks in 5 consecutive patients, or 800mg for 1 week, 400mg for 4 weeks and 200mg for 6 weeks in 17 patients, converted AF to SR in 9(40.9%) patients 3 to 6 weeks after amiodarone was started on. In either group, patients who achieved conversion had smaller LA size(mean 43.7mm) than those who failed conversion(mean50.2mm)(P<0.05). Those who had LA size less than 45mm achieved conversion of AF to SR in 70%, comparing to 16.7% in patients with LA size more than 46mm(P<0.05). Among patients who achieved conversion, LA size was less than 46mm in 77.8% comparing to 23.1% in patients who failed conversion on Amiodarone. Those with coarse AF(46.2mm), althogh the difference was not significant statistically. There was no converstion in patients with LA size greater then 58mm and in patients with coarse AF who concomittantly had MVD. These findings suggest that the efficacy of amiodarone was related to LA size, and to the atrial fibrillation wave form in patients with mitral valvular heart disease.

10.A study on profile change and stability of treatment after wearing face mask.

Skeletal Class III malocclusions are growth-related discrepancies, and the problems are more severe until growth is complete. Causes of skeletal Class III malocclusion are classified into mandibular overgrowth, maxillary deficiency, and combination of the two. Face mask has been recommended for treatment of Class M malocclusion with maxillary deficiency in the early time of growth. Numerous experiments were performed and clinical studies have been reported on face mask ; nevertheless, studies on profile changes and stability after treatment of face mask are considered to be somewhat insufficient. The author selected 50 patients who can be checked for follow-up. They had been diagnosed as skeletal Class III malocclusion with maxillary deficiency and then treated with face mask ; the sample group was divided according to sex, treatment beginning age, palatal suture opening(intraoral appliance). For each group, changing pattern of facial profile and stability of treatment observed, and comparison with 20 Korean normal children(Angle's Class I ). The following results were obtained. 1. Skeletal, dental, and soft tissue measurements indicated more changes in the amounts of maxillary forward movement during face mask treatment. 2. R.P.E. group showed more significant maxillofacial changes and La-Li group showed more dental changes. 3. Growth changes of maxilla induced in the treatment group during wearing face mask were much more than those of normal group. 4. Growth changes of maxilla in the treatment group after treatment of face mask were less than those of normal group. From the obtained data, it can be concluded that. there was a stimulative effect on forward growth of maxilla during the use of face mask however, on removal of face mask, the stimulative effect was eliminated and undergrowth tendency of maxilla resumed.
