1.Overexpression of the E1193-283 find E2384-649 Proteins of Hepatitis C Virus in GST Fusion Forms in E. coli and Their Immunogenicity.

The truncated E1192-283 and E2384-649 genes of hepatitis C virus (HCV) linked to the gene for glutathione 5-transferase (GST) were constructed and their expressions were analyzed. The GST-E1192-283 fusion gene overexpressed the fusion protein in E. coli as a soluble form, while the GST-E1192-383 plasmid did not express expected fusion protein. The purified GST-E1192-283 fusion protein was efficiently cleaved by thrombin. More than 90% pure, HCV E1192-283 protein was obtained by GST-agarose chromatography. The truncated GST-E2384-649 fusion gene expressed the fusion protein mainly as an insoluble form, whereas the GST-E2384-740 did not express the fusion protein. The truncated GST-E1 182-283 and GST-E2384-649 fusion proteins reacted specifically with an HCV patient serum. In addition, mice immunized with either the purified E1192-283 or GST-E2384-649 proteins generated specific antibodies to each antigen. The results suggested that hydrophobic carboxyl portions of the E1 and E2 proteins might affect expression levels as well as the solubility of each fusion protein in bacteria. Also, the truncated E1 protein with Tyr-192 to Ser-283 contained antigenic epitope(s) which could be specifically recognized by an HCV patient serum.

2.A Case of Metastatic Epithelioid Sarcoma.

Epithelioid sarcoma(ES) is a very rare, unique soft tissue sarcoma, which was first fully described by Enzinger in 1970. The tumor chiefly occurs in young adults and affects the soft tissue of the distal extremity. A 46-year-old male patient had slowly growing masses on his left plantar surface, which were diagnosed as ES and mass excision and radiation therapy were done. Eight months later, about 1 × 1 cm sized, well defined multiple erythematous nodules on his scalp were discovered. Although he was treated with chemotherapy, no improvement was gained.

There are several EKG changes in cerebrovascular disease(CVD). The wide prominent of inverted T wave is frequently developed in patients with CVD. There were case reports for CVD in patient with ST-segment elevation without myocardial infarction, but the exact mechanism is unknown. EKG abnormalities associated with subarachnoid hemorrhage were first described by Byer et al, in 1947.1 We report the case of 72 years old female patient who developed subarachnoid hemorrhage and intraventricular hemorrhage with ST-segment elevation.

5.A Case Report of Strumal Carcinoid of the Ovary.

Strumal carcinoid of the ovary is a rare turkor characterized by an intimate mixture of thyroid follicles and carcinoid. Herein is reported an ovarian strumal carcinoid in a serous cystadenoma from a 27-year-old pregnant woman. The thyroid follicular epithelial cells had immunohistochemically thyroglobulin and carcinoid tumor cells contained neuron-specific enolase, chromogranin and carcinoembryonic antigen. In addition, carcinoid cells showed neuroendocrine granules ultrastructurally. Calcitonin and amyloid were not found. This tumor may be originated from pluripotent endodermal germ cells on the basis of morphologic, immunohistochemical and ultrastructural studies.

6.Hypersensitivity myocarditi caused by ceused by cafadroxil.

Hypersensitivity myocarditis is an inflammatory disease of the myocardium usually related to drug allergy. The clinical manifestation may be nonspecific, and the diagnosis is seldom suspected or established during the life. So in most of the reported patients, the diagnosis was made at autopsy. Although, retrospectively, more than 90% of the patients described have had clinically recognizable cardiac involvement before death, tole clinical suspicion of hypersensitivity myocarditis is rare. The presence of nonspecific cardiac finding in a patient receiving any drug associated with hypersensitivity should alert the clinician to the possibility of drug related myocarditis. We experienced a case of hypersensitivity myocarditis accompanying allergic skin lesion in a 58 years old man with a history of taking cefadroxil. Fortunately, the outcome was favorable, therefore we do not have histologic confirmation of the diagnosis.

7.A Case of Skin Metastasis Manifested as a Presenting Sign of Pancreatic Tail Cancer.

A 68-year-old male patient visited our clinic complaining of a single skin lesion on the left shoulder for 5 months. The lesion was a bluish to pigmented, bean-sized nodule. No subjective symptoms and signs accompanied it. Histologically, the tumor mass was composed tumor emboli in the vessels and moderately differentiated neoplastic glands lined by anaplastic cells. These findings were consistent with the metastatic adenocarcinoma. Abdominal sonography and CT scanning showed a mass at the tail of the pancreas. Therefore, we diagnosed this skin lesion as a metastatic adenocarcinoma originating from the pancreatic tail. We report herein a rare case of skin metastasis from pancreatic tail cancer, in which the cutaneous involvement was the first sign of the internal malignancy.

METHODS: This study employed a quantitative observational with cross-sectional design and the subjects were consisted of 189 TB (Tuberculosis) control manpower who were working in both community health centers and private hospitals. Variables included in this study were demographic, social, work-related characteristics, aqhnd self-efficacy. A reliability of the instrument for the performance was Cronbach's alpha=.91. Data were analyzed by t-test, ANOVA and multiple regression using SPSS (19th version). RESULTS: The mean value of the degree of work-related performance of TB control manpower was 4.4+/-0.49 and it was higher than those of general nurses working in any other health care departments. The mean differences in the degree of work-related performance were in self-efficacy, workplace, the number of full charge manpower in TB, the number of participation in in-depth education, and the willingness to job maintenance. In multiple regression analysis, factors associated with TB control manpower' work-related performance were self-efficacy (beta =.164, p=.025), workplace (beta=-.186, p=.023), the number of participation in in-depth education(beta=.339, p<.002), and the willingness to job maintenance (beta=-.157, p=.036). CONCLUSION: Results of this study showed that work-related performances of manpower working in private hospital were higher than those of manpower in public health centers. So, it is necessary to enhance nation's active intervention to improve effectiveness and reduction of risk factors in TB policy.
