Background: Stomach cancer is the most commonly diagnosed malignant tumor in Korea. Early detection by mass screening very important to increase survival rate of stomach cancer, because the prognosis of early gastric cancer is excellent, and specific symptoms and physical findings are rare with early gastric cancer. Methed: 1865 car manufacturing company employees were examinined by gastrofiberscopy at a dispensary of the company from May 6, l992 to July 13, 1992. Results: 1. Number of cases with normal finding was 1262(67.7%), gastritis 422(22.6%), gastric ulcer 47(2.5%), gasrtric cancer 4(0.2%), duodenitis 39(2.1%), duodenal ulcer 63(3.4%), combined ulcer(gastric & duodenal) 2(0.1%,), post operative state 6(0.3%), and others 4(0.3%) in total 1865 examinees. 2. Of 422 cases with gastritis, cases with superficial gastritis were 171 (9.2%), atrophic gastritis 138(7.4%), erosive gastritis 94(5.0%) and hemorrhagic gastritis 19(1.0%). 3. Of 4 cases with stomach cancer, 1 case was advanced stomach cancer and 3 cases were early gastric cancer(two cases of IIc and one case of IIc+III) by macroscopic classification. Conclusion: Mass screening provides an excellent opportunity to detect gastric cancer in populations without symytoms or with nonspecific symptoms. It should be recommended to undergo screening exam for high risk group of stomach cancer.

To evaluate the efficacy of anthelmintic treatment of Enterobius vermicularis in highly endemic population, total 82 children in two orphanage institutes were divided into 4 groups and treated with placebo, 5 mg/kg of pyrvinium pamoate, 100 mg of mebendazole and 10 mg/kg of pyrantel pamoate respectively. Total 4 anal swabs were taken from each child before treatment and the even distributions of positive rate and consecutive results between groups were considered. Follow-up swabs were taken 8 times up to the 40th day after treatment. After remarkable and significant reduction of positive rates up to 19-27th day after treatment in respective groups, the remarkable egg positive conversions were observed 3-4 weeks after treatment. The positive conversion was the earliest and the most remarkable in pyrantel-treated children and the slowest and the least remarkable in mebendazole-treated cases. Also found was that the figures of negative conversion were different with statistical significance between two categories of cases, consecutively positive and alternately positive cases of pre-treatment swabs. This means that the conventional indices of cure rate in E. vermicularis infection may be variable by the selection of subjected cases. Above results mean that the spectrum of susceptible pinworms according to the developmental stages are different between currently available drugs, and the ability to remove pinworms completely in certain developmental stage are also different between chemotherapeutics. These results suggest the need of interval chemotherapy of Enterobius vermicularis infection in heavily infected cases of in highly endemic population to achieve the complete eradication of whole range of pinworms in intestine.

PURPOSE: Postpartum depression (PPD) is a common postnatal problem. PPD has a negative influence on maternal functioning and child development. We examined the result of Edinburgh Postnatal Depression Scale (EPDS) after delivery. METHODS: Three hundred two postpartum mothers were enrolled in the study. The Korean version of EPDS was completed and scored at postpartum visits. Mothers whose score was 10 or more were classified as an EPDS positive for PPD. We reviewed their medical records and determined risk factors for positive EPDS result. RESULTS: The positive rate of EPDS was 27.8% (84/302). Among various factors, a newborn's body weight more than 4.0 kg was statistically related to the positive result. The positive rate of EPDS was significantly higher when performed within postpartum 60 days than after postpartum 60 days. Among 84 positive women, only three mothers visited a psychiatric clinic for further evaluation and management of PPD. CONCLUSION: The EPDS was a useful tool for PPD screening. Obstetricians should consider an EPDS for effective screening of PPD. It would be the next important issue to encourage a screening positive mother to visit a psychiatric clinic.

Basal cell carcinoma(BCC) and squamous cell carcinoma (SCC) are very prevalent neoplasms of the human skin. Ultraviolet radiation in sunlight is a well-established mutagen of the p53 gene and is one of basal cell carcinoma and squamous cell carcinoma. The newly-formed vascular network is important for neoplasms to grow beyond a size of about 1 mm2. Recent reports have suggested the hypothesis that a mutant p53 protein is closely related with capillary density. Immumohistochemistry for p53 protein and CD34 was performed in 20 cases of BCCs and 14 SCCs to evaluated the relationship between p53 protein and capillary density. The results were as follows:1. The microvessels stained by CD34 were mainly located in the interface of tumor cells and stroma. 2. There was no difference in the microvessel density according to the histologic types and age of the patients, but a higher microvessel density was noted in male patients. 3. The aggressive BCCs and the less-differentiated SCCs showed higher p53 immunostaining. 4. The mean microvessel density of cases showing strong positive immunostaining of the p53 gene(54.73+/-17.75) was higher than that of others(39.75+/-18.30). These results suggested that p53 protein expression and microvessel density are not related to the histologic types and age of the patients, but that differentiation and biologic behavior such as the infiltrating property of tumors and the microvessel density are closely related to p53 protein expression.

Thoracic disc herniation is one of the spine disc herniations which has poor prognosis. It is rare in occurrence, as it is only about 0.15-1.7% of all spine disc herniations. Due to anatomical problems and degenerative processes occuring in the thoracic spine and spinal canal, pathophysiological problems and diagnostic difficulties are encountered. Several surgical techniques have been tried and modified but up to now no satisfactory surgical management had been reported. Recently the neurosurgical team, at the St. Paul's Hospital, encountered a patient with T7-8 disc herniation. After all diagnostic tests and examinations were done and the diagnosis verfied, a transthoracic anterior, anterolateral, lateral, posterolateral and posterior apporach was done with iliac bone graft microsurgical technique. The post-operative recovery was uneventful. As there was remarkable improvement during the clinical course as compared to the results done employing other surgical techniques, we wish to share with you our new experience.

Smooth muscle tumors have been classified into leiomyoma and leiomyosarcoma. But the criteria for malignancy are not well defined. Traditionally, mitoses are the most important differentiating feature between leiomyoma and leiomyosarcoma. However mitotic activity was regarded as an acceptable finding in about 20% of cutaneous leiomyoma. The widely used designation "smooth muscle tumor of uncertain malignant potential" is a reflection of the limitation of available criteria to precisely diagnose tumors with borderline atypical features. We present herein a case of a cutaneous smooth muscle tumor of uncertain malignant potential, which is mitotically active, on the abdomen of a 45-year-old man.

Painful 203 patients with chronic pain were treated with transcutaneous electrical nerve stimulation in the CNS & Pain Management Institute of St. Paul's Hospital, Catholic Medical College, Seoul, Korea. Each patient was instructed on the method of autostimulation with variations in for an initial observation frequency and intensity. The effect were recorded daily period of about 3 weeks. The patients were evaluated at suitable intervals and asked to grade the relief of pain on a percentage scale. Four categories of pain relief were established : 80% to 100%(A), 60% to 80%(B), 30% to 60%(C), 0% to 30%(D). This procedure with modern, solid state devise has been tried enough that it can be recommended as a potentials treatment for most types of pain about 90% of pain problems can be managed in this way. The mode of action of transcutaneous Electrical nerve stimulation is most simple and can not be explained only by gate control theory.

Aneurysms located on the proximal(A1) segment of the anterior cerebral artery are known for their rarity. In this study, eight patients, all with aneurysms arising from the proximal(A1) segment of the anterior cerebral artery, were selected from 625 patients who underwent surgical treatment under the diagnosis of cerebral aneurysm. Each cases were reviewed as an attempt to investigate the demographic and clinical features of these patients. The incidence of A1 aneurysm was 1.3%. Multiple aneurysms occurred in one(12.5%) out of the 8 cases and the bleeding was from the A1 aneurysm. The mean age of the eight patients was 56.8 years: there were six women and two men. The aneurysms were classified according to the mode of origin of the aneurysm in relation to the A1 segment: in 4 of the 8 cases, the aneurysms were found to be originated from the junction of the A1 segment and a perforating artery: in which the remaining 4 cases, they are from the A1 segment directly. Computerized tomography(CT) of these aneurysms revealed subarachnoid hemorrhage extending to the surprasellar and sylvian cisterns similar to that of the posterior communicating artery aneurysm or the internal carotid artery bifurcation aneurysm. Bleeding which extends to the interhemispheric cisterns or septum pellucidum, while frequently occurrs during the rupture of anterior communicating artery aneurysm, are rare in these cases. Because there are many differences between the findings in author's series and these in the literature, coordinated studies are necessary to define the demographic and clinical features of A1 aneurysm in Korea.

Epidural injection of 2-4mg morphine for 7-14 days were given to 18 patients with severe chronic pain. All cases except 2 arachnoiditis had considerable amelioration of pain, which commenced within 2-3min, reached a peak in 10-20min, and was effective for 6-72 hours. Using sensory evoked potentials(SEP) examination for pre-and post-morphine epidural infusion in 18 pain patients, we found the morphine inhibited the action of spinal pain receptor system. It is suggested that morphine reached the subarachnoid space and produced its effect by direct action on the specific opiate receptors in the substantia gelatinosa of the posterior horn cell of the spinal cord.
