1.Study of Prolonged Membrane Rupture with Relationship to Management.

At present there is argument for and against the use of prophylatic antibiotics in simple prolonged ruptured membranes. At Il Sin we studied 96 babies of mothers with prolonged ruptured membranes(58 control, 38 treated). We differentiated between the premature and fullerm babies. The number of premature babies were too small for conparision but in the full term control and prophylactic group, we did not find any difference in complication incidence. The refore, we do not think prophylactic antibiotics are indicated in simple prolonged ruptured membrane of fullterm babies. However further studies are necessary in premature babies.

2.To Study Effetct of Dexamethasone in Preventing Respiratory Distress Syndrome in Premature Labour.

At Il Sin Women's Hospital from January 1st to June 30th 1978 there were 463 preterm deliveries(Less than 38 weeks Gestation). 161 of these were between 28-36 weeks gestatin. To study the effect of Dexamethasone in preventing respiratory distress syndrome(R.D.S.) 39 mothers received Dexamethasone before delivery and 85 comparable babies were placed in the control group. 1. R.D.S. was seen in 4(10%) of the treatment group and 12(14%)of the control group. In the control group there is a slightly higher incidence but as the bumbers are small it is difficult to compare with western countreis 2. When comparing the 2 groups the incidence of R.D.S. is greatest in less than 32 weeks gestation. 3. Apgar Scoring is similar in both groups. 4. When Dexamethasone was given for greater than 48 hours the incidence of R.D.S. was less 5. Mortality rate is 10% in treatment group and 24% in control group but majority of deaths are due to severe birth hypoxia or very low birth weight (less than 1000 Gms) 6. The overall incidence of R.D.S. in Il Sin Women's hospital was 29 during the 6 months. This is 0.9% of all livebirths and 6.2% of preterm births being low compared with western countries.

3.Factors associated with New Graduate Nurses' Reality Shock.

PURPOSE: The purpose of this study was to identify factors that affect reality shock in new nurses. METHODS: Participants were 216 newly graduated nurses with less than 1 year experience in 5 university and 10 general hospitals in Busan, Ulsan and Gyeongnam, A self-report questionnaire was completed by the nurses between November 18 and December 25, 2013. Data were analyzed using t-test, ANOVA and hierarchial multiple regression analysis with the SPSS/WIN 21.0 Program. RESULTS: Average scores for work environment and environmental reality shock were 2.63+/-0.33 and 2.66+/-0.43 points respectively. Regression analysis showed that with the nurses' demographic and work characteristics controlled, work environment explained 4.2% of the reality shock. Factors significantly affecting reality shock included nurses' changing residence because of job (beta=.21, p=.001), whether they were able to work on the unit of their choice (beta=-.13, p=.031) and whether they had a choice in days off (beta=-.14, p=.038). CONCLUSION: When these factors are considered, reduction in nurses' reality shock requires improvement in work environment, placing new nurses in a department of their choice and allowing them a choice in off-duty days. These measures would also help achieve organizational goals and develop the new nurses as professional nurses.

4.Hyalinized Hemangioma of the Liver: A case report.

We report a case of 50-year-old man with hyalinized hemangioma of the liver. This tumor was detected incidentally during surgical operation for gall stone and was preoperatively diagnosed as intrahepatic stone. A 2.5 2 cm sized gray to white hard mass was found in the left lobe of the liver. Histologically, the tumor was composed of dense collagenous tissue with marked hyalinization, calcification, and ossification admixed with scattered sclerotic vessels. Abundunt elastic fibers were noted in the hyalinized tissue. Totally hyalinized hemangioma is very uncommon and could be erroneously diagnosed as a malignant tumor by preoperative radiologic examination.

8.The Incidence of Unexpected Antibodies in Transfusion Candidates.

BACKGROUND: Some investigators have done several studies on the frequency and distribution of unexpected antibodies in Korea. But more studies are necessary to estimate the precise frequency of unexpected antibodies. METHOD: In order to determine the frequency of unexpected antibodies, we analyzed the results from 23,735 antibody-screening tests performed for transfusion candidates at Yonsei University Severance Hospital during a recent two-year period (March 1998~ Feb 2000). Screening and identification of unexpected antibodies were carried out using a gel agglutination technique with DiaMed-ID system (DiaMed, Murten, Switzerland). RESLUT: Out of all 23,735 serum samples, positive results were obtained from 109 sera (0.46%). Clinically significant antibodies were found from 0.29% of all the population. Antibodies that were detected most frequently were anti-E+c (33 sera), followed by anti-E (19 sera) and anti-Lea (12 sera). CONCLUSION: We found that Rh antibodies, clinically significant unexpected antibodies, are detected frequently in Korea, too. So we consider that unexpected antibody screening test must be included in pre-transfusion test in Korea and this information is reassuring in assessing the risk associated with blood transfusion in many hospitals of Korea.
