The incidence of colorectal cancer has been increased gradually and it was about 6.9% of all malignancies in Korea. Early diagnosis was recognized as the most important factor influencing the prognosis of colorectal cancer and the incidence of early colorectal cancer was increasing. Thus great change was observed in the treatment of early colorectal cancer, endoscopic therapy as well as curative surgical resection is being accepted. We report a case of effective and safe endoscopic therapy for early rectal cancer in 5S-year-old male patient. He presented with rectal prolapse and bleeding of a year's duration. A large polypoid mass with fine nodular surface and thick, short stalk was seen in the rectum at 4cm above the anal verge during flexible sigmoidoscopy. After the histologic examination of specimen obtained by bite biopsy, CT scan of pelvic cavity and ultrasonogram of abdomen, the tumor was removed safe and completely by 2 sessions of piecemeal polypectomies and strip biopsy. He has been well without symptoms and signs of recurrence of rectal cancer for 20 months.

PURPOSE: This study is exploratory research to check the survey instrument KDSQ-C used to measure cognition for agreement between the elderly and their caregivers. METHODS: Elderly persons from 170 families, who are aged over 65, have never been diagnosed with dementia, and who have caregivers in S City and K City were randomly selected by their nurses. A survey was conducted in the time from September to November, 2009 through personal interviews using a questionnaire. RESULTS: In terms of consistency among items, there was high consistency (0.813) in the responses regarding complex task competence for the statement 'it is hard to reach a destination alone with public transportation'. There was relatively low consistency (0.63) in the responses regard memory for the statement 'they forget an appointment'. When the subject elderly and caregiver live together, as when their children or spouses responded, consistency was greater than when the caregiver lived in a different home. CONCLUSION: KDSQ-C showed high agreement in measurement between the subject elderly and their caregivers, demonstrating that it can be used as a measurement scale for cognition which can be directly applied to the subject elderly.

Clinical history and dermatological examinations were performed in 51 patients with contact dermatitis due to metal working fluid who were working at the metal industry. Patch tests were also performed with standard series and oil cooling fluid series. Chronic lichenified eczema such as lichenified and fissured was the most common clinical type(45.1%). 19 patients had positive reaction to either one or more of the standard series and oil & cooling fluid series. We found ammoniated mercury chloride was the most common sensitizer in standard series, followed by potassium dichromate, 4-phenylendiamine dihydrochloride, cobalt chloride, nickel sulfate, quinoline mix and cabra mix. Microbiocide was the most common allergen(4 of 6 positive result in oil & cooling fluid series.

BACKGROUND: This study was to survey the effect of the degree of spiritual nursing care performance on the spiritual nursing care ability of the nursing studentsMETHODS: The researcher sampled 130 nursing students for a questionnaire survey conducted from September 17 until September 27, 2018. The data of analysis used SPSS 23.0 program.RESULTS: The spiritual nursing care ability was 4.4±0.8(total score 6) and the degree of spiritual nursing care performance was 2.9±1.8(total score 4). The spiritual nursing care ability differed significantly depending on religion(F=7.570, p < .001), the level of spiritual nursing knowledge(F=19.873, p < .001), education type(F=14.626, p < .001), necessity of hospice(t=2.280, p=.024). The degree of spiritual nursing care performance differed significantly depending on spiritual nursing education time(F=2.932, p=.036). The correlation of two variable was statistically significant difference(r=.206, p=.019). The influencing factors on the spiritual nursing care ability was religion, the level of spiritual nursing knowledge, education type, dying experience(R2=0.378, Adj R2=0.353), the degree of spiritual nursing care performance was spiritual nursing education time(R2=0.065, Adj R2=0.043).CONCLUSION: These results show that nursing students are not able to perform spiritual nursing care properly to subjects who need spiritual nursing care. Therefore, it is necessary to develope programs to meet the spiritual nursing care needs of nursing students and to provide practical education in accordance with the program by cooperating with the nursing education staff, clergy and clinical nurse etc. And spiritual nursing care should be taught as a required subjects in the curriculum.

For neurologic symptoms such as mental changes, the etiologies should be thoroughly looked for. Neurologic problems are primarily assessed by medical history, physical and neurologic examination, electroencephalogram, neuroimaging studies. Furthermore, cardiovascular disorders, metabolic diseases, toxic materials and psychologic problems are also evaluated. In some cases that are difficult to find the causes of mental change despite the thorough evaluation and rarely, neurologic shock caused by severe pain can bring about alterness of consiousness. The causes of leading points are discovered frequently in the intussusception in late childhood and lipoma in the small bowel can be one of leading points but it rarely occurs. We report a case of neurologic shock with mental changes from intussusception secondary to a small bowel lipoma in a 13 year-old boy.

PURPOSE: The purpose of this study is to evaluate the effects of a local community based multi-factorial program for high-risk younger and older elderly people. METHODS: The quasi-experimental research design (pretest-post test) was employed. Participants were recruited in Seoul and a total of 98 elders completed an 8-week multi-factorial program for preventing frailty. Descriptive statistics, chi2-test and GLM were used in the data analysis with SPSS/WIN 15.0. RESULTS: The high-risk elderly people in the younger and older stages showed differences in IADL, TUG and BMI, and after being provided with the multi-factorial program for preventing frailty, some effects were shown on improving the total score of frailty, a physical function, TUG, BMI, depression, subjective feeling of health, and social interaction. CONCLUSION: The 8-week multi-factorial program for preventing frailty had positive effects on improving physical, emotional and social functions of the high-risk elderly people. It is necessary to evaluate the effects after individual intervention as well as group intervention and to evaluate the effects of the program by setting a control group in the future.

The avulsion fracture of the calcaneal tuberosity is rare injury. Usually, it occurs from indirect trauma in old patients with osteoporosis or in patients with diabetic neuropathy. Especially, the bone and joint damage occurred in active patient with severe sensory loss or arthropathy related to nerve damage regardless of the cause is referred to neuropathic arthropathy. Generally, a patient with nondisplacement or minimally displacement is treated by conservative therapy and a patient with severe displacement is treated by open reduction and internal fixation. We experienced a 33 years-old woman with diabetes mellitus who had the displaced avulsion fracture of the calcaneal tuberosity without significant trauma and did not treat. We report upon this case at the 2 years follow-up.
