2.Spontaneous Closure of Ventricular Septal Defect: A Clinical Study of 42 Cases.

During a period of 5 years and 3 months, from January, 1980 to march, 1985, 42 cases of ventricular septal defects were observed to be closed spontaneously during their follow up period and were evaluated at pediatric department, hanyang University hospital. 1) Sex incidence shows female preponderance with male to female ration of 1:2. 2) The mean age when they were diagnosed as ventricular septal defect was 5.0+/-6.4 months, and that of VSD murmur last noted was 13.3+/-11.5 months. The mean age when typical VSD murmur disappeared due to spontaneous closure of the defects was 21.1+/-18.7 months. Spontaneous closure of ventricular septal defects disclosed under one year in 22 cases (52.4%). Of those 22 cases, ventricular septal defects were spontaneously closed under 6 months of age in 16 cases(38.8% of whole study population). From the whole study population 90.5%(38 cases) were spontaneously closed under the age of 5 years. 3) Major clinical and physical characteristics before spontaneous closure of ventricular septal defects were typical pansystolic murmur with maximum intensity at left lower sternal border in all cases, palpable thrill in 5 cases(12.8%), ventricular heaves in 4 cases(10.2%) and frequent respiratory infection histories in 27 cases(64.3%). 4) Electrocardiographic findings when they were initially presented as ventricular septal defects revealed normal axis in 28 cases(84.8%), left axis deviation in 3 cases(9.1%), right axis deviation in 2 cases(6.1%) as frontal QRS axis and left ventricular hypertrophy in 8 cases(24.2%), right ventricular hypertrophy in 5 cases(15.6%) and biventricular hypertrophy in 3 cases(8.7%). Other electrocardiographic abnormalities when they had ventricular septal defects were left atrial enlargement in 12 cases(36.4%) and intraventricular conduction delay in 12 cases(36.4%). After spontaneous closure of ventricular septal defects, the electrocardiographic findings revealed normal axis in 36 cases(92.3%), left axis deviation in 3 cases(7.7%) and left ventricular hypertrophy in one case(2.6%). The remaining abnormalities after spontaneous closure of ventricular septal defect were intraventricular conduction delay in 14 cases(35.9%), deep SV6 in 4 cases(10.3%), tall RV6 in 5 cases(12.8%) and long QTc in 1 case(2.6%). Thirty-eight cases(97.4%) do not show ventricular hypertrophy pattern in EKG after spontaneous closure of their defects. 5) When we analyse their frontal plain chest X-ray films after spontaneous closure of ventricular septal defects, cardiomegaly(cardiothoracic ratio)55%) noted in 7 cases(17.9%) and pulmonary plethora in 5 cases(12.8%);while those before the defect were closed spontaneously were 67.6% and 64.7%, respectively. 6) Among 26 cases who underwent follow up 2 dimensional echocardiographic study so-called septal aneurysm were noted with the process of spontaneous closure of ventricular septal defects in 15 cases(57.7%) and the remaining 11 cases do not have any evidence of 'septal aneurysm' even after the completion of spontaneous closure of their defects. In the patient group with so-called 'septal aneurysm' the proportion of female sex was 60% and early systolic clicky sound were heard in 9 patients from 15 cases(60.0%), while those without 'septal aneurysm'were 55% and 18.2%, respectively.

3.A laser Doppler study of gingival blood flow change following periosteal stimulation.

The roots of teeth exposed by gingival recession, may be successfully covered by various type of gingival grafting procedures. Vascularization of the recipient site is an essential determinant of the grafts'survival during the first healing stages. It has been suggested that a procedure by which they stimulate the periosteum presurgically will induce the proliferation of neo-endothelium in the site to be operated. The purpose of this study is to evaluate the variations in the gingival blood flow during 4weeks after periosteal stimulation in patient scheduled to receive gingival grafts and to compare variations in the gingival blood flow between smoker and non-smoker. Laser Doppler Flowmetry(floLAB(R), Moor Instruments Ltd, England ; wave length = 780mm. Max. power =1.6mW) was used to measure the gingival blood flow. 112 sites of 68 male patients (32 smokers and 36 nonsmoker), aged between 23 and 48 years (smoker : 24-44 years, mean=32.6, non-smoker : 23-48 years, mean=28.5) were monitored for the blood flow. Gingival blood flow measured at before periosteal stimulation, 1-, 2-, 3-, and 4-weeks after periosteal stimulation from 10 a.m. to 2 p.m. The difference of blood flow in each measuring time, each measuring site and between smokers and nonsmokers were statistically analyzed by MANOVA. The results were as follows : (1) Blood flow stayed increased for 2 weeks, and then, it was a tendency to decrease(p<0.05). (2) There was no statistically significant difference of blood flow change between smokers and non-smokers. (3) The blood flow at middle site had lower than mesial and distal site during the measuring periods(p<0.05). The present study suggested that blood flow change following peirosteal stimulation was significant difference, thus periosteal stimulation before gingival graft might induce favorable results in gingival recession patient.

4.A Clinical Study of Hypotensive Effect of Tripamide(Normonal(R)) in Essential Hypertension.

The hypotensive effect of tripamide(Normonal(R)) were evaluated in 31 cases of essential hypertension. Fifteen to thirty mg of tripamide per day were administered continuously for 8 weeks. The results were as follows; 1) The systolic blood pressure was significantly lowered from 175+/-15 mmHg(Mean+/-SD) before treatment to 144+/-17 mmHg after treatment(p<0.01), and the diastolic blood pressure was significantly lowered from 106+/-11 mmHg before treatment to 90+/-12 mmHg after treatment(p<0.01). 2) The systolic blood pressure was lowered 20 mmHg or more in 25 out of 31 cases(80.7%), and the diastolic blood pressure was decreased 10 mmHg or more in 25 out of 31 cases(80.7%) at the end of 8 weeks treatement. 3) There was no significantl differences in the level of SGOT, serum cholesterol, serum electrolytes(Na+, K+, Cl-), BUN, creatinine and urinc acid before and after treatment with tripamide. 4) In most cases the subjective symptoms disappeared or became easier after tripamide treatment. 5) Profound weakness was developed in 2 cases during tripamide administration.

5.Ultrasonographic findings of early abortion: suggested predictors.

To investigate predictable ultrasonographic findings of early abortion. To investigate objective rules for the screening of abortion Ultrasonographic examination of 111 early pregnancies between the sixth and ninth week in women who had regular 28 day menstrual cycles was performed. Ultrasonographic measurements of the gestational sac. Crown rump length and fetal heart rate were performed using a linear array real time transducer with doppler ultrasonogram. All measurements of 17 early abortions were compared to those of 94 normal pregnancies. Most of early aborted pregnanices were classified correctly by disciminant analysis with G-SAC and CRL (GSAC=0.5 CRL+15. Sensitivity 76.5%, specificity 96.8%). With the addition of FHR, 94.1 of early abortions could be predicted. In conclusion, ultrasonographic findings of early intrauterine growth retardation. Small gestational sac and bradycardia can be predictable signs suggestive of poor prognosis of early pregnancies.

6.The Effects of Hand Acupuncture & Moxibustion Therapy on Elders' Shoulder Pain, ADL/IADL and Sleep Disorders.

PURPOSE: The purpose of this study was to evaluate the effects of hand acupuncture.moxibustion therapy on elders' shoulder pain, ADL/IADL and sleep disorder. METHODS: This is quasi-experimental with none equivalent control group pretest posttest design. The subjects were randomly assigned to one of Experimental Group A (Ceramic Seoammoxa therapy) (n=20), Experimental Group B (Seoampellet therapy) (n=18), Experimental Group C (Ceramic Seoammoxa and Seoampellet therapy) (n=20), and a control group (n=20). The intervention was applied 3 times per week for 6 weeks. NRS, Song's ADL/IADL scale, Oh, Shin and Kim's sleep disorders scale were used. RESULTS: Hypothesis "Both shoulder pain and the level of sleep disorders of the experimental group A, B and C would be lower than the control group" was supported (p<.001). Hypothesis "ADL/IADL of the experimental group A, B and C would be higher than the control group" was supported (p<.001). In 3 weeks after the experiment, the experimental group A, B and C showed significant difference in change of ADL/IADL (p=.013). In 6 weeks after the experiment, the experimental group A, B and C showed significant difference in change of ADL/IADL (p<.001) and sleep disorders (p<.001). CONCLUSION: Consequently, the hand acupuncture.moxibustion was an effective therapy in relieving shoulder pain and sleep disorders and improving ADL/IADL among elders.

7.Ultrasonographic and pathologic study of ovarian tumors

This is a retrospective study of 161 ovarian neoplasms, all of which were surgically removed and had preoperative sonographic examinations. They were evaluated respect to age, tumor size and its echogenicity. The results were as follows: 1. Of all 161 tumors, physiologic lesions were 67 cases (41.6%), germ cell tumors were 28 cases (17.4%) and serous tumors were 19 cases (11.8%). 2. Of all 161 tumors, right ovarian lesions were 82 cases (50.9%), left were 59 cases (36.7%), and bilateral lesions were 20 cases(12.4%). 3. The most characteristic findings of tumors were as follows: 1) Physiologic tumors were 2-9cm sized (88.1%), anechoice-5% echogenic (79.1%), and developed during 4th & 5th decades (91.0%). 2) Inflammatory tumors were 2-9cm sized (94.1%), 50%-totally echogenic (76.5%), and during 4th & 5th decades (94.1%). 3) Serous tumors were 2-9cm sized (60.3%), anechoic-5% echogenic (89.5%), and during 3rd & 4th decades (84.2%). 4) Mucinous tumors were 5-19 cm sized (77.8%), anechoic-5% echogenic (77.8%), during 2nd-8th decades with diffuse distribution. 5) Endometrioid tumors were 5-14cm sized (100.0%), variable echogenic, and during 3rd & 4th decades (90.9%). 6) Germ cell tumors were 5-14cm sized (75.0%), variable echogenic, during 3rd & 4th decades (82.1%). 7) Parovarian cysts were 2-14cm sized (90.0%), anechoic-5% echogenic (100.0%), with diffuse age distribution. 4. The malignant and borderline malignant tumors were 9 caseswith more than 10cm sized (77.8%), and developed during older than 4th decade (100.0%). Anechoic 3 cases, 1-5% echogenic 2 cases, and totally echogenic 4 cases were found.

10.Effect of Atenolol on Left Ventricular Function in Essential Hypertension.

This study was made to evaluate the effect of oral atenolol, a cardioselective beta-adrenergic blocking agent without intrinsic sympathomimetic activity, on left ventricular function in patient with essential hypertension. Atenolol, 100mg/day, was given to 11 hypertensive patients for 4 weeks, and its effects on arterial pressure, pulse rate, left ventricular dimensions and ejection phase indices of myocardial performance were examined by echocardiography. Echocardiographic studies were performed before treatment and after 4 weeks of atenolol therapy. Arterial pressure fell form 145/90 mmHg to 138/84mmHg after 4 weeks. Pulse rate fell significantly from 69/min to 58/min(p<0.05). Left ventricular end-diastolic and end-systolic dimensions and mean rate of circumferential fiber shortening(mVcf) did not change significantly. Ejection fraction increased significantly from 0.66 to 0.72(p=0.01). This results indicate that atenolol in the resting state has no depressant effect on left ventricular function in patients with essential hypertension.
