1.Clinical and Histophthologic Review in Patients with Henoch: Schonlein Purpura.

Authors review the clinical and histopathologic findings of 41 EIenoch-Schonlein purpur.a patients and attempt, to clarify the association between abnorma] urinary findings and anti-streptolysin 0(ASLO) titer and performed the direct immunofluorescence(',E)II) study on skin lesion to investigate the pathogenetic mechanim of renal involvement. The results were summarized as follows : 1. Ag(, distribution was form 3 month to 63 years and mean age was 18.9 years. male to female ratio was 1.2:1(22 males '. 19 females). 2. In he past history, there were preceding upper respora.tory infections in 25(61:o)patients and drug history in 10(24.3%)patients. 3. (linical manifestations in the order of frequency were as follows : arthralgia (60.9%), hematuria or albuminuria(43.8%), abdominal pain(21.9%), and positive occult: blood test(7.3%)in stool examination. 4. In the laboratory fiindings, leukocytosis was cleveloped in 14.6%, anemia in 2.4%, increased eryttrocyte sec1imentation zate in 12.3% increased ASLG titer in 43.9% of patients, and serum IgG was increased in 4 of 20(20.6)patierits, IgA in 4 of 21(19%)patients, IgM in 1fo 19(5.3o)patients. 5. Because correlation between increased A.LO titer ancl ahnoormal urinary findings wa.s high(x-test, p< 0.0,5), increased ASI,O titer may be inclicator of renal involvement. 6. DIF stuc].y was per formed in 34 patients. All showed positive reactiviy. IgG(17%), IgA(29.4% ), Clq(14.6%), C3(12.2% ), fibrinog.n(82.4%) were deposited around the vessel wall in a granular pattern. 7. There was statistically significant increase in mean serum IgG value in patients with abnormal urinary findings( t test, p < 0.05 ).

2.A Case of Dysplastic Melanocytic Nevus.

The dysplasitc melanocytic nevus(DMN) of the skin is an atypical-appearing melanocytic tumor characterized by intraepidermal atypical melanocytic preliferation. Clinically, DMN has a diameter greater than 5mm, ill-defined irregular borders, irregularly distributed pigmentation surrounded by erythems,. Histopat- hologically, there are numerous discrete nests of atypical melanocytes situated at the dermoepidermal junction of elongated rete ridges. In the dermis, concentric lamellar fibroplasia, lymphocytic infiltrates and neovascularization are noted. We report herein a case of DMN developed on the trunk of 31-year-old Korean male.

3.Three Cases of zosteriform Connective Tissue Nevus.

The connective tissue nevus represents a hamartoma of various components of dermal connective tissue elements, predominantly collagen, elastin, or glycosa minoglycan. Zosterriform connective tissue nevus, described by Steiner, in 1944, clinically shows grouped papules in a zosteriform band without extracutaneous manifestations of family history, and peculiar histopathologic findings. We experienced 3 cases of typical zosteriforrn connective tissue nevus developed on the flank area of 30-year-old male and 21-year-old male, and on the inguinal area of 15-year-old female.

4.A Case of Juvenile Xanthogranuloma Associated with Juvenile Chronic Myeloid Leukemia and Neurofibromatosis.

Juvenile xanthogranuloma(JXG) is a a benign histiocytic proliferative disorder most frequently seen in infants. The usual course is self-limited, but it may be a manifestation of juvenile chronic myeloid leukemia(JCML). In addition to JXG, some patient with JCML may have multiple cafe-au-lait spots and a family history of neurofibromatosis(NF). The association of JXG and cafe-au-lait spots in patients with JCML is of interest in light of description of otherwise healthy children with JXG who also had cafe-au-lait spots. We report herein a case of JXG in 18-month-old boy who also had multiple cafe-au-alit spots, which predated the JCML.

5.A Histopathological Study on the Production of Exfoliation Material in Eyes with Exfoliation Syndrome.

The hallmark of exfoliation syndrome is the appearance of white granular deposits and dandruff-like flakes on the lens,iris and angle. The incidence of glaucoma is high,and due to zonular weakening, complications are likely to occur in cataract surgery. A histopathological study with electron microscopy was performed on the iris and conjunctival tissue in order to clarify the production process of exfoliation material in the eyes with exfoliation syndrome. The materials were obtained from 13 eyes with exfoliation syndrome during surgery for glaucoma or cataract. The clumps of microfibrils were observed in the extracellular matrix, and the exfoliation materials were closely related with microfibrils.It was found that the basic unit of the exfoliation materials was microfibril. The cellular degenerations were observed in the basement membrane of capillary endothelium, especially the cells near the iris vessels. The exfoliation materials were observed around the degenerated cells; the mature or immature exfoliation materials around the iris melanocyte which show the various degrees of degeneration. The more degeneration and melting of the cells, the larger amount of exfoliation materials. This study suggests that the exfoliation materials in the eyes with exfoliation syndrome are derived from the abnormal microfibril, abnormal basement membrane, and degenerated cell itself.

6.A Case of Semicirvular Lipoatropy.

A 21-year-old female is presented with semicircular lipoatrophy, which had developed since birth with annular depressed strophic lesion of the left thigh. Biopsy specimen from the depressed area of left thigh showed no abnormal pathologic findings. But X-ray and CT scan of the lower extremity revealed remarkably reduced fat layer of the left thigh, compared to the right side.

7.Change of Skin Temperature of Workers Using Vibrating Tools in Anthracite Mines.

By implementing epoch-making policies for industrial promotion, the national economy has made a remarkable development. As a result of such economic growth, industrial accidents and occupational diseases have become a serious problem in Korean society. In the presidential order for the execution of the Korean Labor Standard Law, neuritis and other diseases stemming from health impairments due to vibrations in industrial processes are designated to be dealt with as vibration diseases. In the case of vibration disease, industrial accident compensation is not effectively paid. In order to investigate the vibration hazards of rock-drill operation, the authors studied the subjective symptoms and performed physical function tests on a total of 79 persons (vibration exposed group) who used rock-drills, and 39 persons (control group) who did not use rock-drills at anthracite mines. The results of the physical function test were as follows : 1. The right hand was more affected by white finger than the left hand. 2. Independent variables such as duration of rock-drill operation, age, drinking and smoking were identified as statistically significant factors for the occurrence of white finger. 3. In the pain sense threshold, the group with Raynaud's phenomenon showed a statistically higher level than that of the control group. 4. The skin temperature of the group with Raynaud's phenomenon was lower than that of the control group. The recovery time of skin temperature after cooling was delayed compared with the value of the control group.

8.The Analysis of Improvement of Visual Acuity by Occlusion Therapy in Strabismic Amblyopia.

In an attempt to assess the effect of occlusion to improvement of visual acuity, we analyzed data from 55 patients with strabismic amblyopia(mean; 5.5 years old) who received strabismus surgery with occlusion(combined therapy) or surgery only. The results were as follows: 1. The younger the patients at onset, the better the visual acuity obtained. The improvement in visual acuity was higher in 27 of 32 cases(84.4%) with the combined therapy than in 5 of 18 cases(27.8%) who underwent surgery only, and the difference between those two groups was significant statistically(p<0.05). 2. In exotropes, improvement of visual acuity was shown in 19 of 32 cases(59.3%) and there was no significant difference in the improvement of visual acuity between the intermittent and constant types. 3. In 23 esotropes, 18 patients(78.3%) showed the improvement of visual acuity; 13 of 16 cases in infantile esotropia and all of 7 cases in acquired one. 4. In the final results of surgery, the alignment was satisfactory in 50 of 55 cases(90.9%), showing less than 10 prism diopters. From the results of our study, we confirmed that the occlusion during pre-and postoperative periods is the effective therapy to treat amblyopia in the children with strabismic amblyopia, and its importance was reported.

9.Treatment of Acromioclavicular Separation and Fractures by Modified Kenny Howard sling-halter

There are many procedures for the treatment of acromioclavicular separation but there are still controversies concerning the best management of these injuries. The cases of 20 acromioclavicular separations and 3 distal end fractures of clavicle were treated by conservative method with modified Kenny Howard sling-halter using long arm cast at the department of Orthopedic Surgery, College of medicine,Ewha Womans University, during the thirty six months period from March 1982 to February 1985. The following results were obtained. 1. The average duration of immobilization was 5 weeks at the grade II of acromioclavicular separation and fracture of distal end of clavicle and 6 weeks at the grade III of acromioclavicular separation. 2. The functional results were 17 excellents(73.8%), 5 goods(21.9%), and 1 fair(4.3%). 3. The complications were 3 frozen shoulders(13.0%), 2 subluxations(8.6%), and 1 skin necrosis on the elbow(4.3%). 4. The authors immobilized the upper extremity with long arm cylinder cast of 90° flexion position instead of initial harness, so that it was applied easily and the patient felt very comfortable. 5. When the fracture and severe displacement were occurred at the midthird of the clavicle, it could be reduced and maintained with the long arm cylinder cast and strap.

10.A Clinical Study on Surgical Treatment of Spondylolisthesis

Spondylolisthesis, frequently encountered problems with increasing incidence, has been recognizid as an important clinical entity in orthopaedic field. Several methods were applicated as operative treatment of spondylolisthesis; posterior fusion or H-graft, posterolateral fusion, anterior fusion, fixation with internal devices and etc. During the past 12 years from March 1972 to February 1984, 34 patients among the 42 patients were treated surgically at Department of Orthopaedic Surgery of Ewha Womans University Hospital and following results were obtained. 1. The average age was 45.2 years(19 ~ 64 years) and female was predominent(64.7%). Isthmic type was 17 patients(50.0%), degenerative type was 13 patients(38.2%) and dysplastic type was 4 patients(11.8%). Below 40 year old, dysplastic and isthmic type were predominent and degenerative type was more common in the age group above forties. 2. The most prevalent site was L5-Sl; 21 cases(61.8%) and L4-5; 13 cases(38.2%) in orders. In isthmic type L5-Sl was 13 cases and in degenerative type, L4-L5 was 9 cases. By Meyerding classification, grade I was most common(26 patients, 76.5%). 3. The clinical pictures were low back pain only and little improvement with conservative treatment(type I: 9 patients), low back pain with sciatica(type II: 13), type II plus neurologic signs such as motor weakness, sensory change or change of D.T.R(type III, 10) and little low back pain with mainly sciatica(type IV, 2). 4. Of 34 patients, anterior interbody fusion were done in 10 patients ; posterior and posterolateral fusion with decompression were done in 22 patients. Only laminectomy were done in 2 patients(type IV). 5. Fusion rate was observed in 21 patients(95.5%) among 22 patients with posterior or posterolateral fusion and 8(80%) among the 10 patients with anterior fusion. Postopertive functional results by Gill's criteria were excellent or good in 31 patients(91%). 6. There were no difference in fusion rate and functional results in operative methods.
