1.The Association of Compliance with Sick Role Behavior with Satisfaction of Basic Psychological Needs Among Hemodialysis Patients.

BACKGROUND: This study aimed to identify the association of compliance with sick role behavior with satisfaction of basic psychological needs, and to assess the influence of compliance with sick role behavior among hemodialysis patients. METHODS: Participants included 109 patients from outpatient dialysis clinics at 6 major general hospitals located in urban areas in Korea. Data for compliance with sick role behavior and basic psychological needs were collected through a self-reported questionnaire and was analyzed with independent t-test, one-way analysis of variance, Pearson' correlation analysis, and multiple stepwise linear regression analysis. RESULTS: This study showed positive correlations between compliance with sick role behavior and satisfaction of basic psychological needs (r=0.59, P<0.001). The variables predicting compliance with sick role behavior were relatedness (beta=0.28), autonomy (beta=0.29), competency (beta=0.30), and age (beta=0.24). These variables accounted for 38.2% of the variance of compliance with sick role behavior in hemodialysis patients. CONCLUSIONS: Our results indicated that it is necessary to improve compliance with sick role behaviorin order to increase satisfaction of basic psychological needs in hemodialysis patients.

2.Role of Potassium-competitive Acid Blockers in the Treatment of Gastric Acid-related Disorders

Owing to their superior pharmacokinetic and pharmacodynamic profiles, potassium-competitive acid blockers (P-CABs) score over proton pump inhibitors (PPIs) in the treatment of gastric acid-related disorders and may provide clinical benefit in the management of these conditions. Previous studies have compared P-CABs with PPIs for treatment of gastric acid-related disorders, and current data show that P-CABs are non-inferior to PPIs in the treatment of erosive esophagitis and as maintenance therapy. P-CABs are useful for effective healing and as maintenance medications in patients with severe esophagitis. These drugs also aid with healing of peptic ulcers and artificial ulcers secondary to gastric endoscopic submucosal dissection (ESD). Few studies have discussed prevention of delayed ulcer bleeding after gastric ESD and ulcers associated with long-term nonsteroidal anti-inflammatory drug administration. Well-controlled, large-scale prospective studies are warranted in future to compare P-CABs with PPIs.

Kerion celsi is severe inflammatory type of tinea capitis. It occurs almost exclusively in pre-pubertal children. We report a case of Kerion celsi occuring in a 79-year-old woman. Culture of infected hair on Sabouraud glucose agar showed Microsporum canis Farnily history and fungus study suggested the importance of indirect route in Microsporum canis infection.

5.Evaluation of Elecsys 2010 Electrochemiluminescent Immunoassay System.

BACKGROUND: Elecsys 2010 immunoassay system is based on the electrochemiluminescence immunoassay using a ruthenium (II) tris (bipyridyl) label. Since it was the first time to use the system in our laboratory, we would like to evaluate the analytical performances (precision, linearity and recovery rate) and correlation with radioimmunoassay (RIA) and microparticle enzyme immunoassay (MEIA) methods. METHODS: We used precicontrol tumor marker (TM1, TM2) for alpha-fetoprotein (AFP), prostatic specific antigen (PSA) and carcinoembryonic antigen (CEA), Precicontrol universal (Ul, U2) for triiodothyronine (T3) and thyroxine (T4), Precicontrol-TSH for thyrotropin (TSH) and pooled serum for the evaluation of precision and recovery rate. Patients' sera were used for the linearity and comparison study. RESULTS: The coefficients of variatron of Imprecision study were below; 4.0%, 8.7% and 10.2%, respectively in the within-run, within-day and between-day analysis. The recovery rates were 100.5%, 96.1% and 102.5%, respectively in T4, TSH, and AFP. The linearity were y=1.02x-0.182(r=0.99) for T4, y=1.01x+0.12 (r=0.99) for TSH and y=1.01x+0.54(r=1.00) for AFP. T3, T4, TSH, CEA and PSA results showed good correlation with RIA (r>0.90), but AFP showed r=0.88. Also, AFP, CEA and PSA results showed excellent correlation with AxSYM (r>0.99). CONCLUSION: Elecsys 2010 immunoassay system showed excellent precision, recovery rate, clinically acceptable linearity and good correlation with the results obtained by RIA and MEIA methods.

8.Effect of resin sealants on the reduction of microleakage in composite restorations.

The purpose of this study was to compare the ability of three resin surface sealants to prevent microleakage in Class V composite resin restorations. Forty Class V cavities with the occlusal margin in enamel and gingival margin in dentin were prepared on the buccal surfaces of sound extracted molars, and restored with composite resin. Restorations were randomly assigned into one of four equal groups (n = 10): a control group, without resin sealing, and three experimental groups in which margins were sealed with Fortify Plus, Biscover and Permaseal, respectively. Specimens were thermocycled, immersed in a 2% methylene blue solution for 4 hours, sectioned longitudinally, and observed the leakage at the occlusal and gingival margins. The result was analyzed using Kruskal-Wallis test, Mann-Whitney test and Wilcoxon signed rank test. In conclusion, the ability to reduce microleakage at occlusal margins was similar in all of three sealants. However at gingival margin, it depended on the type of used resin surface sealant. At gingival margin, control and Fortify Plus group showed statistically higher microleakage than PermaSeal group, and Fortify Plus group also showed higher microleakage than BisCover group (p < 0.05).

9.Control of Postpartum Bleeding by Rectal Misoprostols: A Report of 3 Cases.

Postpartum hemorrhage is an important cause of maternal mortality and morbidity. Especially uterine atony is the most common cause of postpartum hemorrhage. Conventional method to control postpartum uterine atonic bleeding is based on the use of oxytocin and ergot preparations. Prostaglandin F2alpha analogue such as carboprost can be used to promote contraction when these agents fail to produce uterine contraction. Prostaglandin E1 analogue, misoprostol has uterotonic effect by oral or vaginal administration. They are used to induce labor and first or mid trimester abortion. In postpartum uterine atonic bleeding, misoprostols cannot be used via oral or vaginal route. Recently we have experienced that postpartum uterine atonic bleedings unresponsive to conventional methods were controlled by rectal misoprostols. So we report these cases with a brief review of literatures.
