1.The Effeds of Accommodation and Repeated Tonometry on Intraocular Ptessure in Korean Adults.

As accommodation and repeated tonometry were known to change the level of intraocular pressure, sometimes they may give a misleading indication for the diagnosis and management of early glaucoma, ocular hypertension and low tension glaucoma. The object of this study was to investigate the effects of accommodation and repeated tonometry on intraocular pressure in Korean adults. Repeated tonometry was performed on 120 normal Korean eyes with visual acuity of 20/20 or more under controlled condition of accommodation and relaxation using Goldmann applanation tonometry and Schiotz tonometry. The results were as follows: 1. In repeated Goldmann applanation tonometry, a statistically significant reduction in mean intraocular pressure was produced during accommodation but not during relaxation. The maximum reduction of 3.25mmHg was produced during four minutes of accommodation and significant reduction of 2.01 mmHg was shown in first one minute of accommodation. 2. In repeated Schiotz tonometry, a statistically significant reduction in mean intraocular pressure was produced during accommodation but not during relaxation. The maximum reduction of 3.71mmHg was produced during four minutes of accommodation and significant reduction of 2.39 mmHg was shown in first one minute of accommodation.

2.A Case of Citrobacter Freundii Conjunctivitis.

We shall devide the specific types of conjunctivitis into four main categories: (1) infectious conditions, (2) allergic conditions, (3) irritative conditions, and (4) inflammations occuring in association with diseases of the skin and mucous membranes. Bacterial conjunctivitis is the most common types of infections conjunctivitis. The common causes of bacterial conjunctivitis are Moraxella, Hemophillus, Staphylococcus, Streptococcus, Pneumococcus, Gonococcus, Corynebacterium diphtheriae. The genus citrobacter is closely related to the Salmonella-Arizona group of Entrobacteriaceae. They are usually recovered as commensals or as secondary pathogens from patients with altered host defences. The most commonly involved sites are the urinary and respiratory tracts. A 6 month old Korean male was found to hyperacute conjunctivitis OU which had developed for about 15days. Citrobacter freundii was recovered from the conjunctival exudate through smear, culture and biochemical study. Beside his conjunctivitis OU, this child was physically healthy and not found to have immunodeficiency. The conjunctivitis was treated and recovered with topical 0.5% Gentamycin and amoxycyillin 400mg.

3.A Clinical Study on the Regional Ejection Fraction and Regional Wall Motion In Acute Myocardial Infarction.

Regional left ventricular dysfunction is common in patients with coronary artery disease and accurate analysis of regional dysfunction is of particular interest. For the quantitative assessment of regional dysfunction, we measured regional ejection fraction by radial sector division method in 19 patients with acute myocardial infarction and 13 normal controls who had multigated blood pool scan. And two independent observer analyzed regional wall motion using 5 point grading system in 18 patients undergoing radionuclide ventricular cineangiography. The results obtained were as follows : 1) Regional wall motion scores for the gated blood pool study agreed completely in 72 of 108 segments (66.7%) and agreed within 1 grade in 88 of 108 segments(81.5%) and agreement rate is lowest in the septal area. 2) Global left ventricular ejection fraction was 63.2+/-4.2% in normal controls, 36.6+/-6.8% in extensive anterior wall infarction group and 52.6+/-9.7% in inferior wall infarction group. The value of extensive anterior wall infarction group was significantly lower than that of inferior wall infarction group(p<0.005). 3) Regional left ventricular ejection fraction by radial sector division method in normal control group were as follows : area 1 ; 56.5+/-6.7%, area 2 : 77.9+/-4.8%, area 3 ;84.3+/-5.5%, area 4 : 76.8+/-6.6%, area 5 ; 84.7+/-7.6%, area 6 ; 85.9+/-11.2%, area 7 ; 75.5+/-12.3%, area 8 ; 74.9+/-14.0%, area 9 ; 75.5+/-8.8%, area 10 ; 54.2+/-11.0%, 11 ; 34.5+/-16.3, area 12 ; 37.1+/-18.0%. 4) Mean regional ejection fraction in 7 patients with anterior wall infarction showed significantly lower values in area 4 to area 8, and in area 2 to area 5 in case of inferior wall infarction group. 5) We thought that regional ejection fraction obtained by radial sector division method is valuable index for the management and evaluation of patients with coronary artery disease.

4.Difference between Nasal and Temporal Pupil Cycle Time of Koreans.

A small beam or slit of light focused at the pupillary margin will induce regular, persistent oscillations of the pupil. The period of these cycles is the pupil cycle time that can be easily measured and expressed in milliseconds. When the efferent part of the light reflex of the pupil is normal, determination of the period of these cycles in milliseconds provides a number which may be correlated with conduction time in the anterior visual pathway. Cox and Drews reoorded pupillary response to alternating half-field visual stimuli and found that stimulation of the temporal visual field (nasal retina) caused more pupillary constriction than did equivalent stimulation of the nasal field(temporal retina). These findings imply an asymmertric decussation of pupillomotor pathways in the midbrain in humans as well as greater sensitivity of the nasal retina. We measured nasal and temporal pupil cycle time on 200 normal eyes of Koreans by half-field stimulation of the nasal and temporal retina. And we found that: 1. The mean temporal pupil cycle time was 1071 +/- 92m sec, in right eye and 1068 +/- 81m sec, in left eye. The mean nasal pupil cycle time was 894 +/- 63m sec, in right eye and 899 +/- 57m sec, in left eye. Difference between nasal and temporal pupil cycle time was 177 +/- 73m sec, in right eye and 169 +/- 64m sec, in left eye. 2. The difference of pupil cycle time between the right and left eyes was 3 +/- 84m sec, in temporal retina and 5 +/- 61m sec, in nasal retina. The difference of pupil cycle time between the two eyes was not significant statistically. 3. The mean temporal pupil cycle time of the two eyes was 1069 +/- 85m sec, and the mean nasal pupil cycle time was 897 +/- 62m sec. Difference between nasal and temporal pupil cycle time was 172 +/- 74m sec. In 95% of normal population, the discrepancy between temporal and nasal pupil cycle time was shorter than 253m sec. 4. The mean temporal pupil cycle time was 1066 +/- 86m sec, in male and 1072 +/- 83m sec, in female. The mean nasal pupil cycle time 901 +/- 63m sec, in male and 894 +/- 59m sec, in female. The difference of pupil cycle time between the male and female was not significant statistically.

5.PCR-Based Sensitive Detection of Wood-Decaying Fungus Phellinus linteus by Specific Primer from rDNA ITS Regions.

Based on the rDNA ITS sequences data, specific primer set for PCR detection of wood-decaying fungus Phellinus linteus was designed. The length of PCR products using designed primer set(SHF and SHR) was about 540 bp. Among 11 species, 17 isolates of Phellinus spp. including Phellinus linteus, P. pomaceus, P. spiculosus, P. baumi, P. pini, P. igniarius, P. gilvus, P. biscuspidatus, P. weirii, P. johnsonianus, P. robutus, and P. igniarius, seven isolates of Phellinus linteus showed about 540 bp-sized single band. This molecular technique could offer a useful tool for detecting and identifying Phellinus linteus.

6.Effect of a Pholiota adiposa Extract on Fat Mass in Hyperlipidemic Mice.

The purpose of this study was to investigate the effect of a Pholiota adiposa extract on fat mass in hyperlipidemic mice fed on a high-fat diet. The water extracts from P. adiposa (ASI 24018) were not affected in the total triglyceride contents and epididymal fat mass in mice fed on a high-fat diet, but the retroperitoneal fat mass decreased significantly. This result suggests that the P. adiposa extract may be a potential candidate for use as a functional food that can act as a prophylactic against hyperlipidemia. However, the P. adiposa extract showed no effect in the total triglyceride contents and epididymal fat mass.

7.Antioxidant activities and determination of phenolic compounds isolated from oriental plums (Soldam, Oishiwase and Formosa).

The purposes of this study were to determine phenolic compounds and to evaluate antioxidant activities of plums (Soldam, Oishiwase and Formosa). Soldam contains the highest amount of total phenolics among cultivars (Formosa: 4.0%, Oishiwase: 3.3%, Soldam: 6.4% for total phenolic) as well as the total flavonoids of which constituents were mainly myricetin and anthocyanidin. The antioxidant activities were measured by DPPH, ABTS radical scavenging, and SOD-like activities. The DPPH radical scavenging activity of Korean plum extracts (200 microg/mL) showed more than 43%, and the Soldam turned out to be the highest : ID50 value: 160-177 microg/mL for Formosa and Oishiwase; 58-64 microg/mL for Soldam. The ABTS radical scavenging activity of Korean plum extracts (200 microg/mL) was found to be more than 50%. The SOD-like activity of Korean plum extracts (200 microg/mL) showed more than 70%. Among three kinds of cultivars, Soldam had the highest antioxidant activity. The nitrite scavenging activity of Soldam was 61.5%, which is the highest, compared with that of the other cultivars, about 50%. From these results, Korean plums turned out to be phytochemical rich fruit as well as to show high antioxidant activities.

BACKGROUND: Scutellaria baicalensis Georgi extract is used as a traditional herbal medicine. The efficacy of Scutellaria baicalensis Georgi extract is known for antioxidative activity, antiinflammation effect, antibacterial effect, inhibitory effect of melanin synthesis, sun protection effect, antiallergy effect, and etc. OBJECTIVE: We confirmed the cell viability or inhibitory effect of melanin synthesis in HaCaT (human keratinocyte cell line) and B16F10 (murine melanoma cell line) cells and the skin safety test through a clinical test (dermal irritation study) for Scutellaria baicalensis Georgi extract, according to the extraction methods. METHODS: We checked the cell viability, using MTT assay and inhibitory effect of melanin synthesis in B16F10 cells or HaCaT cells for thirty one Scutellaria baicalensis Georgi extract, according to the extraction methods. Then, we evaluated the skin safety for selected eight Scutellaria baicalensis Georgi extract through a primary dermal irritation test. RESULTS: Among the thirty one Scutellaria baicalensis Georgi extracts, according to the extraction methods, we selected eight Scutellaria baicalensis Georgi extracts that were not detected with cell toxicity in HaCaT cells and B16F10 cells, and could have inhibited the melanin synthesis in B16F10 cells. The selected eight Scutellaria baicalensis Georgi extracts identified the skin safety through a primary dermal irritation test. CONCLUSION: We expect clinical trials for whitening efficacy based on inhibitory effect of melanin synthesis and human skin safety for Scutellaria baicalensis Georgi extracts.

9.The Safety Evaluation of a Potent Angiogenic Activator, Synthetic Peptide (SFKLRY-NH2) for the Skin Application.

A novel synthetic hexapeptide (SFKLRY-NH2) that displays angiogenic activity has been identified by positional scanning of a synthetic peptide combinatorial library (PS-SPCL). This study was carried out to investigate the irritation of the SFKLRY-NH2 on the skin. The tests were performed on the basis of Korea Food and Drug Administration (KFDA) guidelines. In results, cell toxicity is not appeared for SFKLRY-NH2 in HaCaT cells and B16F10 cells. SFKLRY-NH2 induced no skin irritation at low concentration (10 microM), mild irritation at high concentration (10mM). We consider that this result is helpful for saying about the safety of SFKLRY-NH2 in clinical use.

10.A Case of Endoscopic Band Ligation Therapy for Gastric Bleeding in Patient with Rendu-Osler-Weber Disease.

Hereditary hemorrhagic telangiectasia (HHT), also known as Rendu-Osler-Weber disease, is a rare autosomal dominant disorder characterized by hemorrhage from nasal, mucocutaneous and gastrointestinal telangiectasia, in addition to vascular anomalies in other organs, particularly in the pulmonary, hepatic and cerebral circulations. The most common clinical manifestations are epistaxis and gastrointestinal bleeding. Endoscopic band ligation is effective for non-variceal, non-ulcer bleeding. It has the advantage of ease of use and is relatively inexpensive. We report a case of endoscopic band ligation therapy for gastric bleeding in patient with Rendu-Osler-Weber disease.
