Intracranial aspergillosis is rare and often fatal, and the brain is the third most frequent origine involved, precede by the lung and the gastrointestinal tract. Two routes of infection are considered important for infection of the central nervous system with aspergillus ; direct spread of infection, mostly within the paranasal sinuses, or hematogenous spread, usually originated from the lung. Paranasal sinus mucocele is a chronic expanding lesion lined by sinus mucosa and containing thickened mucus. Complication arise as a result of infection or expansion of mucoceles. Mucoceles that invade adjacent area, such as the orbits or anterior cranial fossa, can cause complication by a mass effect. The authors report their surgical experience of intracranial aspergillus abscess combined with large frontal mucopycele.

During the 6 year period, frorn May 1, 1989 to August 31, 1995, 40 cases of endometrial carcinoma were encountered at College of Medicine, University of Ulsan, Asan Medical Center. The purpose of this study is to investigate the patient's clinical characteristics and to correlate these findings with the there histopathologic results. The results were as follows : 1. There was an increasing tendency of endometrial carcinoma during 3-year time interval between May 1, 1989 and August 31, 1995. 2. Age distribution of the patients with endometrial cancer showed; 10% for the 31s ages<40, 27.5% for the 41.

7.Blood Alcohol Concentration and Traumatic Brain Damage.

Alcohol intoxication is often a contributory factor to the accidental head injury and may greatly aggravate the situation due to respiratory depression, cerebral edema and alteration in clotting mechanisms. Blood alcohol level was measured in 1261 patients with head injury attending the emergency room of Chung-Ang university hospital during one year from July, 1988. Blood alcohol concentration over 0.5g/L was defined as positive. The results are as followed : 1) Among 1261 patients of head injury, 89 patients showed positive blood alcohol level(7.05%). The mean blood alcohol concentration was 1.673g/L. 2) The male to female ration was 8.9 : 1 and majority of patients were 3rd and 4th decade. 76.6 percent of blood alcohol positive patients were attended between 8pm and 4am. 3) The common mechanisms of injury in blood alcohol positive group were pedestrian road traffic accidents(27.1%), fall down(24.7%) and home accidents(14.6%). 4) The level of consciousness on admission was significantly worse in the blood alcohol positive group than in the control group(P<0.01). 5) The common associated injuries were mostly in the extremities(22.5%) and chest(6.7%), but there was no significant difference in both group. 6) The prognosis was worse in the blood alcohol postive group than in the control group(P<0.01).

8.The Activity of c-Jun N-terminal Kinase (JNKb) in Patients with UIP.

BACKGROUND: TNF-alpha is related to the generation of lung fibrosis in patients with UIP. The precise mechanism leading to lung fibrosis by TNF-alpha is unknown. However, the activation of a transcription factor like AP-1(down stream of c-jun N-terminal kinase, JNK) by TNF-alpha may be related to the induction of fibrogenic cytokines like PDGF or IGF-I. Furthermore, JNK was reported to be activated in the radiation-in-duced lung fibrosis model. This study examined JNK activity in patients with UIP. METHODS: The expression of phosphorous JNK(p-JNK), macrophage/moncoyte specific markers, CD68, and cytokeratin was evaluated by immunohistochemical (IHC) staining of lung tissues from patients with UIP and lung cancer. An in vitro kinase assay was performed with alveolar macrophages obtained by a bronchollung cancer. An in vitro kinase assay was performed with alvolar macrophages obrtained by a bronchol avleolar lavage from patients with UIP and healthy persons as the control. RESULTS: The IHC stain showed that p-JNK is expressed in the almost all of the alveolar macrophages and smooth muscle cells in patients with UIP. In case of the normal areas of the lung from patients with lung cancer, the alveolar macrophages showed little p-JNK expression. Interestingly, increased JNK activity was not found in the in vitro kinase assay of the alveolar macrophages obtained from both patients with UIP and healthy persons as the control. Furthermore, 10 ng/ml of TNF-alpha failed to increase the JNK activity of the alveolar macrophages in both patients with UIP and healthy people. CONCLUSION: The JNK was activated constitutionally in patients with UIP. However, the role of JNK in the pathogenesis of lung fibrosis needs to be clarified.

9.Successfully Treated Obturator Hernia in Spite of Delayed Operation.

Obturator hernia is a rare type of pelvic hernia and occurs most commonly in elderly and debilitated women. It is still a challenge for surgeons to diagnose precisely in early stages because of its nonspecific symptoms and consequently delayed diagnosis could lead to high morbidity and mortality. We experienced a 92-year old patient who was diagnosed as obturator hernia which was confirmed by computed tomography scan of the abdomen and pelvis. The operation was delayed due to the refusal of family members but eventually done after 12 days from initial diagnosis. After manual reduction of small bowel impacted into right obturator foramen, segmental resection of impacted small bowel and anastomosis was done. The hernial defect was closed by primary closure with Dexon suture material. After the operation, the patient was discharged without significant complications. We report here successful results of delayed operation for obturator hernia.

10.Usefulness of CT Fluoroscopy-guided Percutaneous Needle Biopsy in the Presence of Pneumothorax During Biopsy.

PURPOSE: When pneumothorax occurs during a percutaneous needle biopsy, the radiologist usually stops the biopsy. We evaluated the usefulness of computed tomographic (CT) fluoroscopy-guided percutaneous needle biopsy in the presence of pneumothorax during biopsy. MATERIALS AND METHODS: We performed 288 CT fluoroscopy guided percutaneous needle biopsies to diagnose the pulmonary nodules. Twenty two of these patients had pneumothorax that occurred during the biopsy without obtaining an adequate specimen. After pneumothoax occurred, we performed immediate CT fluoroscopy guided percutaneous needle biopsies using an 18-gauge cutting needle. We evaluated the success rate of the biopsies and also whether or not the pneumothorax progressed. We classified these patients into two groups according to whether the pneumothorax progressed (Group 2) or not (Group 1) by measuring the longest distance between the parietal pleura and the visceral pleura both in the early and late pneumothorax. Additionally, we analyzed the relationship between the progression of pneumothorax after biopsy and 1) the depth of the pulmonary nodule; 2) the number of biopsies; 3) the presence or absence of emphysema at the biopsy site; and 4) the size of the pulmonary nodule. RESULTS: Biopsy was successful in 19 of 22 nodules (86.3%). Of the 19 nodules, 12 (63.2%) were malignant and 7 (36.8%) were benign. Twelve patients (54.5%) were classified as group 1 and 10 patients (45.4%) as group 2. The distance between the lung lesion and pleura showed a statistically significant difference between these two groups: < or = 1 cm in distance for group 1 (81.8%) and group 2 (18.2%), and > 1 cm in distance for group 1 (30%) and group 2 (70%), p < 0.03. Yet the number of biopsies, the presence or absence of emphysema at the biopsy site and the size of the pulmonary nodules were not related to the progression of pneumothorax (p > 0.05). CONCLUSION: When early pneumothorax occurs during a biopsy, CT fluoroscopy guided percutaneous needle biopsy is an effective and safe procedure. Aggravation of pneumothorax after biopsy is affected by the depth of the pulmonary nodule.
