1.The Relationship between Health Value Cognition, Health Promotion Behavior and Health Examination Results Among Transit Corporation's Workers.

OBJECTIVES: This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition. METHODS: The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers. RESULTS: In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group. CONCLUSIONS: In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

2.Inhibition of Phospholipase A2 Ameliorates the Acute Lung Injury Induced by E Coli Endotoxin via Reduced Production of Oxygen Free Radicals in the Lung.

BACKGROUND: In order to understand the pathogenetic mechanism of sepsis-induced acute lung injury, inhibition of phospholipase A2 (PLA A2) was carried out in an endotoxin-induced septic lung model. METHODS: Sprague-Dawley rats were divided three groups; sham group, endotoxin group (instillation of E coli endotoxin, 100microgram/rat, type 017) and mepacrine group (the non-specific PLA2 inhibitor, 50 ml/kg intraperitoneal injection after endotoxin treatment). Five hours after endotoxin treatment, protein contents, neurophils counts, gamma-glutamyl transpeptidase (GGT) activity and surfactant concentrations in the bronchoalveolar fluid (BAL), meyloperoxidase (MPO) and PLA2 activity in the lung were measured. A morphological study for the effect of the endotoxin and mepacirne, and a cytochemical electron microscopy for detection of hydrogen peroxide in the lung were also performed. RESULTS: Endotoxin increased the concentrations of protein, the number of neutrophils, and GGT activity in the BAL fluid, MPO and PLA2 activity in the lung but mepacrine decreased these parameters (P < 0.001). The light density of surfactant was increased by the endotoxin (P < 0.001), but mepacrine diminished this pathological change. In the light microscopic findings, the endotoxin caused pulmonary accumulation of neutrophils, atelectasis and transudation of intravascular protein was observed. In contrast, mepacrine lessened these pathological findings. In ultrastructural findings, adhesion of neutrophils to endothelial cells, necroses of type II cells and endothelial cells, and the damage of lamellar bodies were observed after the endotoxin treatment, which recovered with mepacrine. In the cytochemical electron microscopy for detection of hydrogen peroxide in the lung, the deposits of cerrous perhydroxide were increased by the endotoxin but mepacrine decreased deposits of cerrous perhydroxide. CONCLUSIONS: Inhibition of PLA2 in an endotoxin induced acute lung leak showed protection against oxidative stress by a diminution of neutrophilic respiratory bursts and a decreased production of free radicals. It is suggested that PLA2 has a pivotal role in causing acute oxidative stress in endotoxin induced acute lung injury.

4.Treatment of Acromioclavicular Separation and Fractures by Modified Kenny Howard sling-halter

There are many procedures for the treatment of acromioclavicular separation but there are still controversies concerning the best management of these injuries. The cases of 20 acromioclavicular separations and 3 distal end fractures of clavicle were treated by conservative method with modified Kenny Howard sling-halter using long arm cast at the department of Orthopedic Surgery, College of medicine,Ewha Womans University, during the thirty six months period from March 1982 to February 1985. The following results were obtained. 1. The average duration of immobilization was 5 weeks at the grade II of acromioclavicular separation and fracture of distal end of clavicle and 6 weeks at the grade III of acromioclavicular separation. 2. The functional results were 17 excellents(73.8%), 5 goods(21.9%), and 1 fair(4.3%). 3. The complications were 3 frozen shoulders(13.0%), 2 subluxations(8.6%), and 1 skin necrosis on the elbow(4.3%). 4. The authors immobilized the upper extremity with long arm cylinder cast of 90° flexion position instead of initial harness, so that it was applied easily and the patient felt very comfortable. 5. When the fracture and severe displacement were occurred at the midthird of the clavicle, it could be reduced and maintained with the long arm cylinder cast and strap.

5.A Clinical Study on Surgical Treatment of Spondylolisthesis

Spondylolisthesis, frequently encountered problems with increasing incidence, has been recognizid as an important clinical entity in orthopaedic field. Several methods were applicated as operative treatment of spondylolisthesis; posterior fusion or H-graft, posterolateral fusion, anterior fusion, fixation with internal devices and etc. During the past 12 years from March 1972 to February 1984, 34 patients among the 42 patients were treated surgically at Department of Orthopaedic Surgery of Ewha Womans University Hospital and following results were obtained. 1. The average age was 45.2 years(19 ~ 64 years) and female was predominent(64.7%). Isthmic type was 17 patients(50.0%), degenerative type was 13 patients(38.2%) and dysplastic type was 4 patients(11.8%). Below 40 year old, dysplastic and isthmic type were predominent and degenerative type was more common in the age group above forties. 2. The most prevalent site was L5-Sl; 21 cases(61.8%) and L4-5; 13 cases(38.2%) in orders. In isthmic type L5-Sl was 13 cases and in degenerative type, L4-L5 was 9 cases. By Meyerding classification, grade I was most common(26 patients, 76.5%). 3. The clinical pictures were low back pain only and little improvement with conservative treatment(type I: 9 patients), low back pain with sciatica(type II: 13), type II plus neurologic signs such as motor weakness, sensory change or change of D.T.R(type III, 10) and little low back pain with mainly sciatica(type IV, 2). 4. Of 34 patients, anterior interbody fusion were done in 10 patients ; posterior and posterolateral fusion with decompression were done in 22 patients. Only laminectomy were done in 2 patients(type IV). 5. Fusion rate was observed in 21 patients(95.5%) among 22 patients with posterior or posterolateral fusion and 8(80%) among the 10 patients with anterior fusion. Postopertive functional results by Gill's criteria were excellent or good in 31 patients(91%). 6. There were no difference in fusion rate and functional results in operative methods.

6.Clinical Evaluation on Diaphyseal Fractures of the Both Forearm Bones in Adults

Fractures of both forearm bones present specific therapeutic preoblems encountered infrequently in the fractures of other shafts of long bones. Although some of these fractures can be treated conservatively, majority of fractures are treated by open reduction and internal fixation for accurate anatomic reduction and rigid fixation. From January 1979 to December 1986, seventy two cases of both forearm bone fractures had been treated by four different methods at the Department of Orthopaedic Surgery, College of Medicine, Ewha Womans University and results were obtained as follows : 1. The most common fractures site was middle third of radius and ulna. 2. Comminuted fx was most common in radius and oblique fracture was most frequent. 3. Eleven cases were treated conservatively and 61 cases were treated by open reduction. Among them, 2 cases were fixed with intramedullary nailing, 36 cases by plating and 23 cases by combined method. 4. The time required for bony union was shorter in group of open reduction and plating than in groups of the other treatment. 5. The functional result was satisfactory in 53 cases(74%) and was best in group of open reduction and plating. 6. Among 5 cases of nonunion, 3 cases were developed in open fractures.

7.Clear Cell Sarcoma of Flexor Tendon in Wrist: A Case Report

While benign tumors of tendons and aponeuroses are observed commonly and have been defined adequately, malignant tumors originating from these tissues are rare. Among those reported most had been classified as synovial sarcoma or fibrosarcoma until Ezinger in 1965 described a new type that he called “clear cell sarcoma” of tendon and aponeurosis. Recently, Ultrastructural and histochemical analysis has revealed that the tumor cells contain melanin in the form of melanosomes in varying stages of development and currently, the neoplasm is considered a primary malignant melanoma of the soft tissue. A csse of clear cell sarcoma of flexor tendon in the wrist is reported, with an emphasis on its electron microscopic description.

10.Nine-Year Results of Excimer Laser Photorefractive Keratectomy for Myopia.

PURPOSE: To evaluate the long term stability, complications, and causative factors in eyes that had photorefractive keratectomy (PRK) for myopia. METHODS: We investigated the results of photorefractive keratectomy (PRK) on 72 consecutive myopic eyes (45 patients) with follow-up of 9 years by using Eximed UV 200LA laser (Summit technology, USA). The patients were divided into two groups according to preoperative myopia : group 1 with diopters between -3.25 D and -6.50 D, group 2 with diopters above -6.50 D. During the follow-up period, 1, 2, 3, 5, 7 and 9 years, refractive error, uncorrected and best corrected visual acuity and corneal opacity were examined. Preoperative myopia ranged from -3.25 D to -9.12 diopters (D), with astigmatism of less than 1.50 D. The excimer laser was set to a maximum correction of -6.50 D at a 5.0mm diameter ablation zone. Patients with more than -6.50 Dmm of myopia had double-pass PRK with two different ablation zone sizes (5.0 and 4.5 mm). The data were analyzed using paird T-test and Regression analysis. RESULTS: Mean refractive error 9 years after PRK was -2.10+/-0.99 D. It was -1.67+/-0.71 D in moderately myopic eyes (less than 6.50 D) and -2.60+/-1.04 D in highly myopic eyes (more than 7.0 D). The Myopic regression rate markedly decreased after 2years in moderate myopic group but after 3 years in highly myopic group (P<0.01). The most possible causative factor for myopic regression were pre-PRK refraction (P<0.01) and its relative risk was 4.21: At 9 years, 92.3% eyes achieved uncorrected visual acuity of 0.5 or better group1, and 78.7% in group2. Moderate corneal haze was present in 2eyes in group 2. CONCLUSION: According to these results, we thought that eximer PRK was effective for myopia under -9.62 D. Myopic regression was stabilized after 3 years of PRK.
