1.A Case of Left Marked Hydronephrosis Due to Aberrant Renal Vessels.

1. We have reported here a case of left marked hydronephrosis due to left aberrant renal vessel, which was treated by ablative procedure. 2. Aberrant renal vessel is a rare disease, but is a relatively common cause of uretero-pelvic obstruction with intrinsic ureteropelvic stenosis. Clinically it is important that this disease may he easily complicated with the conditions such as obstruction, hydronephrosis and infection, and also, the surgeon may inadvertently lacerate the structure with resultant profuse hemorrhage during the operation.

2.A Clinical Observation on Urolithiasis.

A clinical study was made on 85 cases' of urinary lithiasis among the number of 3795 out-patients, during 5 years and 8 months from January 1964 to August 1969, and 80 cases were analyzed by a chemical method. (70 cases were safely reserved stone, 10 cases were by litholapaxy.) The results were as following: 1. The incidence of urinary calculi was 2.2%, male to female ratio being approximately 3.7: l. 2 83 cases of urinary calculi were found in the Chonnam district and 2 cases were found in the Chonbuk district 3. The age of the patients ranged from 20~40 years in approximately 65.6%. 4 The location of the urinary calculi was in ureter in 47%, kidney (29.4%), bladder(14.2%) and urethra (9.4%). 5. The most favored predilection of ureteral stone was lower third of ureter in approximately 47% of all cases 6. In a number of urinary calculi in each location 83.5% (71 cases) was single. The most common sire, was below 0.5cm in its diameter, and weight, below 0.5Gm on 41 cases (51.25%). 7. The clinical symptoms of upper urinary tract lithiasis showed flank pain in 81.5%, hematuria in 35. 4%, and suprapubic discomfort in 30. 8%, ,but in lower urinary tract, painful urination was in 85%, urgency in 60%, stoppage of urinary stream in 65% and hematuria in 40%. 8. The microscopic hematuria showed 87.1%, which incidence was higher in lower urinary tract calculi, and pyuria was revealed in 39. 9%. 9. The most frequent surgical intervention was ureterolithotomy in 38.5% (35 cases). 10. The results of chemical analysis of 80 urinary calculi showed the mixed type of calcium-phosphate and calcium oxalate in 40% (32 cases), calcium oxalate in 12. 5% (10 cases), calcium phosphate in 12.5% (10 cases), the mixed type of calcium-phosphate and ammonium-phosphate in 10% (8 cases), calcium-carbonate in 10% (8 cases) and urate in 3.75% (3 cases). 11. Cases of bilateral urinary calculi were revealed in 13.8%.

4.A Case of the Centrineurogenic Acute Respiratory Distress Syndrome Following by Near-hanging Injury.

In suicidal hanging, a death often occurs within minutes. f rescued, most victims later succumb to the respiratory failure secondary to pulmonary edema, bronchopneumonia, delayed airway obstruction, and acute respiratory distress syndrome(ARDS). Out of these, non-cardiogenic pulmonary edema is likely due to neurogenic factors or negative intrathoracic pressure. A 36-year-old female was brought to the Department of Emergency Medicine after the suicidal hanging attempt. She was unconscious but responsive to painful stimuli. A laryngeal stridor was present. The lung sound of the patient's was dear. Fourteen hours after admission, a chest X-ray showed bilateral diffuse interstitial infiltrates in both lung fields, which was compatible with ARDS. The patient was intubated and placed on a volume respirator with PEEP. She improved over the next 48 hours and a repeat chest X-ray aim showed marked clearing of the infiltrates. We report on the pathophysiologic results of this pulmonary complication and the implication of this finding, regarding the treatment of the patient.

5.Statistical analysis of the prevalence and trend of obesity in elementary school children in Kwangju.

To estimate the prevalence and trend of obesity in primary school-age children, obesity survey was performend in children aged 6 to 12 years(5,226 boys and 5,048 girls)in Kwangju in 1991. Obesity was defined as overweight if more than 20 per cent above the median weight for height and age(relative weight>120%). The following results were obtained: 1) The prevalence of obesity that exceeded the weight centiles at each height by more than 97th percentile was 8.78. Boys (9.70%)was not significantly higher than in girls(7.82%)(p>0.05). 2) The prevalence of obesity that exceeded the standard weight for height by more than 20% was 8.82%. Among them, 5.45% were mild obesity and 2.93% were moderate obesity and 0.44% were severe obesity. The prevalence rate of obesity was no significant differences by sex(p>0.05). 3) Regionally, the prevalence of obesity was a lower rate in Kwangiu in 1991 (8.82%)than that in Seoul in 1989(11.68%).

6.Combined Pneumocystis Carinii Pneumonia and Miliary Tuberculosis in a Patient with AIDS.

Patients with AIDS frequently present with pulmonary complications which are associated with a high mortality rate and infections are the most important cause of lung infiltrates. In addition to pneumonia caused by Pneumocystis carinii, which was noted in early reports of the syndrome, a variety of other severe pulmonary disorders may occur. Frequently more than one organism is found in a single patient and among these, combined infections of Pneumocystis carinii and cytomegalovirus are the most common. We experienced a case of combined Pneumosytis carinii and Mycobacterium tuberculosis infection as a pulmonary manifestation of AIDS in a 38-year-old man. In bronchoalveolar larvage, bronchial washing and brushing, and sputum smear specimens, Pneumocystis carinii organisms were recognized, especially in Gomori's methenamine silver stains. Transbronchial lung biopsy specimen revealed intra-alveolar frothy exudates composed of collections of Pneumocystis carinii organisms as well as several granulomas with central caseous necroses.

8.Nutrient Intake of the Rural Elderly Living in Kyungnam -Focusing on Health and Aging Status, and Life-Satisfaction.

The purpose of this study was to investigate the difference in nutrient intake according to the level of self-perception of health status, aging status and life satisfaction of the rural elderly. The factors for the study were surveyed by interview method. The subjects were 270 people(71 male, 129 female) aged over 65 years(73.5 +/- 5.6ys) in the Ham-an area. The obtained results as follows : By evaluation of self-perception of health status, 57.5% of subjects answered they are in a bad health condition. The 91.5% of subjects had diseases(rheumatitis & arthritis 31.4%, cardiovascular disease 20.2%, gastric disease 10.2%). The women had more diseases than the men(p<0.01). The subjects took medical treatment in private hospital(40.5%) and public health centers(35.0%). The men showed better level of aging status(p<0.001) and life satisfaction index(p<0.01) than the worsen. Living with spouse influenced the aging status(p<0.05) and the more pocket money influenced life satisfaction(p<0.05) and aging status(p<0.05). The elderly who eat regularly 3 times a day(p<0.05) and have a good appetite(p<0.001) appeared to have positive effect on the self perception of health status and aging status. An increasing level of the self-perception of health status and regular exercise worked to improve aging status(p<0.001). The habits of smoking and alcohol drinking, however had no effect on any index. The self-perception of health status affected the nutrient intake, but only in female elderly. The aging status and the life satisfaction index related overall positively to the intake of nutrients. In conclusion, the study shows that gender did influence nutrient intake in the elderly. The women who live alone rated lowest in social resources and health condition therefore their nutrient intake was also extremely in deficit. For successful aging, a program for rural elderly is needed, i.e. actions to provide minimum economic life, food delivery and psychological/physical health care through regional public health centers.

9.A study of the Nursing Interventions performed by the ICU nurses to the patients with Cerebrovascular disorders.

The purpose of this study was to analysis of nursing interventions performed by the ICU nurses to the patients with cerebrovascular disorder practically from one university hospital in Seoul. The data were collected from 15 nurses with 86 cerebrovascular disorder cases from one ICU with the questionaire to write frequency of nursing care done by the surveyee from May. 2, 2000 to July, 3, 2000 and the list of 66 nursing interventions selected out of 433 NIC(Nursing Interventions Classification) of Iowa University which were translated into Korean(44 items) and core nursing interventions by ICU nurses(22 items: Kim, Su-Jin, 1997). The data were analysed with SPS8 program. The results are as follow: 1. The most frequently used nursing interventions were vital sign monitoring, fall prevention, cerebral edema management, dysreflexia management, neurologic monitoring, cardiac care, communication enhancement, technology management, bed rest care. respiratory monitoring in rank. 2. The most frequently used nursing intervention domains were 'Physiological: Complex', 'Physio-logical:basic', 'Behavior', 'Safty', 'Health system' in rank. In the domain of physiological: basic, the most frequently used nursing interventions were bed rest care, urinary elimination management, tube care: urinary, physical restraints in rank. In the domain of physiological: complex, the most frequently used nursing interventions were cerebral edema management, dysreflexia management, neurologic monitoring, cardiac care in rank. In the domain of behavior, the most frequently used nursing interventions were communication enhancement. touch, active listening in rank. In the domain of safty. the most frequently used nursing interventions were vital sign monitoring, fall prevention in rank. In the domain of health system, the most frequently used nursing interventions were technology management. specimen management in rank. 3. some difference of the frequency practicing the nursing interventions according to the shift of duties was found. For example, medication administration was common at day duty, touch was practiced at evening duty, temperature regulation was performed.

10.A Study on Korean Adults' Health Status, Smoking Perception, and Self-efficacy of Smoking Cessation according to Smoking Status.

PURPOSE: The present study purposed to investigate Korean adults health status, smoking perception. and self-efficacy of smoking cessation according to whether smoking or not. METHODS: This study conducted a questionnaire survey of 694 adults at D County in Korea during the period from the 10th of June to the 10th of July 2004. The instruments used for this study were scales on health status, smoking perception and self-efficacy: of smoking cessation. Data was analyzed by frequency and percentage, mean and SD, chi2-test, t-test, ANOVA using SPSS 11.0 program. RESULTS: The subjects of this study were 503 men and 191 women, and 60.8% of the male subjects and 6.8% of the female subjects were smokers. With respect to health status (t=-2.58, p=0.010), smoking perception (t=10.59. p<0.000) and the self-efficacy of smoking cessation (t=-23.04, p<0.000), there were significant differences according to whether smoking or not. CONCLUSIONS: It is necessary to make repeated study to find other variables that affect adults smoking and to develop smoking cessation programs that enhance the self-efficacy of smoking cessation.
