1.Serous Cystadenoma of the Pancreas: A case report.

Serous cystadenoma of the pancreas, also known as microcystic adenoma or glycogen-rich cystadenoma, is an unusually benign tumor. It is usually large and composed microscopically of many small cysts lined by small, cuboidal or flattened cells containing abundant glycogen. It has been suggested that serous cystadenoma probably arise from the ductular cells or centroacinar cells. Herein, we report on a case of serous cystadenoma of the pancreas in a 55-year-old female. The tumor, measuring 13.5x11.5x10.0 cm, was located in the head of the pancreas and the cut surface revealed a sponge-like appearance due to innumerable tiny cysts containing clear serous fluid. Microscopic analysis showed cystic spaces lined by cuboidal cells with intracytoplasmic glycogen.

Local cutaneous reactions have been reported at injection sites of interferon therapy, but these are usually erythema or rarely induration. Skin necrosis at the injection site is rare. We describe here a patient with multiple sclerosis who presented with cutaneous necrosis at the injection sites of interferon-β. Biopsy of the necrotic lesion showed dermal vessel thrombosis and complete ischemic coagulative necrosis of epidermis and dermis.

5.Phytosphingosine-1-phosphate stimulates chemotactic migration of L2071 mouse fibroblasts via pertussis toxin-sensitive G-proteins.

Phytosphingosine-1-phosphate (PhS1P) was found to stimulate an intracellular calcium increase via phospholipase C but not pertussis toxin (PTX)- sensitive G-proteins in L2071 mouse fibroblasts. PhS1P also activated ERK and p38 kinase, and these activations by PhS1P were inhibited by PTX. Moreover, PhS1P stimulated the chemotactic migration of L2071 cells via PTX-sensitive Gi protein(s). In addition, the PhS1P-induced chemotactic migration of L2071 cells was also dramatically inhibited by LY294002 and SB203580 (inhibitors of phosphoinositide 3-kinase and p38 kinase, respectively). L2071 cells are known to express four S1P receptors, i.e., S1P1, S1P2, S1P3, and S1P4, and pretreatment with an S1P1 and S1P3 antagonist (VPC 23019) did not affect on PhS1P-induced chemotaxis. This study demonstrates that PhS1P stimulates at least two different signaling cascades, one is a PTX-insensitive but phospholipase C dependent intracellular calcium increase, and the other is a PTX-sensitive chemotactic migration mediated by phosphoinositide 3-kinase and p38 kinase.

6.Congenital Syphilis: An Uncommon Cause of Gross Hematuria, Skin Rash, and Pneumonia.

Although congenital syphilis can be prevented with prenatal screening, the disease remains problematic. Currently, there are no cases that describe hematuria and pneumonia related to congenital syphilis. We report a case of congenital syphilis that involved nephrotic syndrome and pneumonia alba in a 22-day-old male infant whose mother did not receive adequate prenatal care. The congenital syphilis diagnosis was confirmed with a serologic test and the patient recovered with penicillin treatment. Clinical findings may be subtle in neonates and delayed recognition occurs frequently, thus complete prenatal screening is critical for congenital syphilis prevention. Immediate serologic testing should be performed to obtain a differential diagnosis if an infant is delivered by a mother that has not received appropriate prenatal examinations.

7.Effects of Self Efficacy Promoting Programs on Self Efficacy, Self Care Behavior and Psychosocial Adaptation in Patients with a Colostomy.

PURPOSE: This study was conducted to investigate the effects of self-efficacy promoting program on self-efficacy, self-care behavior and psychosocial adaptation in patients with a colostomy. METHOD: A non-equivalent control group pre test post test design was used. The self efficacy promoting programs was composed of a CD image program based on varicaious experience, education and telephone coaching program based on verbal persuasion, and Stoma care practice, and Self care performance based on performance accomplishment. This study was carried out from July 2, 2005 to April 20, 2006 and 21 patients with a colostomy at one of 2 hospitals participated. Descriptive statistics, chi-square-test, and Mann-whitney U test were used to analyze the data. RESULTS: There were significantly different for specific self efficacy, self care and psychosocial adaptation between the experimental group and control group. CONCLUSION: The self efficacy promoting program for patients with stomas was effective in improving degree of specific self efficacy, self care and psychosocial adaptation.

8.Aspectual Comparison of the Skin Changes in Hairless Mice According to the Aging Type.

The purpose of this study was to compare the skin changes in female SKH-1 hairless mice between UVB irradiated photo-aged group and endogenous aged group. The UVB irradiation and endogenous aged groups showed poor skin conditions when compared with normal (N) group in terms of the skin erythema, water content and TEWL (transepidermal water loss). For the changes in gross observation and replica image analysis on wrinkle of the skin tissue, UVB irradiation group showed thicker, wider and deeper wrinkles than the changes seen in N group, whereas endogenous aged group showed thinner, narrower and shallower wrinkles than that of UVB irradiation group. In histopathological findings, UVB irradiation group and endogenous aged group showed thickened epidermis, increased dermal inflammatory cells, decreased collagen and elastic fiber content, increased number of degranulated dermal/subcutaneous mast cells, and lower expression quantity of TGF-beta in dermal layer when compared with N group, but to a lesser extent in aged group than the changes in UVB irradiation group. UVB irradiation group and endogenous aged group showed significantly higher xanthine oxidase activity, lower superoxide dismutase and catalase activities, and higher expression of MMP-3 mRNA in skin than N group. Therefore, aspectual comparison of the skin change in hairless mice between photo-aged and endogenous aged groups showed different each other, and these results will be useful for skin aging research.

9.A Survey of Breast-Feeding in Jeon-ju City Area.

PURPOSE: While the rate of breast-feeding has been rising recently in many countries, it has been gradually falling in Korea. Accordingly, we took a survey in Jeon-ju to discover the actual state of breast-feeding and find a solution. METHODS: Eight hundred and sixty-four questionnaires of filled out by mothers who visited the seven local pediatric clinics and the clinic of pediatric department in the Presbyterian Medical Center(PMC) in Jeon-ju from January 1998 to June 1998 were analysed by ANOVA, cross analysis, frequency analysis and correlation analysis using SPSS program. RESULTS: In the case of six-months-old babies, the breast-feeding rate was 35.6%. The breast-feeding rate was higher when it came to a normal delivery, and was also related to the inperiority of infant birth order and lack of maternal academic background(P<0.05). The main reasons to switch from successful breast-feeding to formula-feeding before 6 months of age were insufficient breast-milk(49.3%). The survey shows the best thing for a infant's health is breast milk (86.0%). CONCLUSION: In order to increase the rate of breast-feeding, it's essential to educate people in school and it is necessary to create a hospital environment in which mothers can easily begin breast-feeding as soon as possible after delivery.

10.Effect of High Doses of Propofol on Myocardium in the Isolated Rat Heart.

BACKGROUND: Propofol, an intravenous anesthetic agent, is known to have potential beneficial cardiac effects through a cardiac L-type calcium channel blocking effect and free radical scavenging activities in a number of myocardial stunning models at clinical concentrations. We investigated the effect of high doses of propofol on myocardium in the isolated rat heart to find out whether it has a cardiac protective effect on a normal, not stunning, conditioned heart. METHODS: Twenty male Sprague-Dawley rats were put under general anesthesia to monitor the heart and perfused with a modified Krebs solution with a Langendorff model. After 30 minutes of stabilization, isolated rat hearts were given 0.5, 1, 5, 10, 50, 100 and 250 micrometer of propofol in an increasing manner for 10 minutes, respectively. At every period, left ventricular pressure, dp/dt, heart rate and coronary flow were recorded and Na+, K+, Ca2+, Mg2+ and protein leakage in the effluent perfusate were measured to observe the myocardial cell damage. RESULTS: Significant negative inotropic and chronotropic effects were seen at higher than 100 micrometer. Propofol dose-dependently increased coronary flow at concentrations of 100 micrometer or lower. Marked potassium and protein leakage in the effluent perfusate were found at a higher concentration of 100 micrometer propofol. CONCLSIONS: These findings suggest that a high dose of propofol exerts direct myocardial depression and myocardial cell damage, instead of a protective effect, in the isolated rat heart.
