1.Transcatheter arterial chemoembolization of hepatocellular carcinoma with portal vein invasion.

Transcatheter arterial chemoembolization(TACE) is an imperative method for the managment of inoperable hepatocellular carcinoma(HCC). It is well known that primary HCC frequently invades the portal venous system and forms a tumor thrombus obstructing the portal blood flow which makes unfavorable prognosis of patiebt. We retrospetively reviewed 58 patients who reveived TACE(minimum 3 times) of HCC invading into portal venous system. Group 1(n=29) which showed peripheral portal vein invasion had better clinical and laboratory response. Group 2(n=17) which showed first order portal branch invasion had similar response to Group 3(n=12), which had main portal invasion. Group 1 showed no difference in survival time between TAC and TACE, but, in Group 2 and 3, emboliation with chemotherapy made longer survival than chemotherapy only. Clinical level of AFP was meaningful in Group 1 and 2 as decreasing value. Our results provides that careful selection of TACE and case by case Coil/Gelfoam embolization can improve the mean survival and clinical response when HCC evidently invades portal venous system.

4.Effect of azithromycin on gingival overgrowth of organic transplanted patients.

Management of gingival overgrowth due to medication is bothersome because plaque control measurement alone did not prevent gingival overgrowth. The best treatment of drug induced gingival overgrowth is discontinuing use of the associated drugs. In this report we attempt to evaluate the short term effects of azithromycin which shown to be of some benefit on gingival overgrowth due to medication. We studied 16 organic transplanted patients who had the gingival overgrowth. All patients received oral azithromycin once daily for 5 days. We measured papillary overgrowth index(POI) every interdental areas before treatment and after 2 and 4 week. Severity and frequence of the papillary overgrowth is tend to reduce progressively in 2 to 4 weeks after treatment in all patients. But no case is completely resolved within 4 weeks. We could find the partial resolution of gingival overgrowth with a 5 day treatment of azithromycin. It is not known whether the response to azithromycin was mediated through its antibacterial effect or another mechanism. We suggest that the treatment of azithromycin could be added to periodontal management of patients with gingival overgrowth due to medication.

6.The Effects of Follicular Fluid on in Vitro Maturation of Bovine Follicular Oocytes.

The present experiments were conducted to evaluate the effects of follicular fluid on maturation of bovine follicular oocytes. TC medium 199 seemed to be in adequate for this purpose since a high proportion (ranging 84.1. to 97.0%) of the oocytes were able to resume meiotic division in both media-with or without the addition of follicular fluid. This implies a possible similarity between TC medium 199 and follicular fluid with regard to the components initiating meiosis. Actually, TC medium 199 contains amino acids, vitamins and carbohydrates most of which are also found in follicular fluid. For this reason, the effect of follicular fluid on the ovum maturation was investigated by applying Krebs-Ringer bicarbonate solution (SECM), which was main1y composed of the salts, pyruvate and lactate. When the oocytes were cultured in SECM without the addition of follicular fluid, only 7-14% of them resumed meiotic division within 30 hours. However, when follicular fluid was added, the proportion of oocytes undergoing maturation was sharply increased to about 70%. Among the groups cultured in media containing different concentrations of follicular fluid, the proportion of the oocytes in meiosis remained constant, In pure follicular fluid, 87-89% of the oocytes showed enhancement of meiotic division. The presence of the follicular fluid contributed to a decrease in the production of degenerative oocytes. As a consequence it has been noted that addition of follicular fluid to the culture medium provides a more beneficial environment for cow oocytes. Meiotic division is initiated and production of degenerative oocytes is inhibited.

7.CT findings of intraventricular tumor.

About one tenth of all CNS neoplasms involves the brain. Due to their location in the ventricles they often present similar nonspecific clinical manifestation. Localization and differential diagnosis are dependent on radiological investigation. For the identification of specific CT characteristics of the intraventricular tumors and the differental diagnosis, we retrospectively analyzed 22 pathologically proved cases seen on CT. Important differential features included age and sex of the patient, the location within the ventricle, and the morpholgic appearance of the mass and density on CT before and after intravenous administration of contrast material. Meningiomas (4 cases) and a germinoma showed increased density on the precontrast CT scans, and demonstrated dense uniform enhancement of the postenhanced scan. Choroid plexus papillomas (3 cases) showed dense uniform contrast enhancement. Intraventricular neurocytomas (3 cases) demonstrated characteristic attachment to the septum pellucidum, confinement of the lateral and third ventricle, and calcification within the mass Colloid cysts (2 cases) showed characteristic location of anterosuperior aspect of the third ventricle. In conclusion, CT findings of intraventricular tumors are usually nonspecific. The location of the mass and the patient's age are the most helpful information in the differential diagnosis.

8.Mesenteric and Omental Cyst: CT Findings.

PURPOSE: Mesenteric and omental cysts are uncommon lesions found all age groups. They elicit interest because of their unclear pathogenesis and confusing terminology. MATERIALS AND METHODS: CT findings of 12 case with mesenteric and omental cysts were described and compared with surgical and pathologic findings. RESULTS: In mesenteric and omental cyst, the histologic diagnoses were lymphangioma(7 cases), nonpancreatic pseudocyst(3 cases), mesothelial cyst(2 cases). Lymphangiomas were usually multiloculated with enhancing wall, located in the small bowel mesentery. And these cystic lesions were frequently attached to bowel and required resection of a bowel segment. In three cases of pseudocyst, thick and enhancing wall was shown in unilocular cyst. Two cases of mesothlial cyst were located in greater omenturn, showed very thin wall in unilocuation. CONCLUSION: The CT features of the mesenteric and omental cysts are fairly characteristic. Identification of lymphangioma, which shows a multilocuation and enhancing wall, is important due to frequent bowel resection in operative field.

The diagnostic possibility and the incidence of ectopic pregnancy are increasing due to several factor. Furthermore, radioimmunoassay of serum β-HCG and US make it possible to diagnose early before the tubal rupture. A conventional surgical approach has been considered as a choice of treatments. Until recent years, however, surgery may have considerable risk of infertility and sugical hazard. In view of the natural tendency of some ectopic pregnancies to terminate in tubal abortion or complete resorption, we tried to cure ectopic pregnancy with nonsurgical transcervical intervention. Five of 8 patients with unruptured tubal pregnancies, it was possible to inject PGF2a into the tubes or gestational sac. 4 of them were ended up with satisfactory reduction of serum β-HCG level and improvement of symptoms & signs. In only one patient, β-HCG level remained a high level continuously, so salpingectomy was done. As a result, we consider that fluoroscopy-guided transcervical interventional procedure is a useful treatment modality for early unruptured ectopic pregnancy.
