This study was conducted to determine the correlation between weighted needle pinprick sensory threshold(PPT) and sensory nerve conduction tests. The subjects were 53 healthy controls, 31 diabetic patients without peripheral neuropathic symptoms(DM) and 36 diabetic patients with peripheral neuropathic symptoms(DN). PPT was measured on the index and little fingers, bilaterally, as well as under the lateral malleolus, bilaterally. In electrophysiologic assessment the left and right median, ulnar and sural nerves were studied. Each mean PPTs was high in order of controls, DM and DN. Age adjusted PPT was significantly different among three groups on right little finger(p<0.05) and left malleolus(p<(0.05), but not significantly different between DN and DM on other sites. Each sensory nerve conduction velocity and amplitude was statistically significantly different among three groups(p<0.05). Correlations of PPT with sensory nerve conduction velocity and amplitude were statistically significant on each site and ranged from -0.4203(left malleolus) to -0.5649(right index finger) and from -0.3897(left index finger) to -0.6200(right index finger), respectively. When electrophysiological study is not feasible, measurement of PPT may be helpful for the assessment of peripheral sensory neurological function.

The spotted grouped pigmented nevi is one of special forms of non-giant congenital melanocytic nevi. Histologically the nevus cells tend to proliferate around the skin appendages. We have experienced a case of spotted ground pigmented nevi in a 11 year old girl who had a pigmented plaque consisting of grouped brown black regularly distributed papules on the left inguinal region since birth. Histology revealed intradermal nevus. The skin appendages, especially the hair follicles and eccrine sweet ducts were enveloped by the neuvs cells. Electron microscopic study revealed that this nevus cells were mostly of type B cells. We would like to consider that the spotted grouped nevi are closely related pathogenically with the skin appendages.

It has been established that total hip replacement arthoplasty is one of the most excellent treatment modalities of adult hip joint disease. But aseptic loosening of the acetabular component is a main cause of revision and acetabular defects must be supported to engage the acetabular cup. Many authors have been managed this problem with the various methods of using acetabular ring, bone cement, autogenous or allogeoous bone graft and bipolar endoprosthesis. As the time goes on, it is inevitable that the numbers of revision arthroplasty cases will increase. We observed 36 patients, 39 cases of revision arthroplasty from 1981 and analysed the cases. 27 cases were treated by autogenous lilac block bone to support the acetabular defects. Complete or incomplete iliac block bone was grafted to superior, medial and posterior aspect of acetabulum. Some screws were inserted when they are needed for bone graft fixation. Hip spica casts were applied in 24 cases for 2 or 3 months and crutches were used for about 3 months. Pre-and postoperative hip joint functions by Harris score was increased form average 58.2 to 81.3(23.1 points increment). Sixteen cases were followed up for more than one year and all the bone grafts were united well without bone resorption. We found that autogenous iliac block bone can support the acetabular defects satisfactorily in revision arthroplasty cases.

Kaposi's sarcoma is a rare tumor comprising 0.1 per cent of all malignancies worldwide. There is, however, an increased ineidence following renal transplantation, immunosupression and in the acquried immunodeficency syndrome(AIDS) Kaposi's sarcoma has been shown to involve every organ of the body except the brain. Gastrointesinal involvement is the most common extracutaneous site of involvement. Gaatrointesinal Kaposis sarcoma is preaent in approximately half of patients with cutaneous Kaposis sarcoma and the acquired immune deficiency syndrome(AIDS). Although usually asymptomatic, gastrointestinal Kaposi's sarcoma may cause pain, bleeding, diarrhea, obstruction, intussusception, perforation. malabsorption, and protein-losing enteropathy. Three distinct gastroscopic appearances of Kaposi's sarcoma have been described: maculopapular, polypoid, and umbilicated nodular lesions. We report a case of Kaposis sarcoma of the stomach.

The potential of malignant melanoma to metastasize to all parts of the body is well known. Metastatic melanoma of the stomach may present with vague gastrointestinal symptoms, abdominai pain, or gastrointestinal bleeding. When gastrointestinal symptoms occur in a patient with known melanoma, gastric metastases should be considered. Melanoma metastatic to the stomach develops multiple small ulcerating masses. These sharply delineated submucosal lesions have been described as having a "bull's eye" or "target" configuration. Barium X-ray study, endoscopy, cytology study, and biopsy may yield the diagnosis. The prognosis is poor. We report a case of metastatic melanoma of the stomach with the review of the literature.

BACKGROUND: It is well known that excessive thyroid hormone in the body is associated with bone loss. However, the mechanism by which thyroid hormone affects bone cell metabolism remains unclear. It has been shown that thyroid hormones stimulate osteoclastic bone resorption indirectly via some unknown mediators secreted by osteoblasts, This study was undertaken to determine if interleukin-6 (IL-6) or interleukin-11 (IL-l1) could be the mediator (s) of thyroid hormone-induced bone loss. METHODS: We treated primary cultured human bone rnarrow stromal cells with 3,5,3-triiodo-thyronine (T) and measured basal and interleukin-l (IL-1)-stimulated IL-6/IL-ll production. We also investigated the possible modulating effect of 17B-estradiol (17B-E2.) on thyroid hormone action. RESULTS: T3 at 10 (-12) ~ 10 (-8) M concentration, significantly increased the basal IL-6 production in a dose-dependent manner, and also potentiated the stimulatory effect of IL-1 on IL-6 production. However, T failed to elicit a detectable effect on basal or IL-1-stimulated IL-11 production. Treat#ment with l7B-E2. inhibited IL-1-stimulated IL-6 production, but the effects of T3 on IL-6 production were not affected by 17/B-E. CONCLUSION: These results suggest that thyroid hormone may increase bone resorption by increasing basal IL-6 production and potentiating IL-1-induced IL-6 production from osteoblast-lineage cells, and these effects were independent of estrogen status.

PURPOSE: Tetanus toxoid specific IgG titers were measured in 76 cases on children ranging from neonates to 18 yr old adolescents according to the frequency of DPT vaccination. METHODS: The Enzyquick tetanus Kit was used to measure tetanus toxoid specific IgG titers by the ELISA method. RESULTS: 1) In infants less than 2 months-olg, the mean tetanus toxoid specific IgG titer was 1.07 IU/ml. 2) In infants ranging from 2 months to 12 months of age, the mean was 3.59 IU/ml in 11 infant with primary vaccination, and 2.05 IU/ml in 2 infants with out vaccination. 3) In children ranging from 1 year to 2 years of age, the mean was 3.60 IU/ml in 4 children with booster vaccination, and 1.43 IU/ml in 4 children without bosster vaccination. 4) In children ranging from 2yrs to 4 yrs of age, the mean was 2.60 IU/ml in 11 children with booster vaccination, and 1.03 IU/ml in 5 children without booster vaccination. 5) In children ranging from 4 years to 7 years of age, the mean was 3.64 IU/ml in 7 children with booster vaccination, and 0.76 IU/ml in 5 children without vaccination. 6) In children ranging from 7 years to 11 years of age, the mean was 1.19 IU/ml in 4 children with booster vaccination, 0.44 IU/ml in a ten years old gir 1 without booster vaccination, and 0.93 IU/ml in 3 children whose vaccination history was unknown. 7) In the 11 year old to 18 year old age group, The mean 1.23 IU/ml in 5 children with booster vaccination, and 0.84 IU/ml in 8 children whose vaccination history was unknown. CONCLUSIONS: The antibody titers for tetanus toxoid is adquate to protect tetanus infection, if DPT vaccination is performed as scheduled. But the antibody titers fell rapidly, when the booster vaccination was not performed after primary vaccination. In conclusion, the booster accination after primary vaccination is necessory for the prevention of tetanus infectioin.
