1.A Case of Myocardial Infarction caused by a Variant Angina during Treatment with beta-blocker of Intramural Hematoma.

Variant angina is characterized by repeated attack at rest associated with ST-segment elevation on ECG and caused by the spasm of coronary artery. But, the pathogenesis of spasm is not well known. A 44-year old man was transferred for the management of intramural hematoma at descending thoracic aorta and uncontrolled hypertension. We started to control hypertension with nitroprusside, propranolol, amlodipine, and doxazocin. At 4th hospital day, severe chest pain, dizziness, and diaphoresis were developed, and ECG showed not only ST-segment elevation on lead II, III, aVF but also 2 degree AV block(Mobitz type II). CK-MB revealed 52.3 ng/dl. When coronary angiography performed emergently, it showed total occlusion of right coronary artery (RCA) and diffuse minimal narrowing of left anterior descending coronary artery (LAD). After nitroglycerin was infused via right coronary catheter, the RCA was opened completely, and reperfusion arrhythmia was developed. Medication were changed to nifedipine, diltiazem, nicorandil, isosorbide mononitrate and he had no more chest pain.

4.Treatment of Subtrochanteric Fractures of the Femur with Zickel-Nail

The treatment of subtrochanteric fracture of the femur is one of the most difficult problems. Open reduction and rigid internal fixation is the choice of treatment nowaday. Zickel(1967) reported a new fixation device to treat subtrochanteric fracture of the femur which was designed to provide more rigid fixation in both major fragments, control angulation and rotation, and permit earlier ambulation. The authors reviewed 14 cases of subtrochanteric fracture of the femur which were treated with Zickel-Nail from June, 1979 to November, 1980. The results were obtained as follows: 1. The average age of patients was 44.7 years. 2. In 10 cases without associated injury, parallel-bar walking was possible within postoperative one week. In 8 cases of these, crutch walking with partial weight bearing was possible within postoperative 2 weeks. In others, there were some benifits in bed-side care because of rigid internal fixation which was provided with Zickel-Nail. 3. There were no significant postoperative complicatons except 3 cases of technical error. 4. There was no limb shortening except one case which was old complicated nonunion with severe osteoporosis and had a new fracture during Zickel-Nail procedure. 5. There were no differences in radiological bony union between pathologic fracture and traumatic fracture except one case with liver cell carcinoma of subtrochanteric region of the femur which we could not do follow-up study.

7.Clinical analysis of Acromioclavicular and Coracoclavicular Ligament Injury

The acromioclavicular and coracoclavicular ligament are usually injured by traffic accident or during sports. So, various methods of treatment has been introduced. Fourty four cases of acromioclaviclar and coracoclavicular ligament injuries were admitted and treated at Koryo General Hospital from March 1985 to February 1989. 26 cases were treated by operative method, 18 cases by conservative method in Grade 1, 2 and 3 injuries. The following results were obtained. 1. These injuries were more prevalent in male and more in left side. 2. The most common cause of injuries was traffic accident. 3. Among 44 cases, 13 cases were Grade 1, 9 cases Grade 2 and 22 cases Grade 3 by Allman's classification. 4. 18 cases were treated conservatively and 26 cases operatively. Conservatively treated 18 cases include 15 cases by Kenny-Howard sling and 3 cases by skillful neglect. And operatively treated 25 cases include 12 cases by modified Phemister method(46.2%), 2 cases by modified Bosworth method(7.7%), 2 cases by acromioclavicular plate method(7.7%), 2 cases by Stewart method(7.7%) and one case by Mumford method(3.8%). 5. There were good functional results with the K-wire fixation method after the acurate diagnosis by operative method in the Grade 2 injury. And in the Grade 3 injury we could get the functional results with the modified Phemister method.
