2.Cutoff Serum Prostate Specific Antigen Values by Age Decade in Men Clinically Free from Prostate Cancer.

In order to define PSA levels by age decade in men without clinical evidence of prostatic adenocarcinoma, so called non-cancer, Tandem-R assay for PSA was performed in male patients older than 40 years who had visited to our department. We defined the non-cancer population as men with a normal PSA value( 4.0ng/ml or less) and negative digital rectal examination findings. Furthermore, patients with an abnormality in either PSA value or suspicious digital rectal examination underwent transrectal ultrasound and pathologically excluded prostatic cancer from the population by transrectal prostate biopsies or transurethral resection. A total of 203 men entered the protocol, of whom 192 satisfied our criteria of non-cancer population. New mean, standard deviation and cutoff serum PSA values by age proposed as mean + 2 standard deviations were derived as follows, 0.84 0.68ng/ml( 2.2ng/ml) in the 40 to 49-year group, 2.03 + 1.36ng/ml( 4.8ng/ml) in the 50 to 59-year group. 2.02 + 1.7lng/ml( 5.4ng/ ml) in the 60 to 69-year group, 3.16 + 1.91ng/ml( 7.0ng/ml) in the 70 to 79-year group and 4.52 + 4.03ng/ml(12.6ng/ml) in the 80 to 89-year group.

4.Treatment of Renal Injury in a Patient Presenting Pituitary Diabetes Insipidus Associated with Bilateral Hydronephrosis: A Case Report.

We report the case of a 24-year-old man suffering from a pituitary diabetes insipidus associated with massive urinary tract dilatation with infravesical obstruction. The patient had been admitted and diagnosed to bulbous urethral stricture in our department. Endoscopic internal urethrotomy had been performed for his urethral stricture, but his abdominal distention had not been improved. About 1 year later, he was revisited to emergency room due to painful gross total hematuria and abdominal distention after trivial abdominal blunt trauma. On the abdominal CT finding, the right renal pelvis was collapsed, from which the contrast media was extravasated. The right pyeloplasty, nephrostomy, ureteral stent indwelling and suprapubic cystostomy were performed. During immediate postoperative management, extreme fluid and electrolyte imbalances were occurred. However, we overcame these troubles using hypotonic saline and hormonal replacement. So we reported this case with literatures.

Leiomyoma is a benign tumor, which usually occur at the smooth muscle of the gastrointestinal tract and uterus but rarely occurs on the corpus cavernosum of the penis. We experienced a patient with leiomyomas on the bilateral corpus cavernosa of the penis accompanied with impotence in a 60-year-old man. The patient was treated with complete local excision and has remained well without recurrence for l year, So we report this case with review of literatures.

8.Median Nerve Residual Latency in Normal Controls and Patients with Diabetes Mellitus.

Residual latency is the difference between the expected and measured terminal latencies in nerve conduction study. The main contributors to the residual latency are the nerve tapering in the hand and fingers and the neuromuscular delay. We measured median motor and sensory residual latencies in the controls and in patients with diabetes mellitus(DM) to establish the normal values, to evaluate the diagnostic value of the residual latency in diabetic polyneuropathy. we studied 50 healthy controls and 100 diabetic patients with or without polyneuropathy. The normal residual latency values were 1.42+/-0.41 msec(mean+/-SD) in motor part and 0.44+/-0.20 msec in sensory part of median nerve. The standard deviation of residual latency in median motor nerve was decreased by 12% as compared with that of distal latency in the patient with diabetic polyneuropathy. Duration of DM and age were not related to the residual latency of median nerve. The results suggest that the residual latency of median motor nerve provides a narrower normal range in the diagnosis of diabetic polyneuropathy irrespective of duration of DM or age.

9.Clinical Application of Digital Infrared Thermographic Imaging for the Prediction of Foot Ulcer Development in Diabetic Patients.

OBJECTIVE: To investigate the regional differences of skin blood flow and to evaluate the effects of foot temperature on the severity of neuropathic pain and to predict the development of plantar foot ulceration by measuring of the temperature variations on the plantar surface of feet in the diabetic patients. METHOD: We measured the temperature variations on plantar surface of the feet in controls (n=18) and diabetic patients with(n=20) or without(n=23) polyneuropathy. The surface temperature from the 3rd metatarsal head(MTH), greater toe(GT), heel, medial and lateral longitudinal arch(LA) was measured by Digital Infrared Thermographic Imaging(DITI). RESULTS: The mean foot temperature of diabetic patients with polyneuropathy was significantly increased compared to controls or diabetic patients without polyneuropathy(p<0.001). The surface temperature readings of the GT, medial LA and the 3rd MTH tended to be increased in controls and patients with polyneuropathy. The mean plantar surface temperature was significantly increased according to the duration of diabetes mellitus(DM)(p<0.05). CONCLUSION: The results suggest that DITI provides a diagnostic modality in the prediction of neuropathic foot and increased risks of foot ulcer development in the diabetic patients.

10.Pseudocapsule of hepatocellular carcinoma: CT and US versus pathologic correlation.

The encapsulated hepatocellular (HCC) is a pathologic subtype of HCC. It is a well defined tumor that tends to grow slowly, and has a better prognosis than any other gross forms of HCC. Twenty surgically resected HCC were evaluated retropectively to correlate the thickness of pseudocapsules in pathology with those in computed tomography and ultrasound. At a histologic examination, pseudocapsules of seven cases were composed of two layers, an inner compact fibrous zone and outer loose fibrous zone interlaced with compressed liver parenchyma containing small vessels and newly formed bile ducts. Sonographic thickness and pathologic measurements of pseudocapsule relatively well correlated, but the former slightly overestimated the thickness of pathologic pseudocapsule (r=0.825, y=2.56x-1.23, P<0.05). On the other hand, thickness in CT and pathologic measurement did not correlate well. Thirteen cases showed one layer of pseudocapsule in which two cases were composed of thin layer of compact fibrosis and eleven cases composed of loose fibrosis. There were poor correlations in this group between thickness of pseudocapsules in pathology and those in images. Image overtly overestimated the thickness of the pseudocapsules in pathology. In conclusion, radiologic pseudocapsule of HCC may represent the compressed liver parenchyma as well as the fibrous pseudocapsule.
