2.Tendon Transfer for Radial Nerve Paralysis and Multiple Extensors Rupture

Loss of radial nerve function in the hand results in a significant disability and so cannot extend the wrist, thumb & fingers according to the injury levels. Therefore the patient has great difficulty in grasping objects, especially power grip. Tendon transfers to restore function of extension of wrist and fingers are among the the best − most predictable transfers in the upper extremity. We performed 13 cases of tendon transfers for radial nerve palsy and extensive extensor ruptures from 1987 to 1993. The results were evaluated according to Arbitrary Value Method. Among 13 cases 30% of excellent, 46% of good, 24% of fair and no poor result were obtained and the better results were obtatined in low radial nerve lesion.

3.A Case of Leiomyoma of the Ovary in Postmenopausal Women.

Leiomyoma is one of the rarest solid tumor of the ovary. Approximately 50 cases have been published to date. However, most reported leiomyoma of the ovaries were small and rarely induced serious symptoms. We report a case of ovarian leiomyoma in 57-year-old woman which has been experienced in our haspital with brief review of literature.

4.Clinical comparative Study Between Uncemented Anatomic Stem with and Without Lateral Porous Pad

Authors performed a retrospective clinical and radiologic comparative study for 43 uncemented primary total hip arthroplasty which include 20 hips without lateral porous pad and 23 hips with lateral porous pad in same kinds of implant(Zimmer Anatomic") to investigate the role and significance of the lateral porous pad. The average follow up was 35 months and 38 months for without and with lateral porous pad respectively. The results were as followings: 1. Clinical and radiological results of group with and without lateral pad were similar together except incidence of radiolucency at zone 1 on anteroposterior radiograph in group without lateral pad was markedly higher compare to that in group with lateral pad. 2. The extent of lateral porous pad was not sufficiently enough to make better clinical and radiological results in stem with lateral pad compare to stem without lateral pad. 3. Stems without lateral porous pad had no evidence of bone ingrowth on lateral pad while stems with lateral pad had bone ingrowth on lateral pad with more than 50% in incidence. So, we think that presence and increase of extent of porous pad would have better clinical and radiological results.

5.A Study on Clinical Application of Harada's Scoring Method to Kawasaki Disease: Suggesting the revision of the criteria for IVGG treatment of KD in Korea.

PURPOSE: We performed a study on clinical benefits of Harada's scoring method, problems of present criteria and requirement of the revision of that criteria for intrvenos gamma-globulin (IVGG) treatment of Kawasaki disease (KD) in Korea. METHODS: The patients of 104 cases who visited to Department of Pediatrics of Chungnam National University Hospital and diagnosed as KD were objective group. We classified the patients to high and low risk groups as two methods of Harada's scoring method and present criteria for IVGG treatment of KD in Korea, and analyzed the results after treatment by means of echocardiogram. RESULTS: 1. Characteristics of the patients 1) Duration to diagnosis: Mean duration was 6.0 3.5days after onset of fever. 2) Age of onset and male to female ratio: the patients of 84.6% were the age of 4 years or less, and male to female ratio was 1.8 to 1. 2. Coronary aitery lesion (CAL) abnormalities according to items of Harada's scoring method and classification of risk groups 1) The age of less than 12 months and CRP of 3+ or more were significant predictive factors 2) Classification of risk groups was significantly predictive in CAL development. 3. Classification of total cases The patients reserved to administration for IVGG according to Harada's scoring method and present criteria for IVGG treatment of KD in Korea were 63.5% and 50% respectively. If so in present criteria for IVGG treatment of KD in Korea, 50% of the KD patients were excluded for that application. 4. Outcome after treatment according to selective IVGG use by two methods 1) Selective IVGG use by Harada's scoring method: There was no significant difference in development of CAL between low risk group and high risk group, resulting in 1 case (14.3%) of 7 cases and 5 cases (9.4%) of 53 cases respectively. 2) Selective IVGG use by present criteria for IVGG treatment of KD in Korea: There were more significant cases at low risk group than those at high risk group in development of CAL, resulting in 3 cases (42.9%) of 7 cases and 5 cases (12.2%) of 41 cases respectively. 3) Outcome after treatment in patients not administered IVGG as low risk groups according to two methods: There were more significant cases in low risk group at present criteria for IVGG treatment of KD in Korea than those at Harada's scoring method in the development of CAL, resulting in 3 cases (42.9%) of 7 cases and 1 case (14.3%) of 7 ases respectively. 4) Outcome after treatment in patients administered IVGG as high risk groups according to two methods: There was no significant difference between high risk groups at both methods in the CAL development, resulting in 5 cases (12.2%) of 41 cases at present criteria for IVGG treatment of KD in Korea and 5 cases (9.4%) of 53 cases at Harada's scoring method. CONCLUSIONS: Harada's scoring method had more benefits in clinical application, selection of risk group, and outcome after treatment, So we propose the revision of present criteria for IVGG treatment of KD in Korea, suggesting another method as indication for selective IVGG treatment.

7.A Case of Nodular Fat Necrosis Associated with Pancreatitis.

Nodular fat necrosis is a rare and often overlooked sign of pancreatic disease. It is manifested as painful subcutaneous erythematous nodules, often found on the legs. We report a case of nodular fat necrosis associated with pancreatitis. The patient was a 14-year-old girl who presented with erythema nodosum-like tender subcutaneous nodules on both her lower extremities. The serum amylase and lipase levels were markedly elevated. The change of serum calcium and eosinophils were not so remarkable. Histopathological findings showed diffuse subcutaneous fat necrosis and ghost-like cells with thick shadowy walls and no nuclei. The lesions involuted gradually without any specific treatment and the underlying pancreatitis was followed up in the department of medicine.

9.IS6110 based DNA Fingerprinting of Mycobacterium tuberculosis Isolated from The Nationwide Tuberculosis Prevalence Survey in 1995.

One hundred and thirty-eight strains of Mycobacterium tuberculosis isolated from the 7th nationwide tuberculosis prevalence survey in 1995 were subjected to the restriction fragment length polymorphism (RFLP) analysis using IS6110 probe to define the representative fingerprinting patterns of Korean strains of M. tuberculosis and to evaluate the usefulness of DNA fingerprinting in tracing the transmission link of M. tuberculosis. Among 138 strains, 129 different IS6l10 RFLP types were identified. The number of bands in IS6110 RFLP types diversed from 1 to 20, and the majority (75%) was 9 to 14 bands. The RFLP patterns of 8 out of 15 strains isolated from the follow-up survey of one and half year later after the 7th national TB prevalence survey were unchanged when compared with previous RFLP patterns. Fifteen (11%) out of 138 strains were grouped in 6 IS6110 clusters; 2 with 10 copies, 2 with 12 copies, 1 with 14 copies, and 1 with 17 copies. These clusters were unable to be subclassified by IS1081 or (GTG) probes except one cluster by pTBN12 probe. The transmission links of 2 clusters were deducible; one from household and another from neighborhood, but those of remaining clusters were unclear because they had no contact one another. The results suggest that vigorous transmissions in tuberculosis are still progressing in Korea.

10.A Case of Acquired Tufted Angioma.

Acquired tufted angioma is a benign, slowly progressive angioma with a typical histological pattern that was first described by Wilson-Jones in 1976. We report a case of acquired tufted angioma in a 19 year old female who had erythematous papules and plaques on the right thigh. Histopathological findings showed multiple capillary lobules in a cannonball arrangement scattered throughout dermis, which was diagnostic of acquired tufted angioma.
