Lumbar hernias are rare posterolateral abdominal wall defects that may be congenital or acquired. There are two types of lumbar hernia, the superior lumbar hernia through Grynfeltt triangle, and the inferior lumbar hernia through Petit triangle. Many techniques have been described for the surgical repair of lumbar hernias including primary repair, local tissue flaps, and conventional mesh repair. But these open techniques require a large skin incision. We report a case of superior lumbar hernia, which was successfully repaired using a laparoscopic approach.

4.Voluntary Habitual Hip Dislocation in a child

Voluntary habitual dislocation of the hip is very rarely reported in children, and only 13 cases have been reported in the literature. We add a new case in a 5 year old boy. The boy did not have previous history of trauma, skeletal and/or soft tissue anomalies, nor known connective disease. He was treated by intertrochanteric varusinward rotation osteotomy of the affected hip on February 25, 1985 and was followed until January 11, 1993. Postoperative convalescence was uneventfully successful, and the hip developed well without any obvious dysplasia at the latest follow-up.

6.Factors Associated with Cord Blood Leptin Concentration of Full - Term and Preterm Newboms.

PURPOSE: Leptin is the product of the of gene secreted by adipocyte. The serum leptin concentration reflects the amount of adipose tissue in the body. We investigated whether leptin concentration was detectable in cord blood of newboms and assessed the effect of gestational age, birth weight, birth length, placental weight, body mass index and ponderal index on cord blood leptin concentration in full-term and preterm newboms. METHODS: Eighty-two newboms were enrolled in this study. Standard growth curves were used to categorize infants as appropriate (AGA), large (LGA), or small for gestational age (SGA). Gestational age, birth weight, length and placental weight were measured. Maternal age, weight and height were measured. Cord blood and maternal venous samples were collected and serum was separated and stored at -70C. Leptin was measured by radioimmunoassay. RESULTS: The mean leptin concentration in full term babies was 8.9+/-9.6 ng/ml compared with 3.5+/-0.6 ng/ml in preterm babies. Leptin concentrations in cord blood of full term babies were significantly increased in cornparison with those in cord blood of preterm babies. Mean serum leptin concentration of SGA infants was significantly lower than in AGA infants and that of LGA was higher than in AGA infants. In full-term and preterm neonates, there was significant positive correlation of leptin concentrations with birth weight, gestational age, birth length, BMI, ponderal index and placental weight. No gender differences were found in cord blood leptin. Maternal age, maternal body mass index, matemal weight at baseline (before pregnancy) and at birth were not significantly correlated with leptin concentration of full-term and preterm neonates. There was no significant correlation between leptin concentration of cord blood and matemal leptin concentration. CONCLUSION: The serum leptin concentration is highly correlated to the size of adipose tissue mass of newborn infant. Leptin may be responsible for regulation of body mass of human neonates and can be used as marker of the nutritional status and growth of fetal and neonatal period.

7.Post-Laminectomy Swan Neck Deformity in Adults

Five adult cases of post-laminectomy swan-neck deformity are reported. The factors causing the deformity, prophylactic measures, and treatment of the deformity are discussed. It is strongly adviced to the neurosurgeons and orthopedic surgeons that they should be aware of this unwanted complication of the multi-level cervical laminectomies, and that they should provide every preventive measures before post-laminectomy swan neck deformity develops. Anterior interbody fusion spanning the entire unstable segments is preferably recommended together with application of pre-fusion traction.

8.Hand, Foot, and Mouth Disease: Clinical and Virological Investigations.

Hand, foot and mouth disease, a distinctive clinical syndrome caused by a coxsackie virus, is clinically characterized by vesicles appearing on the hands, feet and in the mouth. The infection begins with a fever and mouth lesions consisting of small vesicles surrounded by red areolae on the buccal mucosa, tongue, soft palate and gingiva. The disease usually lasts spontaneously between 7 to 10 days after onset. We observed 20 cases of hand, foot, and mouth disease from July, 12th to September, 4th, 1979 and examined histopathologically and virologically. We have isolated only one viral strain showing cytopathic effect on HeLa cell among the five cases of acute stage and also observed that viral particle in the electronmicroscope.

9.Two cases of allergic contact dermatitis to sodium fusidate.

Sodium fusidate, obtained by fermentation of the fungus, Fusidium coccineum, has a steroid structure and shows a very high antitaphycoccal activity. The allergic potentirl of sodium fusidate is low and few cases of contact allergy to sodium fusidate have been reported. We present two cases of allergic contact dermatitis to Fucidin cintentione developed postoperative dressing of a skin biopsy and the other after laser treatment of a vascular nevi. The patch test results showed positive reaction to sodium fusidate in both cases.

10.Granular Cell Tumor of the Esophagus: Analysis of five cases with review of four additional cases in Korean literature.

Granular cell tumor(GCT) has been identified in all organ systems, most often in the oral cavity and subcutaneous tissue. GCT of the esophagus has been considered rare. However, this neoplasm is being recognized with increased frequency since the use of fiberoptic endoscopy has become widespread. We report 5 cases of granular cell tumors involving the esophagus, all of which were incidentally found. Four additional cases of GCT which appeared in the Korean literature were reviewed.
