1.Virulence of Entamoeba histolytica according to the strains in Korea III. Amebicial response of antiamoebic agents on several strains of Entamoeba histolytica in vitro.

The amebicidal activity of traditional anitiamoebic drug (emetine, carbasone, diodoquin, chloroquine, atabrine, chloramphenicol and tertracycline) and newly appeared chemicals(niridazole, metronidazole and No. 8603 substance) were assayed by in vitro experiment using five strains of human originated E, histolytica. The variety of amebicidal activity of drugs by the strains were discussed. Ranges of amoebicidal activity of traditional antiamoebic drugs kept almost similar titers of previous reports at the concentration; 1:5,000 to 1:20,000 with emetine hydrochloride, 1:10.000 to 1:20,000 with carbarsone, 1:8,000 to 1:16,000 with diodoquin, 1:50,000 with chloroquine, 1:1,000 to 1: 4,000 with atabrine ,1:1,000 to 1:2,000 with chloramphencol and 1:5,000 to 1:8,000 with tetracycline. The newly appeared chemicals showed higher amebicidal titres at the concentration; 1:500,000 to 1:5,000,000 with niridazole, 1:50,000 to 1:100,000 with metronidazol and 1:100,000 to 1:500,000 with No.8603 substance. Emetine, chloramphenicol and No. 8603 substance showed amebicidal activities at lower concentration to intestine originated amebae (YS 14, YS 15 and NAMRU II strain) than to liver originated amebae (YS 24 and YS 25 strain ), while carbarsone, chloroquine and metronidazole showed the activity at higher concentrations. Diodoquin showed lower amebicidal titres to trophozoite borne amebae (NAMRU II, YS 24 and YS 25 strain) than to cyst borne amebae(YS 14 and YS 15 strain), but niridazole showed converse results. The concentration of atabrine for amebicidal activity was not constant according to strains of the amoeba, but tetracycline showed almost settled titers.

2.Revision Arthroplasty with Cementless Total Hip Replacement

It is well known that the incidence of revision arthroplasty for the loosened total hip has been progressively increasing recently. And the problem is that the loosening rate of revision arthroplasty for the failed total hip replacement is much higher as compared with that of primary total hip replacement. In revision arthroplasty, Amstutz, in 1982, reported 9% incidence of mechanical failure and 43% incidence of femoral and 61% incidence of acetabular progressive radiolucencies with follow-up for an average of 2.1 years, and Pellicci, in 1985, reported 29% incidence of failure for total hip replacement that have been revised once in the radiolucent zone with the follow-up for an average 8.1 years, and Kavanagh, in 1985, reported that probable loosening occurred in 25% of revised acetabular components with follow-up for an average of 4.5 years. Cementless total hip replacement is successful in more than 90 of the primary cases for the last 5 years in our department. Therefore, not only in primary total hip replacement but also in the revision, it seems to be reasonable to perform the cementless total hip replacement as an alternative, particularlly, in young age group. During the period from April 1982 to December 1984, we have revised 12 loosened total hip replacement with cementless type. Average follow up was 1 year and 7 months. The age range of the patients was 23 to 64. The results are very promising without any evidence of loosening in all cases.

4.Metronidazole in amoebiasis 1. The use of metronidazole on the treatment of hepatic and pulmonary amoebiasis.

Total 9 cases of extraintestinal amoebiasis, amebic liver abscess and amebic lung abscess, were treated with metronidazole. The diagnosis and progress check were done by (1) stool examination, (2) size and pain of liver, (3) hematological examination, (4) urinanalysis, (5) X-ray or liver scanning (6) examination of aspirated pus from liver and (7) immobilization test of E. histolytica. Results are summarized as follows: Amebic hepatitis: Metronidazole was given to 3 cases with daily dose of 0.9-1.2 gm for 3-7 days. Clinical symptoms were completely restored by the treatment. Amebic lung abscess: One case was treated with metronidazole for 3 days with the daily dose 1.2 gm. X-ray findings showed remarkable improvement by the treatment. An additional administration of the drug for 6 days resulted complete resolution of the pneumonic changes. Amebic liver abscess: Four cases were treated with metronidazole. The daily doses were 0.4-2.4 gm and continued for 2-10 days. Aspiration of the abscess for 1-10 times or continuous drainage of pus were performed during the course supplementarily. All cases were recovered to normal by the combining treatment. No remarkable side effect was noticed by the drug administration, and no relapse was experienced up to date.

5.A Case Report of Correction of Mandibular Prognathism by Intraoral Oblique Splitting Osteotomy of Mandibular Rami.

This is a report of 2-cases of mandibular prognathism corrected by Intraoral oblique splitting osteotomy of mandibular rami. The Intraoral oblique splitting osteotomy is a modification of sagittal split osteotomy of ramus and it is documented by Yoshida, on 1985. By this method, authors obtained the following results. 1. The patients' esthetic, psychological and functional problems were dissolved by setback of mandibular prognathism. 2. The postoperative infection splitted bone segments fracture, paresthesia of the face and T.M.J. dysfunction were not appeared. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. The patients could open their mouths in normal range after a week of intermaxillary fixation removal. 4. The soft tissue changes of lower lip and chin were about 1:1 to the hard tissue changes. 5. During intermaxillary fixation period and postoperative orthodontic treatment, slight relapse was observed. Now, the patients are under postoperative orthodontic treatment.

6.Virulence of Entamoeba histolytica according to the Strains in Korea II. Studies on the Pathogenicity of Entamoeba histolytica Strains in Rats.

The experimental infection of rats with seven strains of Entamoeba histolytica were carried out according to animal ages, number of inoculated amebae, rat strain differences and rat-culture passages. The rat cecal scoring technique of Neal (1951) was utilized to measure the invasiveness of the parisite. The results are summarized and concluded as follows; 1. In the infection of Sprague-Dawley strain rat with YS9-strain and NAMRU II-stran amebae, which wem confirmed highly invasive to the membrane of cecum of rabbits in the previous reports (Cho,1968; Cross,1968), remarkab1e invasiveness was observed in the 30-day-old rat groups with the average cecal score above 5.0. Although no statistical differences of virulence by the number of inoculations showed in rat groups, the cecal scores were markedly reduced in the 50,000 amebae inoculated rats. 2. The hybrid albino rats were considered unsuitable for virulence study of E. histolytica, since the invasiveness of the amabae was inconsistant. 3. The virulences of YS 14 and YS16-strains from cyst carrier showed no virulence, YS 15 from cyst carrier and YS 24 from liver abscess were moderately invasive, and only YS25 from liver abscess showed highly invasive as with YS 9 and NAMRU II-strain amebae. By rat-culture passage, YS14-strain and YS24-strain amebae showed marked increase of invasiveness. It was presumed that the rat-culture passage should be indispensably supplememed in the studies on the virulence of E. histolytica.

7.Osteogenic Sarcoma of the Mandible.

Osteogenic sarcoma is a malignant primary tumor of bone composed of a malignant connective tissue stroma with evidence of malignant osteoid, bone and/or cartilage formation. A 23 year old man was admitted to dental department of Yeungnam University hospital with chief complaint of swelling and pain on alveolar ridge of lower right molar region. It was certain of osteogenic sarcoma through clinical and radiographic features and biopsy. Surgical resection of the lesion was performed by partial mandibulectomy and resin plate insertion. By follow up check of the patient, we made good result of functional reconstruction without any sign of recurrence of the lesion.

9.Prevalence of intestinal parasites in Korea.

A survey of intestinal parasites infection among Korean people has been carried out during July 1969 to December 1970. A total of 2,250 stool specimens (male 1,101, female 1,146) was collected from all the provinces and Seoul city in Korea. The specimens were examined routinely by direct fecal smear, zinc sulfate flotation and formalin-ether sedimentation techniques. The results are summarized as follows: Of 2,250 specimens examined, l,803(80.l per cent) were positive for intestinal parasites. The positive rates of intestinal helminths were 1,644(73.1 per cent) among 2,250; Ascaris lumbricoides 46.0 per cent, Trichocephalus trichiurus 46.8 percent, hookworm 6.8 per cent, Trichostrongylus orientalis 7.0 percent, Clonorchis sinensis 12.1 percent, Enterobius vermicularis 1.6 per cent, Hymenolepis nana 0.7 percent, Taenia species 0.3 per cent, Metagonimus yokogawai 0.04 percent, Fasciolidae 0.04 per cent and one case of lung fluke Paragonimu westermani. The positive rstes of intestinal protozoa were 786(34.9 per cent); Entamoeba histolytica 6.4 per cent, Entamoeba coli 20.5 percent, Endolimax nana 10.0 per cent, Giardia lamblia 5.1 per cent, Trichomonas hominis 1.1 percent, Chilomastix mesnili 0.5 percent, Iodamoeba butschlii 0.6 percent, Enteromonas hominis 0.7 percent, Dientamoeba fragilis 0.1 per cent and one case of Isospora hominis. Sexual distribution of helminths and protozoan infections showed higher rate in female than that of male, except C. sinensis, H. nana, Taenia species or G. lamblia Infections of T. trichiurus, hookworm, T. orientalis, C. sinensis, Taenia species, E. histolytica, E. coli and E. nana increased with age. Conversely, H. nana and G. lamblia infections were more predominent in younger ages.

10.Clinical Observation on Congenital Heart Disease in Adult.

BACKGROUND: With the advance of the techniques of echocardiography and cardiovascular surgery, early detection and successful cardiovascular surgery of congenital heart disease is possible in infant as well as in child. And with the advance of the social insurance, the new case of adult congenital heart disease with mild cardiovascular symptom or frank symptom of the pulmonary hypertension is decreasing. We statistically analyze the new case of adult congenital heart disease. METHOD: 92 patients who were diagnosed to congenital heart disease by echocardiography from January 1993 to June 1998 were studied. 2.25 MHz probe for two-dimensional and Doppler echocardiography and biplane 5 MHz phased-array probe for transesophageal echocardiography(Ultramark-9) were used. RESULT: Among 92 patients, 45 patients(48.9%) were male and 47 patients(51.1%) were female and 6 patients(male : 2, female : 4) had multiple congenital heart disease. 61 patients(66.3%) had no definitive cardiovascular symptom and right bundle branch block was most common electrocardiographic abnormality. 32 cases(32.8%) were atrial septal defect, 21 cases(21.5%) ventricular septal defect, 12 cases(12.3%) patent ductus arteriosus, 8 cases(8.2%) congenial bicuspid aortic valve and so on. Female predominance was noted in ventricular septal defect and endocardial septal defect, while male predominance in bicuspid aortic valve and discrete subaortic stenosis. Atrial septal defect and patent ductus arteriosus were no sex difference. 23 cases(25.0%) were between 20 29, 17 cases(18.5%) between 15-19, and 2 cases(2.2%) over 70 years old. CONCLUSION: Our analysis shows similarity to previous report. With advance of the technique of echocardiography and cardiovascular surgery, a few new case of adult congenital heart disease can be diagnosed hereafter.
