2.A Case of Acute Reticulate Hyperpigmentation on the Face and Neck.

Multiple, reticulated, dark brown colored, macules and patches suddenly developed on the fac, and neck of a 48-year-old Korean woman two days after a traffic accident. On physical examination and laboratory tests including serum melanocyte-stimulating hormone, estrogen and progesterone level, no abnormalities were found except cervical pain. Histological examination of hematoxylin-eosin stained sections revealed increased melanin pigmentation in the basal layer, but the number of melanocytes was not changed in DOPA stained section. Topical application of 2% hydroquinone and 20% azelaic acid ointments had been applied successively for two months each without any apparent improvement. Herein we present a case of reticulate hyperpigmentation on the face and neck, which is very acute and whose causative factors are not certain.

4.Two Cases of cutis Marmorata Telangiectatica Congenita.

Cutis naarmorata telangiectatica congenita(CMTC) was first lesribed by Van Lohuizen in 1992. Cases of CMTC were reported later under various names, includir gan Lohuizens syndrome, congenital generalized phlebectasia. congential phlebectasia and reticulatever scular nevus. It is charaterized by persistent livedo reticularis, telangiectasia and phlectnsia from birch hand atrophy and superficial ulceration of the overlying skin are uncornmonly encountered. They have a steady improvement with time, however various congenital abnormalties, such as varicosities, hen ariomas, hypoplasia or hypertrophy of soft tissue and bones, rarely generalized congenital fibrornatos ectal and genital anomaliies may be associated in approxirnately 50 percent of patients. In case 1, the patient was 45-day-old female infant with reticulated mottling and atrophy of the skin on the right leg. In case 2, the patient was 30-day-old male infant who depressed, bluish, and reticulated patches on the both upprarms, right side trunk, buttock and loen leg. Decreased circumferences of the affected extremities vas observed in case 2. No other assotated anomalies were found in physical and X-ray examinatiorwlin both cases. The skin biiopsy of bothed similar findings, which revealed mild dilatation and proliferation of capillaries, mild inflammatoryel infiltration and extravasated red blood cells in the dermis. The clinical follow-up, alneast all of th lisions disappeared spont.anously in 1 year at case 1, and some of lesions also disappeared in 3 month, 11 case 2. In addition, we reviewed and summerized the reported cases in Korea.

5.A Case of Eccrine Carcinoma with Lung Metastasis.

Eccrine carcinoma does not have a characteristic clinical appreranc, but does have a high incidence of metastatic spread. It may arise anywhere in the skin. The microscopic patterns that of an adenocarcinoma, which may be confused with metastatic adenocarcinoma. In the classical type the histologic features varies from fairy well-differentiated tubular structures in some areaslo anaplastic carcinoma in other areas. The patient was a 51-year-old man who showed a painful, tender, right red-calored, 4 x 5crn-sized tumor mass on the right angle of the lower lip for ten years. In the diological examinations multiple metastatic foci were seen on both lung fields, Histopathologic findings of the skin and lung biopsy specimens revealed that of the classic type of eccrine carcinoma like the bove descriptions. Immunocytochemical studies showed a positive reactivity to anti-cytokeratin anti-ibody, anti-CEA antibody, and anti-S-100 protein on the skin and lung biopsy specimens and no he reactivity to anti-S-100 protein on the lung mass. Electronrnicroscopic findings revealed eccrine glaidlar structures such as nunierous mitochondria and dense core granules in the cytoplasm of neoplastic cells. We report a case of eccririe carcinoma with lung metastases baed on the clinical, radiological and histologic findings.

7.A Case of Cutaneous Side Reaction of Collagen Implantation.

Injectable Collagen Implan, is composed of purified bovine dermal collagen emulsified in saline and lidocaine(0.3%) base, and has been promoted for correction of soft tissue contour defects. Although collagen is a relatively nontoxic, nonimmunologic and biolegically nonreactive biomaterial, some side reactions have been reported. We report herein a case of cutaneous side reaction to collagen implantation in a 38-year-old female. The patient presented with puritic, irritating, multiple, erythematous nodules on both the temporal and interorbital area, which had been observed since 1 day after collagen(KOKEN ATELOCOLLA-GEN IMPLANT-Japan) implantation. Histopathologic findings showed dense infiltration of acute and chronic inflammatory cells in the dermis, especially the perivascular and periadnexal area.

8.Induction of DNCB Oral Tolerance in Mice.

Tolerance to contact hypersensitivity was induced by feeding of different DNCB doses in mice. A total of 40 mice were divided into 4 groups(control group, 6 mg feeding group, 10 mg feeding group, 14 mg feeding group) in experiment I, Degree of tolerance to contact hypersensitivity was rneasured by incremert rate of ear swelling after challenge with DNFB. Experiment 2 was performed in the same method of cxperiment: I with addition of 3 mg DNCB feeding group. The increment ratee were significantly decreased in DNCB feeding groups in experirnent 1 and 2(p<0.0l). But there were no differences statisticalIy between increment rates of DNCB feeding groups.

9.Outcome of External Ventricular Drainage according to the Operating Place: the Intensive Care Unit versus Operating Room.

BACKGROUND: External ventricular drainage (EVD) is an important procedure for draining excessive cerebrospinal fluid (CSF) and monitoring intracranial pressure. Generally, EVD is performed in the operating room (OR) under aseptic conditions. However, in emergency circumstances, the operation may be performed in the intensive care unit (ICU) to save neuro-critical time and to avoid the unnecessary transfer of patients. In this study, we retrospectively analyzed the risk of EVD-induced CNS infections and their outcomes according to the operating place (ICU versus OR). In addition, we compared mortalities as well as hospital and ICU days between the CNS infection and non-CNS infection groups. METHODS: We reviewed medical records, laboratory data and radiographic images of patients who had received EVD operations between January, 2013 and March, 2015. RESULTS: A total of 75 patients (45 men and 30 women, mean age: 58.7 +/- 15.6 years) were enrolled in this study. An average of 1.4 catheters were used for each patient and the mean period of the indwelling catheter was 7.5 +/- 5.0 days. Twenty-six patients were included in the ICU group, and EVD-induced CNS infection had occurred in 3 (11.5%) patients. For the OR group, forty-nine patients were included and EVD-induced CNS infection had occurred in 7 (14.3%) patients. The EVD-induced CNS infection of the ICU group did not increase above that of the OR group. The ICU days and mortality rate were higher in the CNS infection group compared to the non-CNS infection group. The period of the indwelling EVD catheter and the number of inserted EVD catheters were both higher in the CNS infection group. CONCLUSIONS: If the aseptic protocols and barrier precautions are strictly kept, EVD in the ICU does not have a higher risk of CNS infections compared to the OR. In addition, EVD in the ICU can decrease the hospital and ICU days by saving neuro-critical time and avoiding the unnecessary transfer of patients. Therefore, when neurosurgeons decide upon the operating place for EVD, they should consider the benefits of ICU operation and be cautious of EVD-induced CNS infection.

10.Studies on the mites(Order Acarina) in Korea I. Suborder Sarcoptiformes.

Acarid mites were isolated from samples collected in Seoul during the period of October 1975 to August 1978. Samples consisted of stored foods such as rice, flour, sugar, dried fishes, dusts of houses and freight offices.
