1.A Comparision of Normocholesterolemia and Hypercholesterolemia in Middle-Aged Men in Kwang-ju.

The purpose of this study was to investigate the differences in anthropometric, hematologic, and lipidologic data between normocholesterolemia and hypercholesterolemia and influencing factors in hypercholesterolemia in middle-aged men in Kwang-ju. 179 middle-aged men were involved. Anthropometric data(body weight, height, body circumferences and skinfold thickness) and hematologic data(blood pressure, fasting blood glucose, albumin and hemoglobin) were measured. Bleed lipid profile(total cholesterol, triglyceride, phospholipid, HDI-cholesterol, LDL-cholesterol and VLDL-cholesterol) were analyzed. To determined using questionnaire. The subjects were classified as hypercholesterolemic based on The Guideline for Korean Hyperlipidemia. body mass index, systolic blood pressure, waist and hip circumference, and triceps, subcostal, abdomen, subscapular and suprailliac skinfold thicknesses of the hypercholesterolemic group were significantly higher than in the normocholesterolemic group. The concentration of cholesterol, triglyceride, phospholipod. HDL-cholesterol, LDL-cholesterol and VLDL -cholesterol of the hypercholesterolemic group were significantly higher than normocholesterolemic group. The concentration of cholesterol has a significantly positive correlation with body mass index, diastolic blood pressure, waist circumference, waist/thigh ratio and skinfold thickness(triceps, subcostal, abdomen, and supraillic skin-fold thickness). The general tendency of socioeconomec status, frequencyu of meat intake, smoking amounts and duration and duration and drinking frequencies and amount were higher in the hypercholesteolemic group but not significant. In conclusion, hypercholesterolemia in middle-aged men was closely correlated with upper body fat obesity and most of them had an accompanied high triglyceride level tendency.

2.Follow-up Study of the Cemented Polished Femoral Stem for More than Five Years.

PURPOSE: Surface finishing of a cemented femoral stem is a subject of controversy even though the contemporary cementing techniques have improved results. Using the Versys Heritage femoral stem, we evaluated the outcome of using a polished surface. MATERIALS AND METHODS: The subjects of this study were 95 hip arthroplasties in 82 patients and we used a cemented polished femoral stem with the 3rd generation cement technique and all the surgeries were done between October 2000 and August 2003. There were 58 male patients (64 hips) and 24 female patients (31 hips). The mean age at the time of the index arthroplasty was 52.3 years (26~74 years), and the average body mass index was 24.2+/-2.75 (19.1~29.8). The average follow up period was 80.7 months (64~109 months). All the hips were evaluated clinically by the Harris hip score and the thigh pain, and they were radiologically assessed by the cement grade and the presence of osteolysis around the femoral stem, as well as the presence of stress shielding of the proximal femur. RESULTS: At the final follow up, the Harris hip score for all the patients had improved from preoperative 58.9 (17-83) to post operative 91.7 (72~100). The cement grade was measured using Barrack's method. Of the 95 hips, 45 (47.3%) cases were grade A, 48 (50.5%) cases were grade B and 2 (2.1%) cases were grade C1 at the final follow up. There was 1 case of definite loosening. Stress shielding was noted in 65 (68.4%) cases of the zero grade and 19 (20.0%) cases of the 1st grade. CONCLUSION: In this study, the cemented polished femoral stem showed excellent results at the mid term with a minimum follow up of 5 years. But a longer-term follow-up study will be needed for further understanding the implications of cemented polished femoral stem.

3.Spontaneously Ruptured Congenital Arteriovenous Malformation of Unilateral Renal Agenesis in Woman with Urinary Retention.

Congenital renal arteriovenous malformations are rare. Moreover, rupture of congenital arteriovenous malformations in the unilateral renal agenesis with uterine malformation is more rare. We present a case of acute urinary retention due to large hematoma of spontaneously ruptured congenital arteriovenous malformation of unilateral renal agenesis in a 33-years-old woman.

4.The Study of the Changes of Myocardial Histology and Left Ventricular Function according to the Total Doses of Doxorubicin in Rabbits.

PURPOSE: We'd like to evaluate the relationship between the degree of cardiac damage and that of cardiac function according to the total injected dose of doxorubicin. METHODS: 12 rabbits(body weight : 2.0-3.2 kg) were used and 30 mg/m2/week of doxorubicin hydrochloride was injected intravenously. The cardiac function was checked under anesthesia, after which the thorax was opened and cardiac samples were evaluated with light and electron microscopics. RESULTS: Regardless of total injected dose of doxorubicin, there was no difference between the general appearance, sizes and thickness of both atria and ventricles of doxorubicin injected rabbits and those of normal control. The rabbits with total cumulative dose <180 mg/m2 showed no differences with the rabbits of normal control in the cardiac histology. The rabbits with cumulative doses of 210 mg/m2 had an increased number of vacuoles in the interspaces of the myofibrils and glucogen granules in myocytes. The rabbits with the cumulative dose of 240 mg/m2 had focal necrosis and degeneration of myocytes with fibrous cells infiltration. The rabbits with cumulative doses of 300 mg/m2 demonstrated severe and diffuse degenerations with markedly decreased numbers of myocytes and increased intracellular vacuoles. All the rabbits with total cumulative doses <240 mg/m2 showed no difference in the aortic pressure, left ventricular(LV) maximal dP/dt and left ventricular end diastolic pressure(LVEDP) compared to normal control. The rabbit with total cumulative doses of 300 mg/m2 showed no difference in the aortic pressure and LV maximal dP/ dt, but revealed significantly elevated LVEDP compared to normal control. CONCLUSION: These results showed doxorubicin induced cardiotoxicity is dose related and may be certain at total cumulative dose >200 mg/m2, although cardiac function is normal.

5.A Case of Malignant Eccrine Poroma.

Malignant eccrine poroma is a rare malignant cutaneous tumor arising from intraepithelial eccrine duct and it may develop either spontaneously or from a long standing eccrine poroma. We report a case of malignant eccrine poroma on the antihelix of the right ear in a 37-year-old female, previously diagnosed as a benign eccrine poroma with shave biopsy specimens. Histological findings are well-defined tumor cell nests disseminated in dermis. Each cell has a large, hyperchromatic atypical nuclus. The atypical mitoses, dyskeratoses, intracytoplasmic vacuolations and duct-like structures are observed. She was treated with wide surgical excision and local skin flaps.

6.Effect of renin inhibition on an experimental glomerulonephritis - a preliminary report.

PURPOSE: We performed this study in order to investigate the effect of direct renin inhibition on an experimental animal model with nephrotoxic serum nephritis and tried to give useful information for clinical research and renin inhibitor treatment. METHODS: Thirty BALB/c 6-week-old male mice were divided into 4 groups: control group (CO, n=5), control-treatment group with aliskiren (CT, n=5), disease group (DO, n=10), and disease treatment group with aliskiren (DT, n=10). Nephritis was induced by an intravenous injection of 0.25 mg/g weight of rabbit anti-GBM immunoglobulin G. Model 2002 Alzet mini-osmotic pumps (Durect Corp.) for aliskiren infusion were implanted into CT and DT. Each group strain was sacrificed serially one at a time on day 14. We estimated the protein-creatinine ratio in 12-hour-collected urine (UP/Cr) and measured the mesangial matrix score in the PAS-stained kidney of each strain. RESULTS: One strain at CT and DT died on day 6 and 7, respectively. Each group strain was sacrificed serially at a time on day 10 because DO were seriously ill. The UP/Cr of each group is as follows: CO, 31.24+/-6.54 mg/mg, CT, 23.38+/-13.60 mg/mg, DO, 112.72+/-10.97 mg/mg, DT 114.07+/-32.30 mg/mg. There was no significant difference between DO and DT. The mesangial matrix score of each group was CO, 0.23+/-0.10; CT, 0.13+/-0.03; DO, 1.90+/-0.48; and DT, 1.28+/-0.41, respectively, and there was a significant difference between DO and DT in the extent of mesangial matrix expansion (P=0.008). CONCLUSION: We found that renin inhibition was able to suppress the mesangial matrix expansion in experimental mice with acute nephritis, although there were no significant differences in UP/Cr.

7.Penile Squamous Cell Carcinoma Diagnosed following Treatment of Urethrocutaneous Fistula after CO2 Laser Therapy for Misdiagnosed Penile Lesion: Report of a Case.

The complications of the treatment of penile lesion are wide ranging, urethrocutaneous fistula being one of the less common. This complication affects the ventral aspect of the penis. We present a rare case of urethrocutaneous fistula after vaporization of penile lesion by CO2 Laser. One fistula was at ventral aspect of the penis. A 43-year-old male presented with passage of urine from one opening. He had undergone a vaporization by CO2 laser, 2 times. Urethrocutaneous fistula repairs was performed with biopsy of the edge of fistula site. Squamous cell carcinoma was confirmed. He subsequently underwent a partial penectomy. It is very important that biopsy was performed before vaporization of even small size skin lesion. This case is reported along with a brief review of the literature.

8.A Case of Recurrent Pyogenic Granuloma with Multiple Satellites.

Pyogenic granuloma is a common proliferative lesion that often occurs shortly after a minor injury or infection of the skin. Typically the lesion grows rapidly for a few weeks before stabilizing as an elevated, bright red papule, usually not more than 1 to 2 cm in size. Recurrence after surgery or cautery is not rare. The development of multiple satellite angiomatous lesions at and around the site of a previously destroyed pyogenic granuloma is a rare and alarming event. Herein we report a case of recurrent pyogenic granuloma with multiple satellites that developed after excision of a single lesion of pyogenic granuloma in an 8-year-old boy.

9.A case of transfusion-related acute lung injury induced by anti-human leukocyte antigen antibodies in acute leukemia.

Transfusion-related acute lung injury (TRALI) is a noncardiogenic pulmonary edema that occurs during or within 6 hours after transfusion. Risk factors for TRALI, which is relatively common in critically ill patients, include recent surgery, hematologic malignancy, and sepsis. Here, we report a case of TRALI induced by anti-human leukocyte antigen (anti-HLA) class II antibodies (HLA-DR) occurring after transfusion of platelet concentrates in a patient with acute leukemia. Although most patients with TRALI show improvement within 48-96 hours, our patient's condition rapidly worsened, and he did not respond to supportive treatment. TRALI is a relatively common and serious adverse transfusion reaction that requires prompt diagnosis and management.
