1.3 Cases Report of Myositis Ossificans Progressiva

Myositis Ossificans progressiva is a very rare disease and not more than five cases have been reported in this country. The symptoms usually appear before the age of six and no case has ever been reported in medical literature in which patient is older than the age of twenty. This author reports here that a 52 years old male was diagnoed as a myositis ossificans progressiva. The patient developed his symptoms when he was lynched by a club during the Korean conflict at the Korean conflict at the age of thirty. Another interesting fact is that is that his daughter has also developed similar symptoms which appeared following an accident when she was run over by an angry cow at the age of six.

2.Pigmented Eccrine Poroma on the Scalp.

Eccrine poromas mainly involve the non-hairy surfaces of the skin, however, have also been found on hair bearing areas. Pigmentation in the tumor cells is rare but has been mentioned in a few papers. The following case documents an example of a pigmented eccrine poroma which occurred in an unusual location; the posterior scalp.

4.A Clinical Study on Baseball Pitcher's Elbow

History taking, physical examination and x-ray study of both elbows were made on 55 senior high school, college and vocational baseball pitchers who visited the Department of Orthopedic Surgery of Hanyang University Hospital from March 1, 1980 to April 30, 1980. The results of this study were as follows: 1. Average age of 13 high school pitchers was 17.2 years, of 20 college pitchers was 20.1 years, and of 22 vocational pitchers was 23.4 years and average duration of pitching of high school pitchers was 3.8 years, college pitchers was 7.4 years, and vocational pitchers was 9.8 years. 2. Overhand type pitchers were 39(70.9%) and it was the most common pitching style. 3. 45(81.8%) complained elbow pain and the order of frequency was medial, lateral, and posterior. 4. 45(81.8%) showed hpertrophy of forearm and 38(69.1%) showed hypertrophy of arm. 5. Valgus deformity of the elbow joint was observed In 34(61.8%) and flexion contracture of the elbow joint was in 41 (74.5%). 6. Changes on x-ray were ossicle, traction spur, humeroulnar hypertrophy, narrowing of joint space, calciflcation of soft tissue and others. 7. Changes on x-ray were in proportion to duration of pitching and especially combined Iesions were observed more often ln overhand type pitchers.

5.The Operative Treatment of the Resistant clubfoot: Comparative study between modified Turco's Operation and combining calcaneocuboid release

The 17 patients with 22 resistant clubfeet were treated with modified Turco's operation with or without combining release of calcaneocuboid joint at Department of Orthopaedic Surgery, Hanyang University School of Medicine between 1896 and 1991. They were analysed preoperatively and postoperatively with chinical and radiological methods. In patients who were treated with combined procedures, the latetal talocalcaneal angle and anteroposterior talo-first metatarsal angle in radiological findings were showed better improvement(p < 0.05), and anteroposterior talocalcaneal angle was also showed better improvement but not significant statistically, than those who were treated with modified Turco's operation alone. The final results of combined procedures were better than that of modified Turco's operation. And the most common characteristic physical findings in resistant clubfoot were small heel, deep longitudinal crease on the medial aspect of midfoot area and a single transverse crease on the posterior heel just proximal to the insertion of the tendocalcaneus. So we suggest these physical findings may be expectance for the resistance of the congenital clubfoot to conservative treatment, but it need further comparative study.

6.The Response of Mantoux Test after BCG Vaccination According to the Clinical Factors..

PURPOSE: During the childhood, Mantoux test is very popular for detecting tuberculous infection and also useful for evaluating the immunity against tuberculosis after BCG vaccination. But the response and conversion rate is affected by the age of vaccination, the quality of vaccine, the duration after vaccination and the frequency of the test. This study was performed to investigate the difference of the response and conversion rate, according to duration after vaccination, gestational age, sex and feeding type. METHODS: The subject was included 85 of infants (71 of full term infants, 14 of preterm infants) and was followed up 3 months, 9 months, 15 months after vaccination, from July, 1992 to June, 1994 in Kangnam St. Mary's Hospital. We injected intradermally the BCG vaccine (0.05ml) which was produced by National Institute of Health and PPD (5TU) was used as a testing material of Mantoux test. Injecting BCG and interpreting the result of the Mantoux test was done by the same person. RESULTS: 1) The positive conversion rates of Mantoux test which was done 3 month, 9 month and 15 month after vaccination were 26.8%, 53.5% and 60.6% in the full-term infant group, and 14.3%, 28.6%, and 50% in the preterm infant group. The conversion rate was significantly lower in preterm infant group as compared with that of the full-term infant group 3 months after vaccination. 2) The sizes of induration in Mantoux test, which was done 3 month, 9 month and 15 month after vaccination, are 7.2mm, 8.5mm, 9.4mm in the full-term infant group and 5.0mm, 6.6mm, 8.2mm in the preterm infant group. The size of induration was significantly lower in the preterm infant group as compared with that of the full-term infant group. 3) There is no difference in conversion rate of Mantoux test according to the sex, feeding type in both groups. 4) Complication rate for BCG vaccine is 9.9%, which is leaded by lymphadenitis, persistent ulcer, and febrile abscess as the order, in the full-term infant group and there are no complications in the preterm infant group. CONCLUSIONS: In those results, the size of induration and conversion rate is increased as the duration after vaccination, then lower conversion rate in the preterm infant group 3 months after vaccination may be due to immunological immaturity of preterm infant.

7.A Case of Group A beta-hemolytic Streptococcal Necrotizing Fasciitis.

Necrotizing fasciitis is a life-threatening, invasive soft-tissue infection that is characterized by widespread, rapidly developing necrosis of the subcutaneous tissue and fascia with relative sparing of the skin and underlying muscle. In type I necrotizing fasciitis, anaerobes and gram negative bacteria are predominant; in the type II form, Group A p-hemolytic streptococcus is a major etiologic agent. It needs immediate surgical intervention for a favorable outcome. A 64-year-old male presented with extremely painful, erythematous, symmetric, tense swelling with yellowish or hemorrhagic blisters on his right upper extremity. Two weeks before the admission, pain on his elbow made him take acupuncture therapy, There was severe tenderness, pitting edema, and his right elbow showed limited flexibility. The patient underwent wide debridement and drainage immediately. Liquefactive necrosis was seen in the subcutaneous fat and superficial fascia without muscular involvement. Group A B-hemolytic streptococci were grown from the tissue culture specimen.

8.Apoptotic Keratinocytes in Acrodermatitis Enteropathica.

BACKGROUND: The cause of acrodermatitis enteropathica(AE) is closely related to zinc deficiency. Zinc is a potent inhibitor of endonuclease. Acute rises in the apoptosis in lymphoid and myeloid cell lines during zinc deficiency has recently been reported. The method of terminal transferase mediated dUTP biotin nick end labeling(TUNEL) is used in situ labelling of apoptotic nuclei in routine tissue sections. OBJECTIVE: The purpose of this study is to clarify our hypothesis that apoptosis resulted from zinc deficiency might cause keratinocytes damages in AE. METHOD: We stained 6 AE biopsy specimen with TUNEL technique. RESULTS: In acroderrratitis enteropathica, apoptotic keratinocytes were shown in the entire epidermis as compared to normal, controlled skin, in which it was found only at the uppermost layer of this stratified epithelium. CONCLUSION: This result suggests that apoptosis resulting from zinc deficiency might play a role in keratinocyte death in AE.

9.The Comparison of the Conversion Rate and Duration after Multipuncture BCG vaccination.

PURPOSE: During the childhood, Mantoux test is very popular for detecting tuberculous infection and also useful for evaluating the immunity against tuberculosis after BCG vaccination. But the response and conversion rate is affected by the age of vaccination, the quality of vaccine, the duration after vaccination and the frequency of the test. Recently percutaneous multipuncture technique of BCG vaccination was introduced and used because of local side effect and technical problem of intradermal BCG vaccination, however there is only a study of conversion rate and side effect at 3month after multipuncture BCG vaccination. We performed this study to evaluate of the difference of the Mantoux reaction and conversion rate according to duration (3month, 9month and 15month) after vaccination, gestational age, sex, and feeding type. METHODS: 174 neonates (155 of full term infants, 19 of preterm infants) who were born Kangnam St. Mary's Hospital were given multipuncture BCG vaccination within 4weeks after birth between July, 1995 and June 1997. All subjects was performed Mantoux test using 5TU PPD (NIH, Korea) at 3month, 9month and 15month. Injecting BCG and interpretating the result of the Mantoux test was done by the same person. RESULTS: 1) The positive rates of Mantoux test which was done 3month , 9month and 15month after vaccination were 38.7%, 58.1%, 71.6% in the full term infant group and 31.6%, 31.6%, 47.4% in the preterm infant group. 2) The size of induration in Mantoux test, which was done 3month, 9month and 15month after vaccination were 7.3 mm, 9.4 mm, and 10.6 mm in the full term infant group and 5.6 mm, 7.7 mm, 9.2 mm in the preterm infant group. The size of induration was significantly lower in the preterm infant group as compared with that of the full term infant group 9month after vaccination. 3) The positive conversion rate, less than 5 mm to more than 10 mm, was 6.3% (3 infants of 48 infants who were negative Mantoux test 3month became positive 9month) and 26.3% (5 infants of 19 infants who were negative 9month became positive 15month) in the full term infant group, and 33.3% (one infant of 5 infants who were negative 9month became positive 15month). 4) There is no difference in conversion rate of Mantoux test according to the sex, feeding type in both groups5) Complication rate for multipuncture is 3.2%, which is leaded by local ulceration and fever in the full term infant group, and 5.3%, which is only fever in the preterm infant group. CONCLUSION: In those result the size of induration and conversion rate is increased as the duration after vaccination, then their smaller size in the preterm group 9month after vaccination maybe due to immunological immaturity of preterm group.

10.Clinical Observation on Bladder Tumor.

A clinical observation was made on bladder tumors of the patients seen in the Department of Urology , Seoul National University Hospital during the period from July. 1967 to June, 1978. The following results were obtained: 1. During the period. 4312 patients were hospitalized, there were 381 cases of bladder tumors giving a rate of 8.8%. 2. The sex ratio was 4.3: 1 with 309 male and 72 female, and 75.1% of the cases were in the age group of 41-70. 3. Hematuria was the most common symptom and occurred in 78.0 %. 4. 76.2% of the cases visited to hospital within one year after the occurrence of initial symptom. 5. Among 147 cases in which IVP was performed. 66.0 % of the cases revealed normal upper urinary tract, and filling defects in bladder was observed in 61.2% of the cases. 6. Pathologic examination was possible on 210 cases.
