1.A Case of Lymphangioma Circumscriptum of the Vulva.

We present a case of lymphangioma circumscriptum in a 17 year-old girl according to tihe clinical and histopathological findings. This case is unusual in that lesions developed on the vulva and thigh without preceding lymphedema. Our patient had a plaque of grouped vesicle-like papules resembling frog's apawn on The both labia majora of vulva and several scattered, skin tag like soft papules on the right upper thigh of theree years duration. Histopathologic findings showed variable sized, dilatated lymphatic channels lined by single layer of normal endothelial cells confined to the only upper dermis.

2.A Case of Livedo Reticularis with Summer Ulcerations.

Livedo reticularis with summer ulceration was first reported by Feldaker et al in 1955, that is characterized by livedo reticularis and ulcerations of the legs or feet beginning primarily the summer or warmer months of the year, and healing the winter months. A 32-year old rnan has suffered from periodic painful ulcerations, which were occurred during the summer, on the both ankles and brown to bluish colored reticulated discoloration on the lower legs for about 10 years. Histopathological findings showed endothelial proliferation and partial occlusion of the dermal capillaries, fibrinoid material on the superficial blood vessels, and partial epidermal necrolysis. This patient. was treated with antiplatelet aggregating medications and antihypertensive drug.

3.A Study of Skin Thickness in Korean.

We studied skin thickness of the twelve body sites in 25 post-mortem subjects. The most thinnest and thickest sites of the skin layers are as follows(unit: um) l. 4.In silico Analysis of Downstream Target Genes of Transcription Factors.

OBJECTIVE: In the previous study, we compiled the differentially expressed genes during early folliculogenesis.1 Objective of the present study was to identify downstream target genes of transcription factors (TFs) using bioinformatics for selecting the target TFs among the gene lists for further functional analysis. MATERIALS AND METHODS: By using bioinformatics tools, constituent domains were identified from database searches using Gene Ontology, MGI, and Entrez Gene. Downstream target proteins/genes of each TF were identified from database searches using TF database (TRANSFAC(R) 6.0) and eukaryotic promoter database (EPD). RESULTS: DNA binding and trans-activation domains of all TFs listed previously were identified, and the list of downstream target proteins/genes was obtained from searche of TF database and promoter database. Based on the known function of identified downstream genes and the domains, 3 (HNF4, PPARg, and TBX2) out of 26 TFs were selected for further functional analysis. The genes of wee1-like protein kinase and p21WAF1 (cdk inhibitor) were identified as potential downstream target genes of HNF4 and TBX2, respectively. PPARg, through protein-protein interaction with other protein partners, acts as a transcription regulator of genes of EGFR, p21WAF1, cycD1, p53, and VEGF. Among the selected 3 TFs, further study is in progress for HNF4 and TBX2, since wee1-like protein kinase and cdk inhibitor may involved in regulating maturation promoting factor (MPF) activity during early folliculogenesis. CONCLUSIONS: Approach used in the present study, in silico analysis of downstream target genes, was useful for analyzing list of TFs obtained from high-throughput cDNA microarray study. To verify its binding and functions of the selected TFs in early folliculogenesis, EMSA and further relevant characterizations are under investigation.

5.Treatment of Patella Fracture

The results of treatment upon 56 cases of fracture of the patella, during the period from July 1975 to October 1978 at the department of orthopedic surgery, Soon Chun Hyang Medical College have been analysed. In sex distribution, 47 were male and 9 were female cases. The incidence was remarkably higher in male and the age group of 20-40. 2. 39 cases were fractured by direct force and 17 cases were indirect force. The most common cause of fracture was traffic accident. 3. The most common site of fracture was midpole and 27 cases were communited type. 4. Our methods of treatment were simple cast, circlege wire loop, tension band technique, partial and total and total patelletomy, etc. as indicated. 5, The mean duration of rehabilitation in the group of immobilized in 10 of flexion was shorter than that of full extension group.

6.Experience of Vascularized Joint Transfer for Segmental Loss of M-P Joint: A Case Report

Restoration of the M-P joint of a damaged or lost joint by trauma, tumor and others is necessary for adequate function of the hand. Several procedure have been described for restoration of the M-P joint. We experienced a case of vascularized joint transter for a lost joint by trauma. After 1 year and 2 months follow-up, there have been no degenerative change and no pain.

7.Lamotrigine and Lithium in the Treatment of Acute Bipolar Disorder.

BACKGROUND:To evaluate the efficacy of lamotrigine on bipolar disorder, both manic and depressed episode, the author compared the effects of lamotrigine with lithium and placebo. METHOD: Forty-seven manic patients with bipolar disorder and 43 depressed patients with bipolar disorder were treated with lamotrigine, lithium or placebo during 6 weeks. Their severity of symptoms were rated weekly using Young's Rating Scale for Mania and Hamilton Rating Scale for Depression. RESULTS: In manic patients the therapeutic efficacy of lamotrigine was lower than that of lithium during baseline to week 2, but after week 3, lamotrigine was as effective as lithium. In depressed patients the therapeutic efficacy of lamotrigine was superior to lithium from week 2 to endpoint. Most patients tolerated well except that one patient developed skin rashes. CONCLUSION: Lamotrigine seems to have antimanic and antidepressant effect as lithium in the treatment for acute bipolar disorder.

8.Superficial Mycoses in Patients with Diabetes Melitus.

BACKGROUND: It has been well known that patients with diabetes mellitus are prone to cutaneous infections, but whether superficial mycoses are truly more prevalent in diabetic patients is still controversial. OBJECTIVE: The purpose of this study was to determine the true prevalence of superficial mycoses in diabetic patients as comparing with non-diabetic, non-immunocompromised individuals. METHODS: We examined for evidence of superficial fungal disease in 137 diabetic patients and compared it with 146 non-diabetic non-immunocompromised ones. Potassium hydroxide preparations and fungus cultures were performed from all suspected lesions. RESULTS: Superficial mycoses were more common in diabetic patients(50%) than controls(31%) (p=0.0004), especially in the age group of 4th and 5th decades9p=0.011). The duration of diabetes mallitus or blood sugar levels were not significantly correlated with the prevalence of superficial mycoses.The organism most commonly isolated was Trichophyton rubrum in both groups(53% vs 79%) and Candida albicans was more prevalent in diabetic patients(34% vs 7%). More than 2 kinds of superficial mycoses were mose common in diabetic patients than controls(38% vs 16%). CONCLUSION: This study shows that the prevalence rate of superficial mycoses in diabetic patients is higher than that in controls and the in controls and the duration of diabetes mellitus or blood sugar levels were not significantly correlated with it.

9.Levels of Urinary Neopterin in Patients with Lepromatous Leprosy and Patients with Erythema Nodosum Leprosum.

In lepromatous leprosy, it is generally believed that there is not only defective CMl specific for M. leprae, but also generalized impairment of CMI and in erythema nodosum leprosum, an immune complex-mediated pathogenesis as well cell mediated immune pathogenesis have been proposed. Neopterin is a pyrazinopyrirnidine compound derived from GTP, its raised excretion has been related to activation of T-lymphocyte/macrophage axis. A study was performed to evaluate generalized CMI status in the LL and ENL and to investigate a relationship between levels of urinary neopterin and disease activity. Urinary neopterin was measured by high pressure liquid chromatography in 25 healthy subjects, in 25 patients with LL and in 25 patients with ENL. The results were as follaws 1. Urinary Neopterin levels of patients with LL was 188.9+147.3umol/mol creatinine, which was higher than that of control group(144.8+40.4umol/mol creatinine)(p<0.01). 2. Urinary Neopterin levels of patients with ENL was 884.1+970.5umol/mol creatinine, which was higher than of control group, and patients with LL(p<0.01, p<0.01). 3. Serial measurement of urinary neopterin from 1 week to 13 weeks after treatment of ENL in 4 cases of ENL showed good correlation between urinary neopterin levels and disease activity. In summary, it thus appears that measurement of urine neopterin in leprosy provides generalized CMI status and reliable index for activity of disease.

10.Imprint Cytologic Features of Epithelioid Sarcoma: A Case Report.

Epithelioid sarcoma is a malignant soft tissue neoplasm with an uncertain histogenesis. We report the imprint cytologic features of epithelioid sarcoma in the left shoulder of a 29-year-old male patient. Imprint cytologic findings showed dissociated and loose aggregates of anaplastic epithelioid cells on the necrotic, bloody, and inflammatory background. Tumor cells were round to polygonal shaped. Tumor cells had vesicular nuclei with abundant cytoplasm. The nuclei were irregular in shape and often eccentrically located. Some tumor cells were oval to spindle shaped. Binucleated and multinucleated cells were found. Intracytoplasmic vacuoles were present. On immunohistochemical stain, the tumor cells were positive for epithelial membrane antigen, vimentin, and CD34.
