2.Doppler Echocardiographic Prediction of Pulmonary Arterial Pressure in Ventricular Septal Defect.

This study was carried out to determine the accuracy of Doppler echocardiography for predicting the pulmonary arterial pressure from right ventricular systolic time intervals in 52 patients with ventricular septal defect. The diagnosis of ventricular septal defect was made by cardiac catheterization and angiocardiography at Dong San hospital, Keimyung University during the period of one year from jan. 1988 to Dec. 1988. Doppler measurements of acceleration time (AT), right ventricular ejection time (RVET), right ventricular preejection period (RPEP), AT/RVET, and RPEP/AT were compared with pulmonary arterial pressure (PAP), measured by cardiac catheterization. The patients were divided into 3 groups : PAP< or =30mm Hg, PAP 31-59mm Hg, PAP??0mm Hg. The following results were obtained. 1) In the groups of PAP< or =30mm Hg, AT was 0.12+/-0.01sec, AT/RVET was 0.47+/-0.07 and RPEP/AT was 0.50+/-0.05. 2) In the groups of PAP> or =60mm HG, AT was 0.06+/-0.01sec. AT/RVET was 0.28+/-0.05. RPEP/AR was 1.51+/-0.21. As the level of PAP increased, Doppler AT, AT/RVET and RPEP/AT showed significant change(P<0.001). 3) The Doppler AT showed relative high correlation(r=-0.76) with PAP measured by cardiac catheterization in all group. 4) The Doppler AT/RVET showed correlation(r=-0.70) with PAP. 5) The Doppler RPEP/AT showed high correlation(r=0.91) with PAP. The Doppler echocardiography was easy to apply in all age groups, and was found useful for detecting pulmonary hypertension in ventricular septal defect and for the follow-up check of the patients. It may help to determine the optimal time for surgery and evaluation of the treatment.

3.Clinieal Study on the Residual Effect of Pancuronium Bromide after Anesthesia.

The residual effect of muscle relaxant given during anesthesia can be assessed by clinical evaluation or by the use of peripheral nerve stimulator. Clinical evaluation of the recovery from neuromuscular blockade is often accompanied by some dangers of residual curarization after anesthsia because many of patinets evaluated by subjective clinical performance often have train-of-four ratio of less than 0.7 that is usually taken to reflect adequate reovery. Clinically the magnitude of antagonism of neuromuscular blockade appears to be dependent on the amount of muscle twitch at the time of anticholinesterase administration rather than on the total dose of muscle relaxant given during anesthesia. Authors evaluated the residual effect of pancuronium after anesthesia by the use of train-of-four(TOF) stimulation to the ulnar nerve. The degree of muscle relaxation was assessed by the train-of-four count, according to which varied dose of anticholinesterase was administered and the reversal time to a train-of-four ratio of 0.7 was checke.The result of this study is summerized below. 1) Reversal time without administration of neostigmine from 4th response to a TOF stimulation to a TOF ratio of 0.7 was 45.4+/-9.3 min. 2) Reversal time to a TOF ratio of 0.7 after adminstration of neostigmine 20ug/kg the 4th response to a FOF stimulation was 14.2+/-3.2 min. 3) Reversal time to a TOF ratio of 0.7 after administration of neostigmine 25ug/kg at the 3rd response to a TOF stimulation was 14.9+/- 4.7 min. 4) Reversal time to a TOF ratio of 0.7 after administration of neostigmine 40ug/kg at the 2nd response to a TOF stimulation was 10.6+/-3.4 min.

5.A longitudinal study on the interrelationship between the growth change of the gonial angle and IMPA.

This study was undertaken to investigate the growth change of gonial angle and interrelationship between the growth change of gonial angle and IMPA in the longitudinal data from 7 years to 15 years. By analyzing the serial lateral cephalograms of 15 males and 18 females of 7 years old to 15 years old who had no abnomality in growth and development and no history of orthodontic treatment, the following conclusion were obtained. 1. Gonial angle gradually decreased with aging in 25 children and in 9 children increased or maintained. 2. There was a tendency that children who decreased gonial angle with aging showed gradual increase of IMPA and children who increased of maintained gonial angle with aging showed decrease of IMPA 3. There was a tendency that regardless of the change of gonial angle, interincisal angle decreased with aging.

10.The Effect of Distraction - Compression on the Fracture Healing in Rats with Osteoporosis.

This study is verifying the hypothesis that distraction followed by recompression may promote fracture healing of the osteoporotic bone on the assumption that distraction induces increased callus formation at the distracted gap and recompression results in fusiform callus, having biomechanically stronger fracture stiffness. Sixty-six Sprague-Dawley rats, in which osteoporosis were induced 6-8 weeks after oophorectomy, were separated into two groups. Group I was control group that had fracture healing by simple external fixation. Group II was experimental group that had fracture healing by monofocal distraction-compression. Radiomorphometric study revealed that there was significantly increased amount of callus and ensuing bone formation, which becehe obvious from 20 days after osteotomy in group II compared with group I. Biomechanical study also showed that tensile break strength was significantly higher in group II compared with group I with time. It is concluded that monofocal distraction-compression method may be one of the useful method to promote fracture healing of the estrogen deficient osteoporotic bone.
