1.Clinical and Histophthologic Review in Patients with Henoch: Schonlein Purpura.

Authors review the clinical and histopathologic findings of 41 EIenoch-Schonlein purpur.a patients and attempt, to clarify the association between abnorma] urinary findings and anti-streptolysin 0(ASLO) titer and performed the direct immunofluorescence(',E)II) study on skin lesion to investigate the pathogenetic mechanim of renal involvement. The results were summarized as follows : 1. Ag(, distribution was form 3 month to 63 years and mean age was 18.9 years. male to female ratio was 1.2:1(22 males '. 19 females). 2. In he past history, there were preceding upper respora.tory infections in 25(61:o)patients and drug history in 10(24.3%)patients. 3. (linical manifestations in the order of frequency were as follows : arthralgia (60.9%), hematuria or albuminuria(43.8%), abdominal pain(21.9%), and positive occult: blood test(7.3%)in stool examination. 4. In the laboratory fiindings, leukocytosis was cleveloped in 14.6%, anemia in 2.4%, increased eryttrocyte sec1imentation zate in 12.3% increased ASLG titer in 43.9% of patients, and serum IgG was increased in 4 of 20(20.6)patierits, IgA in 4 of 21(19%)patients, IgM in 1fo 19(5.3o)patients. 5. Because correlation between increased A.LO titer ancl ahnoormal urinary findings wa.s high(x-test, p< 0.0,5), increased ASI,O titer may be inclicator of renal involvement. 6. DIF stuc].y was per formed in 34 patients. All showed positive reactiviy. IgG(17%), IgA(29.4% ), Clq(14.6%), C3(12.2% ), fibrinog.n(82.4%) were deposited around the vessel wall in a granular pattern. 7. There was statistically significant increase in mean serum IgG value in patients with abnormal urinary findings( t test, p < 0.05 ).

2.MRI of the diffuse axonal injury.

CT has facilitated early recognition and treatment of focal brain injuries in patients with head trauma. However. CT shows relatively low sensitivity in identifying nonhemorrhagic contusion and injuries of white matter. MR is known to be superior to CT in detection of which matter injuries, such as diffuse axonal injury. MR imaging in 14 cases of diffuse axonal injury on 2.0T was studied. The corpus callosum, especially the body portion, was the most commonly involved site. The lesions ranged from 5 to 20 mm in size with ovoid to elliptical shape. T2WI was the most sensitive pulse sequence in detecting lesions such as white matter degeneration, hemorrhagic and nonhemorrhagic contusion. The lesions were nonspecific as high and low signal intensities on T2WI and T1WI respectively. CT showed white matter abnormality in only 1 case of 14 cases. We propose MR imaging as the primary imaging procedure for the detection of diffuse axonal injury because of its multiplanar capabilities and higher sensitivity.

3.Inflammatory Pseud0tumor of the Liver: A case report.

Inflammatory pseudotumor of the liver is a relatively rare entity, and frequently misdiagnosed as a malignant tumor. We report a case of inflammatory pseudotumor involving the liver in a 53year-old man. The liver function test and serum alpha-fetoprotein level were within normal range. His preoperative diagnosis was as hepatocellular carcinoma by radiologic studies, and ultrasonography guided fine needle aspiration cytology and biopsy were done but confirmative diagnosis of malignancy or pseudotumor was not given. Grossly a relatively well marginated reddish brown soft mass with focal hemorrhage, measuring 5.0 cm in the largest diameter, was noted in the left lobe of liver. Surrounding hepatic parenchyma was yellowish brown in color without cirrhosis. Microscopically the mass showed typical findings of inflammatory pseudotumor and the ing liver tissue revealed diffuse fatty change and moderate chronic inflammatory cell on in the portal areas.

5.An Unusual Cause of Carpal Tunnel Syndrome: A Case of Tuberculosis of the Median Nerve

7.A Clinical Study of the Malgaigne Fractures

The incidence of pelvic fractures with or without dislocation of the sacro-iliac or symphysis pubis, is continually increasing. Such injuries not only are serious but can result in disabling complications and deaths. The author reviewed 57 cases of Malgaigne fractures treated at the orthopedic department of Kyungpook National University Hospital during the period from January 1978 to December 1977, and analized as to the cause of injury by mechanical forces, type of fracture, average duration for treatment and complications. We have adopted the classification of unstable fractures of Dunn and Morris. The fractures were classified as followes: vertical shear, 11: bucket handle, 8: lateral compression, 6: straddle, 5: total pelvic disruption, 4: dislocation, 3. Twenty-four patients were male thirteen, female. Traffic accident caused the injury in twenty-seven of the patients, accident of mine and factory, each three. Two fell from heights. Frequent associated soft tissue injuries were rupture of bladder (6 patients) and rupture of urethra (10 patients) The principle of treatment consisted of skeletal traction through the lower limb (supracondyle of femur) on the involved side and balance skin traction on the normal side. Pelvic sling were used. In lateral compression type, reduction was obtained by application of lateral skeletal traction with pelvic screw. Traction was continued for eight weeks. After discarding the traction physiotherapy start in bed and continued for further four weeks. Complete reduction of the displaced fragment and an adequate period of immobilization in recumbency is necessary to obtain the best results in treatment of Malgaigne fractures.

9.A Case of Dysplastic Melanocytic Nevus.

The dysplasitc melanocytic nevus(DMN) of the skin is an atypical-appearing melanocytic tumor characterized by intraepidermal atypical melanocytic preliferation. Clinically, DMN has a diameter greater than 5mm, ill-defined irregular borders, irregularly distributed pigmentation surrounded by erythems,. Histopat- hologically, there are numerous discrete nests of atypical melanocytes situated at the dermoepidermal junction of elongated rete ridges. In the dermis, concentric lamellar fibroplasia, lymphocytic infiltrates and neovascularization are noted. We report herein a case of DMN developed on the trunk of 31-year-old Korean male.

10.Talar Tilt Angle(A Comparative Study with Sagittal Mobility of the Normal Ankle)

The object of this study was to determine the physiological range of talar tilt angle of the Korean and to establish the basis for diagnosis and treatment of the lateral instability of the ankle. The anteroposterior inversion stress view of both ankles was taken in the 108 healthy Korean who had no history of ankle injury or disease. The sagittal stress films were also performed on 96 ankles (48 cases) of these to further define the physiological limits of the sagittal mobility of the normal talus. We compared these two values of normai mobility of talus to deterrnine the significance and relationship of the rwo. The results obtained were as follows: 1) Talar tilt over 10 degrees was seen in only 6 ankles (1.9%) and most of the ankles (279 ankles, 88.4%) showed a tilt less than 5 degrees. 2) Normal talar tilt angle was increased in 30 degrees plantarflexed position than in 90 degrees neutral position of ankle. In 12 cases the value was different between the right and the left; but there was no significant difference between the sexes. 3) The anterior displacement index over 200 was seen in only 5 ankles (5.2%). 4) There was gross positive inter-relationship between physiological. range of talar tilt angle and talar sagittal mobility.
