BACKGROUND: In order to restore severely attrited teeth properly, surgical intervention in the form of a crown-lengthening procedure may be required. And also, proper diagnosis and treatment sequencing is critical to obtain a successful results. Adequate diagnostic wax-up ensures good esthetics and healthy periodontal tissue. CASE DESCRIPTION: This clinical case report describes a diagnostically based protocol for restoration on mandibular anterior teeth with crown lengthening procedure and the treatment of partially edentulous mandible combined with an edentulous maxilla. In addition, the effort to prevent the combination syndrome was described. CLINICAL IMPLICATION: An interdisciplinary diagnosis and examination through visualization of the desired results ensure conservative and more predictable outcome.

PURPOSE: Intraductal papilloma (IDP) is a benign breast disease with malignant potential, for which complete surgical excision is usually recommended. The aim of the present study was to investigate predictive factors for upgrading patients with a benign papillary lesion (BPL). METHODS: This study was an observational study using a prospectively collected cohort. In total, 13,049 patients who underwent a core needle biopsy (CNB) for a breast lesion between January 2009 and May 2015 were enrolled. We reviewed all patients with pathologically confirmed BPL from a CNB. RESULTS: Surgical treatment was performed for 363 out of a total of 592 lesions. According to the pathological differences, the lowest upgrade rate was shown in IDP without atypia (without atypia, 6.0%; with atypia, 26.8%; papillary neoplasm, 31.5%; p<0.001). The univariate analysis showed that, in IDP without atypia, the age at diagnosis, size of BPL on ultrasonography, and density on mammography were associated with upgrading. The multivariate analysis revealed that age >54 years and lesion size >1 cm were significantly associated with upgrade to malignancy (odds ratio [OR]=4.351, p=0.005 and OR=4.236, p=0.001, respectively). CONCLUSION: The indications for surgical treatment can be defined as age >54 years and mass size >1 cm, even in IDP without atypia in the CNB results; this also includes cases of IDP with atypia or papillary neoplasm. Therefore, we suggest that close observation without surgery is sufficient for younger women with a small IDP without atypia.

PURPOSE: Many diagnostic modalities have been used for diagnosis of gastroesophageal reflux disease (GERD). Feeding materials during esophageal pH monitoring may interfere the result of examination and esophageal pH monitoring can not diagnose GER in case of alkaline reflux. The purpose of our study is to evaluate whether scintigraphy can substitiute 24 hr pH monitoring in children with GERD. METHODS: From March 2002 to June 2003, 23 patients (12 boys and 11 girls, mean age 27 months) who have been admitted to Hanyang University Hospital presented with GER symptoms (recurrent vomiting, cough, chest pain, irritability) were included in the study. Scintigraphy and 24 hr pH monitoring were performed in all patients. RESULTS: Six out of the 23 patients (26.1%) had evidence of GER on 24 hr pH monitoring, whereas nine of 23 patients (39.2%) exhibited GER by scintigraphy. Two out of the 23 patients could not be tested because of irritability. Three (14.3%) patients had evidence of GER on both 24 hr pH monitoring and scintigraphy, three (14.3%) patients on only 24 hr pH monitoring, six (28.6%) patients on only scintigraphy, and nine (42.9%) patients had no evidence of GER on both methods. No correlation was observed between 24 hr pH monitoring and scintigraphic results by Fisher's exact test (p=0.523) or Kendal's tau (t=0.678). CONCLUSION: The results of this study demonstrated that there was no correlation between 24 hr pH monitoring and scintigraphy. Therefore these modalities could be used as complementary tests to diagnose GERD.

OBJECTIVE: Currently hospitals are working hard to make their hospital stand out from the rest in the medical market. The government is trying to hard to resolve the problem in regards to false claims for treatment expenses through issued medical prescriptions. There is a known limit to 1:1 service which needs much improvement. This proposed system will help prevent fake medical prescriptions from being issued. METHODS: In order to provide distinguished medical services for the next generation, we have designed and implemented a system that applies the Radio Frequency Identification standard, web services and mobile technology. RESULTS: In the proposed system, the Radio Frequency Identification card is issued to the patient and the hospital system can automatically recognize the patient and a simple SMS message will be sent. Through this method of communication 1:1 customer service can be accomplished. By applying the web services technology, any pharmacy can use the saved medical prescription code contained in the Radio Frequency Identification tag. This in turn resolves the systematic problems with illegal usage. CONCLUSION: Through this proposed system, we offer the functionality of easily applying the Radio Frequency Identification technology to various medical fields. By using a mobile device, it is possible to collect bio data remotely about a patient.

Intussusception is rare in adult and usually caused by organic lesions, although there is a singnificant numbeof so-called idiopathic cases. The diagnosis of intussusception have been made by pali abdomen, barium enema andsmall bowel series. But recently ultrasound and CT make a contribution to diagnose intussusception. CT is not theprimary means for evaluation a gastrointestinal tract abnormality but also provides yaluable informations inevaluating disorders affecting the hollow viscera of the alimentary tract. CT image of intussusception demonstratea whirl like pattern of bowel loops separated by fatty stripe correlating to the intestinal walls. Abdominal ultrasonogram was used as the initial diagnostic test in 2 cases out of total 4 cases, with abdominal mass ofunknown cause. It revealed a typical pattern, composed of a round or oval mass with central dense echoes andperipheral poor echoes. We report 4 all cases of intussusception in adult who were performed by CT and/orultrasound. All cases were correlated with barium enema examination and/or surgical reports.

PURPOSE: Regardless of premature rupture of membranes, the genital colonization of Ureaplasma urealyticum (Uu) in pregnant women could transmit vertical infections of their babies and cause neonatal infections, which ultimately result in infections in many other organs. The purpose of this study is to determine clinical influences on the infants born from mothers suffering genital Uu colonization. METHODS: We retrospectively reviewed the data for analysis of the 219 infants born from mothers who had been tested for genital Uu culture (about 470 cases) for past 3 years from Mar. 2006 to Jun. 2009, in East-West Neo Medical Center, Kyunghee University. The neonatal clinical manifestations such as duration of tachypnea, incidence of oxygen therapy, respiratory distress syndrome (RDS), bronchopulmonary dysplasia (BPD), sepsis, jaundice, feeding intolerance, etc. were investigated during gestational period. RESULTS: In case of positive genital Uu culture of mothers, the preterm infants (gestational period <35 weeks) had symptoms of increased heart rate within 6 hours after birth, prolonged tachypnea, short period of antibiotics use, and lower incidence of RDS and hypocalcemia. Durations of oxygen therapy, the methods and durations of positive pressure ventilation and incidences of BPD of preterm infants were not affected by maternal Uu results. The near and full term infants (gestational period > or =35 weeks) from Uu positive mothers showed the increased incidence of jaundice. CONCLUSION: It is hard to say that genital Uu colonization in pregnant women influence the near and full term infants except jaundice. However, the data analysis of the preterm infants from Uu mothers demonstrated increased heart rate within 6 hours after birth, prolonged tachypnea, short period of antibiotics use, lower incidence of RDS and hypocalcemia.
