2.Effect of UVB Phototherapy on Pruritus of Pityriasis Rosea.

Pityriasis rosea is sometimes accompanied with considerable pruritus. Ultraviolet B irradiation is reported to be beneficial in relieving pruritus. The purpose was to evaluate the effect of UVB phototherapy on severe pruritus of pityriasis rosea. Nine patients with severe pruritus had UVB irradiation three times a week Five patients became symptom free and three showed a marked improvement. One dropped out. UVB phototherapy could be an effective modality for relieving pruritus in pityriasis rosea.

3.Nonpigmenting Fixed Drug Eruption to Etodolac.

Nonpigmenting fixed drug eruption is a form of fixed drug eruption which heals without pigmentation. To leave no traces makes the diagnosis difficult. A 37-year-old man pressented the erythematous patches on both wrists and the left thigh. Several hours before the eruption, he had taken oral drugs for the treatment of osteomyelitis. He had had the same eruptions which were diagnosed as an allergic contact dermatitis, 8 months ago. The patch tests with medicated drugs were negative, but oral provocation with etodolac brought the same eruptions.

4.Traumatic Gerstmann Syndrome: Report of a Case.

Gerstmann syndrome that was caused by a traumatic origin was very rare. We have reported a case of typical Gerstmann syndrome which was caused by a localized head injury. The case showed finger agnosia, right-left disorientation, acalculia and dysgraphia. And also the case revealed amnesic dysphasia and autotopagnosia. The lesion was an egg-shell typed depressed fracture(4 cm in diameter) of the left inferior parietal bone associated with the intracerebral hematoma in the lower parietal lobe which seemed to be the transitional area of the angular gyrus and the second occipital convolution.

5.Malignant Epidural Melanoma in the Frontal Region.

We have reported a case of dumbbell shaped maligfnant melanoma which developed and formed a large mass(2x7x5 cm) in the frontal scalp at the midline, eroded the bone, and invaded directly into the epidural region through the bony defect and formed another large tumor(3x7x8 cm) in the frontal fossa. Several small nodules of malignant melanoma were found in the anterior chest wall, too. In spite of the huge mass in the frontal fossa the case developed only dull headache without any neurological deficits. The frontal mass was successfully excised, of which pathological finding showed malignant melanoma.

6.Evaluation of the Iron Status in the Adolescents in Seoul.

BACKGROUND: During the adolescent growths spurt, iron deficiency becomes more common, particularly with the additional factors of menstrual blood loss in females and the androgen-related rise in hemoglobin concentration in males. This study was conducted to investigate the iron status in the adolescent students. METHODS: Venous peripheral blood was sampled from 178 males(79 of 12-13years, 99 of 15-16years) who were 1st grade students of two boy's middle schools and two high schools and from 186 females(87 of 12-13years, 99 of 15-16years) who were 1st grade students of two girl's middle schools and two high schools. Laboratory procedures included a hemoglobin, hematocrit, serum iron, TIBC and serum ferritin. Transferrln saturation was calculated. RESULTS: 1) Mean values were hemoglobin 13.7+/-0.78 g/dl, hematocrit 40.6+/-2.47%, serum iron 127.6+/-43.02 ug/dl, TIBC 387.9+/-47.73 ug/dl, transferrin saturation 33.3+/-11.38%, and serum ferritin 26.1+/-16.90 ng/ml in 12-13years old males. 2) Mean values were hemoglobin 13.7+/-0.85 g/dl, hematocrit 40.9+/-3.79%, serum iron 99.4+/-34.93 ug/dl, TIBC 387.8+/-45.53 ug/dl, transferrin saturation 26.2+/-9.99%, and serum ferritin in 22.7+/-14.18 ng/ml in 12-13years old females. 3) Mean values were hemoglobin 15.2+/-0.84 g/dl, hematocrit 45.9+/-2.77%, serum iron 144.9+/-51.97 ug/dl, TIBC 419.8+/-52.46 ug/dl, transferrin saturation 34.2+/-10.44%, and serum ferritin 31.2+/-20.91 ng/dl in 15-16years old males. 4) Mean values were hemoglobin 13.2+/-0.89 g/dl, hematocrit 39.9+/-2.78%, serum iron 111.1+/-39.78 ug/dl, TIBC 392.8+/-50.06 ug/dl, transferrin saturation 28.9+/-10.85%, and serum ferritin 21.4+/-17.11 ng/ml in 15-16years old females. 5) The prevalence of iron deficiency was 8.9% and 10.1% in 12-13years and 15-16years old males respectively. In 12-13years and 15-16years old females, the prevalence was 16.1% and 29.3% respectively. The prevalence of iron deficincy anemia was 1.1% and 2.0% in 12-13years and 15-16years old female and absent in male. CONCLUSIONS: Although hemoglobin is normal, we recommend to check other parameters to optimize the identification of individuals with iron deficiency.

7.Selective apoptotic effect of Zelkova serrata twig extract on mouth epidermoid carcinoma through p53 activation.

Apoptosis or programmed cell death plays an essential role in chemotherapy-induced tumor cell killing, and inducers of apoptosis are commonly used in cancer therapy. Treatment with Zelkova serrata extracts was performed in human gingival fibroblast (HGF), mouth epidermoid carcinoma cell (KB), lower gingival squamous cancer cell (YD38) and tongue mucoepidermoid carcinoma cells (YD15). We observed that extract prepared from Zelkova serrata twig selectively inhibited proliferation of various oral cancer cells, but not normal gingival fibroblasts, in a dose-dependent manner. Caspase-8-mediated apoptosis was induced by treatment with the extract only in mouth epidermoid carcinoma and not in other types of cancer cells, including lower gingival squamous cell carcinoma. The selective apoptotic effect of Zelkova serrata twig extract in mouth epidermoid carcinoma was dependent on normal p53 status. Apoptosis was not remarkably induced by treatment with the extract in either lower gingival squamous or tongue mucoepidermoid carcinoma cells, both of which contain abnormalities of p53. Upon treatment with Zelkova serrata twig extract, mouth epidermoid carcinoma cells accumulated in S phase by activation of p21. These data indicate that Zelkova serrata twig extract exerted a cancer type-specific, p53-dependent apoptotic effect and disturbed the cell cycle, which suggests that herbal medicine could be a treatment for specific types of cancers.

8.Effects of Ultra-Marathon Running on Pro-inflammatory, Anti-inflammatory and Endothelial Function Factors in Athletes with Exercise-induced Hypertension.

Running for a long distance in runners with exercise-induced hypertension (EIH) increases high blood pressure (BP) which causes myocardial burden, inducing elevation of cardiac markers. It is assumed to be associated with endothelial dysfunction, but has not been studied so far. The purpose of this study is to exam the responses of anti-inflammatory, inflammatory, and endothelial regulation factors to 100 km ultra-marathon of athletes who have EIH. For this study, we obtain and analysis interleukin-6, tumor necrosis factor-α, interleukin-10 (IL-10), monocyte chemoattractant protein-1, creatine kinase and nitric oxide (NO) from 100 km ultra-marathoners before and right after the 100 km race. There were 40 volunteers who had ever won the 100 km marathon race before more than one time. They had the graded exercise test with measurement of BP for assessing EIH. Among them, eight athletes were categorized to normal control group (NCG) and 10 athletes were assessed by EIH group (EIHG). The graded exercise test with treadmill was performed for the maximal strenuous exercise and BP. EIH was diagnosed as <140/90 mm Hg in normal and ≥210 mm Hg in maximal exercise. Marathoners including EIHG (n=10) and NCG (n=8) participated in the race and blood extracted from them before the race and right after the race. As results, EIHG had lower IL-10 than NCG during resting states and after the 100 km race EIHG had significantly lower NO concentration than NCG. Therefore, the NO in the endothelial is inhibited during strenuous endurance exercise, resulting in vasoconstriction with increasing the BP.

10.A clinical survey of prolonged Q-T syndrome among Korean children with congenital deafness.

The clinical symptom complex characterized by syncopal attacks and sudden death in patients with electrocardiographic anomalies, especially a prolonged Q-T interval is known as the Romano-Ward syndrome. When a similar symptom complex is accompanied by congenital deafness, it is called Jervell and Lange-Nieisen syndrome. This study was undertaken to investigate the investigate the incidence of the latter syndrome in the Seoul area. A total of 1,013 children with congenital deafness who attend schools for the deaf were studied by analyzing their electrocardiograms. A corrected Q-T interval of> or =0.44 seconds was defined as a prolonged Q-T interval. The overall incidence of long Q-T syndrome observed in the deaf in the Seoul area was 0.49% as compared to 0.25% reported from elsewhere in the world. The mean age of patients with long Q-T syndrome was 11.3+/-5.9 years and the first syncopal attacks was 4.6+/-1.1 per patient. A valsalva maneuver and exercise tests in the test group were associated with significant changes in T wave configuration and prolonged Q-T intervals while similar changes were not observed in the control group. A larger survey is needed to derive more statistically significant conclusions.
