1.Two Cases of Extramammary Paget' s Disease Showing a Good Response to Radiotherapy.

We report the clinical evolution of two elderly male patients with extramammary Paget's disease who were treated with radiotherapy. Both cases primarily involved the scrotum and penile root, and were not associated with any underlying or visceral malignancy. The total doses administered were 6000 cGy and 3000 cGy, respectively. A good clinical response was achieved in both cases. No major side effects were observed. The patients are being followed for any signs of recurrence and have been completely asymptomatic to date

3.Development of Integrated ECG Management System based Web with CORBA.

In these days, computing environment turns to web environment and a lot of hospitals build information system. A medical services through the web provide patients and doctors with convenience and help experts to communicate with each other. Initially, web provided static text information, but nowadays needs of interactivity between client and server is increasing. In order to diagnose heart diseases, we use 12 channel ECG, stress ECG, holter ECG, etc. These data are a lot of correlation with each other but there are little system managed them with integration. To enhance management efficiency and share medical information, it is necessary to develop integrated ECG management system based the web. In this paper, we use common object request broker architecture(CQRBA) to implement the system. The CORBA is to apply object oriented concept to web. We can implement application easily and reduce costs to maintain and repay because server objects and client objects exist independently. We developed a System to integrate these data using network and to raise management efficiency. we focus on the performance of object web and the implementation of ECG management system based on the web.

5.Postoperative Antiemetic Effect of Ondansetron and Droperidol in Female Patients Undergoing Breast Surgery.

BACKGROUND: Postoperative nausea and vomiting (PONV) is one of the most common complications following surgery performed under general anesthesia, especially in female patients. A reduction of PONV would improve the overall satisfaction of patients with their hospital experience and the quality of patient care. We have compared the efficacy of ondansetron to droperidol and saline in the prevention of PONV in 90 ASA 1 and 2 patients undergoing breast surgery. METHODS: Patients were randomly divided into four groups to receive pretreatment with a placebo 20 ml, droperidol 1.25 mg and ondansetron 4 mg, or 8 mg. Postoperatively, all episodes of PONV experienced by the patients during the first 24 hours after anesthesia were recorded by study personnel without knowledge of which antiemetics the patients had received. RESULTS: There was no significant difference in the incidence of PONV between the four groups. No major adverse effects were observed in the ondansetron or droperidol groups. CONCLSIONS: The present study demonstrates that droperidol 1.25 mg, ondansetron 4 mg, or 8 mg IV are not superior to a placebo IV in preventing PONV after breast surgery.

6.Two Cases of Lymphangioma in Genitourinary System.

Lymphangiomas are uncommon tumors that are believed to result from abnormal communication between large dermal lymphatic channels and central lymphatic system. Predilection sites are the chest, neck, axilla, buttock and thigh but may occur on almost any area of skin. Retroperitoneum is very uncommon site of lymphangiomas. They show the highest incidence of onset in infancy, the majority are present by age 5 years. Three types are distinguishable: 1) superficial lymphangioma circumscriptum and 2) deep lymphangioma cavernosum and 3) cystic lymphangioma (hygroma). We report two cases of cavernosal type of lymphangiomas with asymptomatic abdominal mass in 4-year-old female and perineal mass in 2l-year-old male patient.

7.Ki-1 Positive T-Cell Lymphoma of Bone in a Child.

Ki-1 monoclonal antibody is a well known marker for Reed-Sternberg cells in Hodgkin's disease, but also occasionally reacts with activated lymphoid cells of either benign or malignant nature. Recently, Ki-1 antibody positive Non-Hodgkin's lymphoma, usually of large cell and/or polymorphous type, has been reported in the lymph nodes, skin, soft tissue, and stomach, but not in the bone. We report a case of multifocal primary bone lymphoma in a seven-year old body involving the left shoulder and right frontal bone, which proved to be a large cell, polymorphous lymphoma, helper T-cell type expressing Ki-1 antigen.

8.A Case of Extensive Classic Kaposi' s Sarcoma Showing Good Response to Radiotherapy.

We report a case of classic Kaposis sarcoma(KS) in a 74-year-old man who had asymptomatic red-dish-blue or dark-brown macules, papules and nodules on his bo hower extrimities. The biopsy specimen showed typical histopathologic features of FS including a highly vascularized lesion with a proliferation of spindle-shaped cells accompanied by etravasated erythrocytes and variable number of inflammatery cells. Many of the spindle-shaped cells associated with the proliferating vessels in KS lesion expressed factor VIII-related antigen. He was treated with 1500 cGy of radiation(300 cGy per day for 5 successive day. Four weeks after radiotherpy, gross and histopathologic findings were mucl improved and healed leaving hyperpigmentation.

9.Treatment of Cardiac Neurosis with Propranolol (Inderal(R)).

Thirty-seven cases of cardiac neurosis were treated with propranolol (Inderal(R)) and following results were obtained. 1) Symptoms, such as palpitation, dyspnea, chest tightness and chest pain were disappeared or improved in all cases. 2) Heart rate reduced significantly after treatment from 99.8 per minutes to 74.8 per minutes. 3) Systolic blood pressure reduced significantly after treatment from 147.6 mmHg to 131.4 mmHg. but diastolic blood pressure were not reduced significantly. These results indicated that propranolol is recommendable medicine for improving cardiac symptoms and tachycardia in cardiac neurosis.

10.An Immunohistochemical Study of Oncogene Interaction in Skin Tumors.

BACKGROUND: Alteration of a wide variety of cellular oncogerms have now been implieated in the causation of many human cancers. However, genetic studies have indicated that an abnormality of a single gene is usually insufficient to elicit the fug transformed phenotype and that two or more genetic leaions may be necessary for this to occur. OBJECT: The purpose of this study is to examine the possibity of oncogene interaction which might be involved in the pathogenesis of human skin tugois. METHODS: The expression of the epidermal growth factor rattor(EGFR), c-jun protein, c-fos protein and p53 protein was assessed in frozen and corresgoAhng paraffin-embedded sections of 37 separate skin lesions (6 actinic keratoses, 6 squamo is ell carcinomas and 25 basal cell carcinomas using immunohistochemistry. RESULTS: Substantial number of the p53 positive cases were negative for c-fos, in contrast to normal tissue, which was p53 negative and c-fos positive. The negative correlation between p53 and c-fos staining was statistically significant(P<0.005). CONCLUSION: Although p53 appeared to have lost its normal role of regulatory early S phase protein in some lesions, in others high levels of p53 were assocated with underexpression of c-fos, reflecting the diversity of c-fos oncogene and the possible down regulation of c-fos expression by high levels of p53 protein.
