4.A Case of Angiokeratoma of the Vulva.

We report s case of angiokeratoma of the vulva occuring in a 36-year-old woman. Numerous grayish dark purple papulovesicular lesions, ranging from 0.2 to 2.0mm in size, were observed on the right labium majus. All lesions were excised under local snesthesia after the diagnosis of angio keratoma was made histopathologically. No new lesions appeared during the follow-up period of one year.

5.An Evaluation of the Accuracy of Mini-Wright Peak Flowmeters in Patients with Asthma and Chronic Obstructive Pulmonary Disease.

BACKGROUND: The peak flowmeter is very useful in monitoring of out-patients as well as those in emergency departments because of its convenience and simplicity with low cost. There have been many studies aimed at determining the accuracy and reproducibility of the peak flow meter in normal population. However, there is a paucity of reports regarding its accuracy in patients with chronic obstructive pulmonary disease(COPD) or asthma. The accuracy of the peak expiratory flow(PEF) measured with a mini-Wright peak flowmeter was assessed by a comparison with the results of a mass flow sensor. METHODS: The PEF measurements were performed in 108 patients aged 19-82 years presenting with either a chronic obstructive lung disease or asthma before and after inhaling salbutamol. The PEF measurements from the mini-Wright flowmeter were compared with those obtained by the calibrated mass flow sensor. RESULTS: The average of the readings taken by the mini-Wright meter were 37-39 l/min higher than those taken by the mass flow sensor. The average percentage error of the mini-Wright meter were higher, ranging less than 300 l/min. The mean of the differences between the values obtained using both instruments (the bias)±limits of agreement(±2 SD) were 37.1±90 l/min for the PEF(p<0.001). CONCLUSIONS: The mini-Wright peak flowmeter overestimated the flows in patients with COPD or asthma. It was also found that the accuracy of the mini-Wright peak flowmeter decreased in its mid to low range. The limits of agreement are wide and the difference between the two instruments is signigicant. Therefore, the measurements made between the two types of machines in patients with asthma or COPD cannot be used in terchangeably.

6.The Effect of the Solution-focused group counseling program on problem-solving capacity of the nursing students.

PURPOSE: To examine the effect of the solution-focused group counseling program on the nursing students' problem-solving capacity. METHODS: The research design was one-group pretest-posttest design in quasi-experimental research. The subjects were 16 sophomore nursing students being in K university located in D city. Korea. The period of this research was from January 9 to February 20, 2002. from January 10 to February 21. 2003. The group counseling program that was developed by Kim, Hyeon Mi (2001) on the basis of the fundamental principles of solution-focused therapy of de Shazer. et al. (1986) and the solution-focused group counseling process proposed by Selekman (1991). LaFountain. et al. (1996) and Metcalf (1998) was used after being modified and complemented for the nursing students. The group counseling was carried out six times for 120 minutes per week. The instruments used for this study were the Problem Solving Inventory developed by Heppner and Petersen (1982) and Scaling question. Collected data were analyzed by paired t-test with SPSS program. RESULTS: 1) Problem solving capacity increased after solution-focused group counseling program, but a significant difference was not revealed(t=0.835, p=0.057). 2) The scores of self-confidence with the problem-solving were significantly increased when it was compared with the 1st and the 6th(t=3.587, p=0.001). CONCLUSION: It seems that the solution-focused group counseling program should be considered as an effective nursing intervention program for the nursing students by improving self-confidence with the problem-solving. As a results, this study suggests that the specific techniques used in the solution focused therapy should be applied actively in areas of counseling and communication training for the nursing students and nurses.

7.A Case of Myeloid Blast Crisis of Ph-positive Chronic Myeloid Leukemia with t(3;21)(q26;q22).

The t(3;21) (q26;q22) is associated with chronic myelogenous leukemia in blast crisis, leukemia evolving from therapy-related myelodysplasia, and with leukemia following other hematopoietic proliferative diseases. The t(3;21) is rare secondary aberration in blast crisis of Philadelphia(Ph)-positive chronic myeloid leukemia, which may be restricted to patients entering myeloid blast crisis. We report here in one case of chronic myeloid leukemia in blast crisis which reveals both t(9;22) (q34;q11), and t(3;21) (q26 ;q22). A 62-year-old male was diagnosed as chronic myeloid leukemia 5 years ago, received hydroxyurea therapy, and admitted because of gingival bleeding and fever. On examination, splenomegaly and leukocytosis with proliferated blasts(91%) in peripheral blood were noted. Bone marrow aspirate showed hypercellularity with severe blast proliferation(92.5%) which revealed all negative in peroxidase and PAS stain. Cytogenetic study of bone marrow cells showed the karyotype 46, XY, t(3;21) (q26;q22), t(9;22) (q34;q11), which might be suspected as myeloid blast crisis. Above finding was confirmed by the result of immunophenotyping(CD13 43.6%, CD34 68.2%, HLA-DR 91.6%). He received intensive chemotherapy, but still sustained proliferation of blasts was noted . The follow up cytogenetic study was as follows: 46, XY, 4(3;21) (q26:22), t(9;22) (q34;q11)/46, XY, t(3;21)(q26;q22), del(8) (q22), t(9:22) (q34,q11)/46, XY (16/3/1). He died soon from severe pancytopenia and sepsis.

8.Hypotensive Efficacy and Safety of Manidipine on the Patient with Essential Hypertension.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

9.Whole Layer Photocoagulation on the Rabbit Retina with Indirect Diode Laser Ophthalmoscopy.

In the treatment of retinopathy of prematurity, it is necessary to coagulate retina including inner retinal layer to achieve regression. However, diode laser has long wavelength, thus damage to bruch`s membrane or choroid can occur when one attempts to coagulate whole retinal layers. We evaluated whether or not we could coagulate whole retinal layer without damage to Bruch`s membrane or choroid in rabbit eyes with indirect diode laser ophthalmoscopy. We palaced laser spots on retina of pigmented rabbits with various laser powers(150, 300, 500mW) and durations(0.3, 0.5, 1.0 second). Fundus photographs were taken immediately after photocoagulation. then we took fundus photographs and enucleated the eyes at one day, one week, 3 weeks, and 5 weeks after photocoagulation to perform pathologic examination. With laser setting of 150mW and 0.3 second, the coagulated retina revealed damaged outer layer but inner retinal layer was spared. We could coagulate whole retinal layer without any damage to Bruch`s membrane or choroid with laser power of 300mW and 0.3 second duration. In 300mW, 0.5 second laser burns and 500mW, 0.3 second burns, internal limiting membrane remained intact while Bruch`s membrane and whole retinal layers were damaged We found whole layer rabbit retina could be coagulated without damage to Bruch`s membrane and choriocapillaries with indirect diode laser opthalmoscopy. and we think it would be necessary to adjust laser power and duration in clinical situation to minimize possible deep tissue injury by diode laser.

10.Cystic Thymic Diseases: CT Manifestations.

PURPOSE: To describe CT findings and differential points of cystic thymic lesions. MATERIAL AND METHOD: We evaluated retrospectively total 19 masses with well marginated cystic lesions at thymic area on CT scans. They were 10 teratomas, 3 congenital thymic cysts, 2 multilocular thymic cysts(associated with thymoma and myasthenia gravis in each), 2 cysts Assciated with thymic Hodgkin's lymphomas an ectopic parathyroid cyst, and an infected thymic cyst. The radiological abnormalities evaluated were thickness of the wall, presence or abscene of septa, mural nodule, solid component, calcification and fat component. RESULTS: All three cases of congenital thymic cysts and an ectopic parathyroid cyst appeared as thin-walled unilocular cyst with homogeneous internal density and without identifiable solid component. In multilocular thymic cyst, there were thick wall and solid components(n=2), thick internal septa and calcifications(n=l). The cysts of teratomas manifested thick walls(n=9), internal septa(n=4), calcifications(n=6), fat components(n=4), and solid components(n=4). Cysts in Hodgkin's diseases appeared as multilocular or unilocular and had thick wall and septa without calcification. infected thymic cyst presented with multilocular cystic mass with identifiable wall and septa, calcification, and solid components. CONCLUSION: The thymic diseases with cystic lesion include teratomas, congenital thymic cysts, multilocular thymic cysts, parathyroid cyst, .and Hodgkin's disease. Congenital thymic cyst and ectopic parathyroid cyst are thin-walled unilocular cystic lesions. Cystic lesions associated with teratoma, Hodgkin's disease, and multilocular thymic cyst are thick-walled cystic lesions with or without solid component.
