2.Laparoscopic transabdominal extraperitoneal mesh repair of lumbar hernia.

Lumbar hernias are rare posterolateral abdominal wall defects that may be congenital or acquired. There are two types of lumbar hernia, the superior lumbar hernia through Grynfeltt triangle, and the inferior lumbar hernia through Petit triangle. Many techniques have been described for the surgical repair of lumbar hernias including primary repair, local tissue flaps, and conventional mesh repair. But these open techniques require a large skin incision. We report a case of superior lumbar hernia, which was successfully repaired using a laparoscopic approach.

3.A Clinical Study of Epiphyseal Injury About the Knee

A Epiphyseal injury in children is common following trauma to result in significant and permanent loss of function and growth. We analyzed 24 cases of epiphyseal injuries about the knee inchildren, who were treated in the Department of Orthopaedic Surgery, Yonsei University, Wonju College of Medicine from Jan. 1980 to Dec. 1987. The results were as follows :1. The sex ratio between male and female eas 2.5: 1, and mean age was 11 years old. 2. Large number of cases were by slip-down, traffic accidents or fall. 3. Epiphyseal injuries about the knee occurred in 12% among the 417 cases of childrens epiphyseal injuries. In point of injury patterns, large number of cases were Salter-Harris type 2 and type 3 injuries in the distal femur and the proximal tibia. 4. They were treated with closed reduction and plaster immobilization in 66%, with operative reduction and internal fixation in 34%. 5. Complications occurred in 6 cases : Knee pain and limitation of motion, angular deformity and limb shorteniing.

4.The Production and Evaluation of the Tissue-equivalent Phantom for the Magnetic Resonance Imaging.

PURPOSE: For the production and evaluation of the tissue-equivalent phantom. MATERIALS AND METHODS: We used agarous gel and oil as a basic component of the mixture and added Tween 80 for the stabilization of phantoms. We did the test for homogeneity and measured T1 and T2 relexation times of each phantom tube. RESULTS: T1 relaxation time ranged from 642 to 2781 msec and T2 relaxation times from 42 to 157 msec. Each phantom was significantly different in T1 relaxation time and T2 relaxation time (p < .0001). CONCLUSION: Tissue equivalent phantom may provide good information on the optimal sequence before MR imaging of patients and may be valuable if it is used with the patients' MR imaging.

5.Microtensile bond strength of single step adhesives to dentin.

This study compared the microtensile bond strength (microTBS) of three single step adhesives to dentin. Occlusal superficial dentin was exposed in fifteen human molars. They were assigned to three groups by used adhesives: Xeno group (Xeno III), Prompt group (Adper Prompt L-Pop), AQ group (AQ Bond). Each adhesive was applied to dentin surface, and composite of same manufacturer was constructed. The bonded specimens were sectioned into sticks with an interface area approximately 1 mm2, and subjected to microTBS testing with a crosshead speed of 1 mm/minute. The results of this study were as follows; 1. The microTBS to dentin was 48.78 +/- 9.83 MPa for Xeno III, 30.22 +/- 4.52 MPa for Adper Prompt L-Pop, and 26.31 +/- 7.07 MPa for AQ Bond. 2. The mean microTBS of Xeno group was significantly higher than that of Prompt group and AQ group (p < 0.05). 3. There was no significant difference between the microTBS of Prompt group and AQ group.

6.A Predictive Index of Left Ventricular Performance after Mitral Valve Replacement in Pure Mitral Regurgitation.

The index on myocardial contractility of pure mitral regurgitation(MR) after mitral valve replacement is believed to be useful in determining proper operation time before irreversible myocardial damage by volume overload. Thus the authors examined pre and post-operative echocardiographic results of 20 cases of pure MR patients who had been admitted to Pusan National University hospital and compared the usefulness of each index. Pre and post-operative echocardiographic results were as follows respectively ; 1) Left ventricular internal dimension in diastole(LVIDd) were 6.49+/-0.19cm and 5.51+/-0.17cm. 2) Left ventricular internal dimension in systole(LVIDs) were 4.26+/-0.12 and 3.79+/-0.18cm. 3) Ejection fraction were 71.28+/-1.57% and 59.24+/-3.05%. 4) Fractional shortening(FS) were 34.09+/-1.18% and 27.21+/-1.84%. 5) Mean Vcf were 1.53+/-0.08cm/sec and 1.18+/-0.09cm/sec. 6) Left ventricular end-diastolic volume index(EDVI) were 196.35+/-18.33cc/m2 and 98.46+/-9.96cc/m2. 7) Left ventricular end-systolic volume index(ESVI) were 55.28+/-5.12cc/m2 and 41.88+/-6.07cc/m2. 8) Left ventricular end-systolic wall stress/ESVI(ESS/ESVI) were 2.3+/-0.21 and 3.91+/-0.83. ESS/ESVI showed significantly(p<0.05) elevated postoperative change, LVIDd, LVIDs, EF, FS, EDVI, ESVI revealed significantly(p<0.05) reduced postoperative change whereas Mean Vcf had no postoperative change. After examining correlation between preoperative and postoperative results of each index, it is believed that LVIDd, LVIDs, and ESS/ESVI were useful prognostic indices but were unrelated to postoperative ejection phase indices such as EF, FS, and MEan Vcf. Especially in case of LVIDd>6cm, LVIDs>4cm, and ESS/ESVI<2, the prognosis was poor due to myocardial damage by mitral regurgitation.

7.Doppler Flow Patterns of Constrictive Pericarditis.

To recognize the hemodynamic change in the constrictive pericarditis, we have reviewed the Doppler echocardiography, cardiac catheterization, and pathology of 6 patients who were admitted to Pusan National University Hospital due to right-side heart failure, diagnosed as constrictive pericarditis and were undertaken pericardiectomy. Doppler echocardiographic findings showed that acceleration and deceleration of early diastolic rapid filling were increased, followed by shortening of duration but there was a tendency to decrease in velocity-time integral of early diastolic rapid filling compared to that of atrial contraction filling in the left ventricle. On the contrary, there was decrease in acceleration of rapid filling in right ventricle, but other indices were comparable to that of left ventricle. The integral of D wave increased relatively compared to that of S wave in superior vena canal flow. Also, there was decrease in peak flow velocity, acceleration and velocity-time integral of aortic and pulmonary arterial flow velocity. One patient who had increased fraction of integral of early diastolic rapid filling compared to that of atrial contraction filling in right ventricle showed that he had higher central venous pressure and D wave was dominant in superior vena caval flow. In conclusion, Doppler flow patterns showed characteristic diastolic filling inpairment and systolic dysfunction in constrictive pericarditis, so that Doppler echocardiography is believed to be a useful method of noninvasive diagnosis and follow-up of hemodynamic change in constrictive pericarditis.

8.A Case of Subcutaneous Granuloma Annulare.

We report herein a case of subcutaneous granuloma annulare occured in a 3-year-old girl who had 7 subcutaneous nodules on the scalp, lumbar area, buttock, right tibial area and both ankles. liistopathologically, multiple foci of complete and incomplete collagen degeneration surrounded by histiocytes in a palisading arrangement were seen in the subcutaneous layer. Some vessel walls were thickened and infiltrated with inflammatory cells and many eosinophils were scattered among the inflamrnatory cell infiltrate around the palisaiing granuloma. Five of the lesions disappeared spontaneously and other s decreased in size without specific treatment in 2 months.

9.Atypical Eruption Due to Chemotherapeutic Agent.

We report a case of atypical eruption due to chemotherapeutic agent in a 60-year-old man who presented with asymptomatic, erythematous, 0.5cm in diameter, confluent, and elevated papules and plaques confined to the face. The patient was previously diagnosed with small cell carcinoma of the lung with liver metastasis. Two months after the diagnosis, a first course of chemotherapy including etoposide was started. Five days after starting the chemotherapy, the patient developed a facial eruption. Histopathologic examination demonstrated increased epidermal mitotic figures, cells in metaphase arrest, basal cell layer hyperpigmentation, prominent dyskeratosis, and squamous atypia. The most distinctive histologic feature was the presence of starburst cells, which are markedly enlarged pale staining keratinocytes containing small basophilic fragments of nuclear debris haphazardly scattered throughout the cytoplasm in a starburst pattern.

10.Anticardiolipin Antibody in Elderly Ischemic Stroke.

BACKGROUND: Although anticardiolipin antibody(aCL)-related stroke is far more frequently identified in younger populations, the role of aCL in the pathogenesis of cerebral infarction may be important also in the old, stroke-prone population. We studied the clinical profiles of aCL- related stroke in elderly patient to look at its role on the pathogenesis of ischemic stroke. METHODS: We analyzed unselected patients with acute ischemic stroke and age matched controls for the presence of aCL, prospectively. Also, we studied the characteristics of these patients based on the conventional risk factors and other clinical, laboratory and radiological features in them. RESULTS: aCL was positive 30 (14.7%) of 203 stroke patients, but 11 (6.1%) in 193 control patients. The proportion of patients having more than one of the risk factors was significantly greater in aCL-positive (24/30, 80.0%) than in aCL-negative patients (100/173, 57.8%). The incidence of aCL positivity was significantly greater in patients having one or more risk factors (24/124, 19.3%) than in patients not having any of the risks (6/79, 7.6%). Two of the patients had prolonged aPTT, and only one had a lupus anticoagulant. A positive ANA and false-positive VDRL were not found in our patients. Radiological findings confer subcortical infarction. CONCLUSION: The elevated aCL are a risk marker for stroke also in the elderly population. Our aCL-positive patients generally had multiple risk factors for stroke and are associated with subcortical infarctions, contrary to previous studies. The role of aCL as a disease marker for ischemic stroke in elderly patients warrants further investigations.
