1.Treatment of Palmoplantar Hyperhidrosis with Iontophoresis.

In the present study, tap water iontophoresis as a method of treatment of palmoplantar hyperhidrosis was evaluated. Eighteen patients, twelve males and six females, with singnificant palmoplantar hyperhidrosis were treated on every weekday for 2 weeks. The hand and foot of the same side were randomly selected for the treatment by tap water iontophoresis according to the the method described by Levit. The selected hand and foot were immersed in a pair of pans containing tap water and exposed to 30V and 3 to 14mA of direct current for 20 minutes. The results obtained were as follows . Among 18 patients treated, 14(77.8%) accomplishei3 sufficient control of hyperhidrosis after 6 12(average 9.1) treatments. Two patients showed a moderate response and one weak response. One patient showed no response. Several patients experienced untoward effects such as mild itching, erythema, tingling sensation, exfoliation, compensatory hyperhidrosis, but the discomfort was not so severe as to stop the treatment

2.A Case of Desmoplastic Trichoepithelioma.

We report herein a case of desmoplastic trichoepithelioma in a 73-year-old female, who is the oldest among the patients of the disease reported in Korea. She had an asymptomatic, hard, annular lesion, measuring 0.6 x 0.7 cm with an elevated border and a depressed center on the left nasolsbial fold. Histopathological findings showed narrow strands of basaloid tumor cells, horn cysts and a desmoplastic stroma.

3.In Vitro Tissue Engineering of Cartilage using Autologous Fibrin Glue and Chondrocytes.

Neomorphogenesis of cartilage using chondrocyte-polymer constructs is a potential source for development of cartilage reconstruction. Current tissue engineering techniques of neocartilage rely on in vivo implantation of polymer-chondrocyte constructs. The purpose of this study was to find a way to bioengineer cartilage in vitro by entrapping chondrocytes in a molded autologous fibrin glue. Chondrocytes isolated from the cartilage of rabbit joints were combined with fibrinogen extracted by a single cryoprecipitation of autologous plasma, and they were then polymerized with thrombin to create a fibrin glue with a final cell density of 2.5x10(6) cells/ml. The collagen for a control study was used as a polymer. The polymer-chondrocyte constructs were cultured for 4 weeks and the fibrin-chondrocyte constructs molded in the shape of a human ear were cultured for 6 weeks in vitro. Morphometric, histochemical, and histomorphometric analysis including glycosaminoglycan quantitation confirmed the following results: 1) Highly-concentrated autologous fibrinogen was easily extracted by a single cryoprecipition of autologous olasma. 2) The fibrin-chondrocyte constructs demonstrated the presence of actively proliferating chondrocytes with the production of cartilaginous matrix(collagen and glycosaminoglycan) at 1 week after culture, as well as gross and histologic evidence similar to those of normal cartilage at 3-4 weeks after culture. 3) The collagen-chondrocyte constructs demonstrated lower degrees of hardness and transparency, as well as a lower density of cells and glycosaminoglycan during the culture period. 4) Neocartilage generated from fibrin-chondrocyte constructs in the shape of a human ear nearly retained their original configuration and size without degeneration for 6 weeks of culture in vitro. This study demonstrated a novel method for bioengineering the molded cartilage in vitro using autologous fibrin glue as a matrix scaffold. The generated cartilage showed gross and histologic evidence similar to those of normal cartilage, retaining the original gross dimension. With further refinement, this may be a new application of tissue engineering for the reconstruction of cartilage.

6.Intestinal parasite survey of Kyungpook National University Hospital patients.

For seven years from 1962 to 1968, fecal specimens of the patients were examined not only for the discovery of intestinal protozoa and helminths, but also to provie data on the incidence of intestinal parasites among the residents of Kyungpook Province, Korea. The Formalin-ether sedimentation, Lugol solution, and Heidenhain's iron-hematoxylin stained films were prepared for the recovery of intestinal parasites. Of 2,414 samples of feces examined, 35.7 percent were found to be infected with one or more species of protozoa. Among six species of protozoa detected, Entamoeba coli was observed most frequently(15.3%) followed by Entamoeba histolytica (11.9%), and then the Enteromonas hominis least often(1 case). For the survey of helminth, of a total of 5,288 fecal specimens examined, 86.7 percent were positive for one or more species of helminths. Trichocephalus trichiurus revealed the highest infection rate (83.6%), while Trichostrongylus orientalis was next (61.6%), and Ascaris lumbricoides was unexpectedly low(40.9%). The incidence for hookworm and Clonorchis sinensis was 22.4 percent and 29.8 percent respectively. Triple infections were found more frequently than double infections, and mixed infections of six or more species of helminths were observed in 3.8 percent of specimens.

7.A human infection of Echinostoma hortense in duodenal bulb diagnosed by endoscopy.

As gastroduodenoscopy performed more frequently, case reports of human echinostomiasis are increasing in Korea. A Korean woman presented at a local clinic with complaints of abdominal pain and discomfort that had persisted for 2 weeks. Under gastroduodenoscopy, two motile flukes were found attached on the duodenal bulb, and retrieved with endoscopic forceps. She had history of eating raw frog meat. The two flukes were identified as Echinostoma hortense by egg morphology, 27 collar spines with 4 end-group spines, and surface ultrastructural characters. This report may prove frogs to be a source of human echinostome infections.

9.Delayed Hemorrhage from Gastroduodenal Artery Pseudoaneurysm following Endoscopic Ultrasound-Guided Fine Needle Aspiration of Pancreatic Head Mass.

Endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) is a well-established procedure for the diagnosis of pancreatobiliary disease. Serious complications such as perforation, pancreatitis, hemorrhage, and sepsis are rarely reported. To our knowledge, delayed hemorrhage after EUS-FNA is very rare and hemorrhage from iatrogenic pseudoaneurysm has yet to be reported. We report a case of delayed hemorrhage from gastroduodenal artery pseudoaneurysm, which developed after EUS-FNA of a solid pancreatic lesion. A 68-year-old man presented with tarry stool 10 days after EUS-FNA of a 1.5 cm-sized pancreatic head mass. Abdominal computed tomography showed a 2-cm-sized intensely enhancing round lesion near pancreatic head. EUS-FNA was negative for malignancy. The patient refused admission for further evaluation. Twelve days later, he reported to the emergency room with persistent tarry stool. Angiography showed a gastroduodenal artery pseudoaneurysm. Subsequent coil embolization resulted in successful hemostasis. The patient underwent pylorus-preserving pancreaticoduodenectomy and was diagnosed with stage IIB pancreatic cancer.

10.Facial Rejuvenations in North East Asians.

In North East Asians, they have many anatomical characteristics different from Caucasians: thick skin, heavy subcutaneous tissue, relatively weak SMAS and platysma muscle, prominent zygoma and mandible. In addition, Caucasians have lean and narrow face, so facial flap can easily be lifted superiorly and posteriorly, but North East Asians have short and wide face, so facial flap cannot be lifted easily in a three dimensional direction especially superiorly and posteriorly. Recent facial rejuvenation has been improved to solve these problems through various combined adjuvant surgery. We performed facial rejuvenation with following surgical emphases:1. Shaving and infracture of zygomatic prominence and multistaged curved osteotomy of the prominent mandibular angle, body and symphysis 2. Earlier Skin resection before flap dissection protects the skin incision margin 3. Sufficient fat removal by facial liposuction4. Plane of dissection; suborbicularis and submalar fat pad in midface: preplatysmal layer in lower face and neck 5. Excision of lateral part of orbicularis oculi muscle for correction of crow's feet. 6. SMAS plication and platysmal sling. Based on our experiences, we offer these personal techniques for facial rejuvenation of North East Asians.
