PURPOSE: The purpose of this study was to evaluate the factors that affected self efficacy, social support and learning achievement in the web-based learning of graduate school students. METHODS: The data were collected from 79 nurses in a web-based learning. A self-reported questionnaire was used to assess the level of self-efficacy, social support and learning achievement. RESULTS: The learning achievement was significantly improved to 4.05 points after web-based learning compared to 2.69 points before web-based learning, and all other items were also improved. Self-efficacy was improved to 3.91 points from 3.66 points after web-based learning and so was social support to 4.18 points from 3.94 points. Fatigue and stress among health behavior characteristics were significantly decreased after web-based learning. CONCLUSION: This study shows that the web-based learning improved self-efficacy and social support of the students. And self-efficacy was an important factor in learning achievement. It is necessary to develop a strategy to improve self-efficacy as well as to manage social support for increasing interactions among the nurses in a web-based learning.

The purpose of this study was designed to develope and test the structural model that explains alcohol consumption behaviors among university students in Republic of Korea. The hypothetical model was constructed on the basis of the literature review and Pender's Health promotion model. Data was collected from questionnaires from 512 university students in Republic of Korea, from August to September, 2000. The reliability of instruments was adequate (Cronbach's alpha= .69-.90). Data analysis was done with SAS 6.12 for descriptive statistics and LISREL 8.13 program for covariance structural analysis. The results are as follows;1. The overall fit of the hypothetical model to the data was moderate. Thus it was modified by male and female models.2. The revised model has become parsimonious and had a better fit to the empirical data (male: x2=87.21 p=.00, GFI=.97, AGFI= .94, NFI=.99, NNFI=1.0, CN=619.17, female: x2=49.29 p=.31, GFI=.45, AGFI= .95, NFI=.99, NNFI=1.0, CN=370.02).3. Self-efficacy was most significant factor and personality of novelty seeking, reward compensation, alcohol expectancy and drinking attitude have significant effects on male alcohol consumption behavior. 4. Personality of novelty seeking was most significant factor and personality of harm avoidance, friend influence, self-efficacies, alcohol expectancy and drinking attitude have significant effects on female alcohol consumption behavior.

LPS and IFN-r induce nitric oxide synthase in macrophages and the resultant NO causes apoptotic cell death in the activated macrophages. NO production and apoptosis were inhibited by N-monomethyl L-arginine (NMMA), a competitive inhibitor of NO synthase. To study the role of BCL-X proteins, RAW 264.7 cells were transfected with the expression vectors with human bcl-Xl or bcl-Xs cDNAs, respectively. Stable transfectants were selected and confirmed by RT-PCR. NO production in response to LPS and IFN-r caused apoptosis in RAW 264.7 cells and vector transfected control cells within 24 hr. Both NO production and apoptosis were inhibited by N(G)-monomethyl L-arginine (NMMA). In contrast, bcl-Xs transfectant appeared slightly susceptible and bcl-X(L)< transfectant appeared slightly resistant, although NO production was similar to control cells. These results suggest that bcl-X proteins play roles in both positive and negative regulation of apoptosis induced by NO.

BACKGROUND: Recent evidences suggest that anesthetic action within the spinal cord is important in suppressing somatic responses to painful stimuli. Intrathecal endothelin-1 (ET-1) is known to have antinociceptive effect. The purpose of this experiment was to determine whether intrathecal ET-1 may influence the minimum alveolar concentration (MAC) of isoflurane in rats and access the role of the spinal cord as the sites of anesthetic action in blocking somatic responsiveness. METHODS: In Sprague-Dawley rats fitted with an indwelling intrathecal catheter, we determined the MAC of isoflurane using a tail-clamp technique as a painful stimulus, combined with end-tidal anesthetic sampling. In experiment 1, the control MAC was determined and changes of control MAC were observed after intrathecal ET-1 (4x10-2 nmol, 4x10-3 nmol) administration. In experiment 2, we observed the effects of L or N type Ca++ channel blocker such as verapamil (50 g) or W-conotoxin (0.5 g) on the MAC after measurement of the control MAC. In experiment 3, after measurement of the control MAC, ET-1 (10-2 nmol) was administered intrathecally and the MAC was determined again. Next, intrathecal verapamil (50 g) or W-conotoxin (0.5 g) was injected. After that, the MAC was determined again. RESULTS: In experiment 1, ET-1 decreased the MAC of isoflurane and its effect was sustained over 2 hours. In experiment 2, the MAC, determined following administration of verapamil or W-conotoxin, was not different from that of the control. In experiment 3, the MAC was decreased after ET-1 administration and then increased following injection of verapamil or W-conotoxin. CONCLUSIONS: These results suggested that ET-1, in relation to calcium, might play an important role in determining the MAC of isoflurane in the spinal cord.

Sunlight is one of the well-established factors which play key roles in the induction and exacerbation of lupus erythematosus. In two patients of discoid lupus erythematosus, we have experimentally reproduced skin lesions by provocative phototesting. Both UVA (100 joules/cm²) and UVB (80 millijoules/cm²) radiation induced the skin lesions. The reproduced skin lesions were clinically and histopathologically consistent with lupus erythematosus.

PURPOSE: This study is to conduct a comparative analysis of influencing factors on the experience of mammography targeting ordinary women and outpatients. METHODS: The target subjects were 116 ordinary women and 105 outpatients, and the study period was around 8 months from May to December. RESULTS: When mammography experience was examined, it was found that mammography experience was conducted in 44.8% of ordinary women and 59.0% of outpatients, but this study showed that there was a statistically significant difference. When the relation between the characteristics of the targeted objects and the experience of mammography was examined, it was found that ordinary women have a lot of experience when they have regular exercises and a high health belief. On the other hand, in case of outpatients. mammography experience was more frequent in the older group. In addition, outpatients had experiences in breast-related diseases or high self-efficacy. It was also found that the influencing factors on the experience of mammography were a high health belief in case of ordinary women, and old ages and high self-efficacy in case of outpatients. CONCLUSIONS: In order to increase the rate of conducting early detection behavior for mammography, it is needed to conduct an intervention that increases health belief for ordinary women, while it is effective to conduct an intervention that increases self-efficacy for outpatients.

Intravenous lidocaine has been used for attenuating cardiovascular responses to endotracheal intubation. To determine the optimal time of administration of lidocaine for attenuating hemodynamic changes induced by intubation, 100 patients were allocated randomly to a control group (without lidocaine) or four treated groups (with lidocaine). Lidocaine 1.5 mg/kg was administered intravenously at 60 (Group 2), 90 (Group 3), 120 (Group 4) and 180 seconds (Group 5) before intubation. Arterial pressure (systolic, mean and diastolic), heart rate and rate-pressure product (RPP) were measured before induction, after lidocaine injection, immediatly before intubation, immediately and 5 minutes after intubation. After intubation, arterial pressure, heart rate and RPP increased significantly in every study groups. In group 2 and 3, however, such changes were attenuated significantly, Postintubation elevation of systolic blood pressure, mean arterial pressure and RPP were effectively prevented in group 2 and 3. Lidocaine effect on diastolic pressure was observed only in group 3. Changes of heart rate were not affected with lidocaine adminiistration. In Group 4 and Group 5, this effect was not observed. In diastolic pressure, suppressive effect of lidocaine in increasing of blood pressure and RPP was observed in Group 3 significantly, but not in Group 2, Group 3 and Group 5. Changes of heart rate show that in all groups injected lidocaine (Group 2,3,4,5) significantly suppressive effect was not observed as compared with control group. These results suggest that intravenous lidocaine (1.5 mg/kg), 60-90 second prior to intubation attenuate hemodynamic responses induced by intubation.

OBJECTIVE: In the pediatric population the skull has not yet undergone ossification and it is assumed that the diagnostic rate of skull fractures by simple X-rays are lower than that of adults. It has been recently proposed that the diagnostic rates of skull fractures by 3-dimensional computer tomography (3D-CT) are higher than simple X-rays. The authors therefore attempted to compare the diagnostic rates of pediatric skull fractures by simple X-rays and 3D-CTs with respect to the type of fracture. METHODS: One-hundred patients aged less than 12 years who visited the Emergency Center for cranial injury were subject to simple X-rays and 3D-CTs. The type and location of the fractures were compared and Kappa statistical analysis and the t-test were conducted. RESULTS: Among the 100 pediatric patients, 65 were male and 35 were female. The mean age was 50+/-45 months. 63 patients had simple skull fractures and 22 had complex fractures, and the types of fractures were linear fractures in 74, diastatic fractures 15, depressed fractures in 10, penetrating fracture in 1, and greenstick fractures in 3 patients. Statistical difference was observed for the predictive value of simple skull fractures' diagnostic rate depending on the method for diagnosis. A significant difference of the Kappa value was noted in the diagnosis of depressed skull fractures and diastatic skull fractures. CONCLUSION: In the majority of pediatric skull fractures, 3D-CT showed superior diagnosis rates compared to simple skull X-rays and therefore 3D-CT is recommended whenever skull fractures are suspected. This is especially true for depressed skull fractures and diastatic skull fractures.

PURPOSE: To elucidate high-resolution CT(HRCT) findings and their pathologic basis in pulmonary oil embolism induced by LipiodoI-Adriamycin emulsion. MATERIALS AND METHODS: Pulmonary oil embolism was induced by infusing LipiodoI-Adriamycin emulsion through a peripheral vein in twelve Yorkshire pigs. Serial HRCT scans were performed on 2rid, 4th, 7th, 14th, and 28th day after the procedure. The pigs were sacrificed immediately after HRCT and histologic specimens were prepared in the same plane and level with HRCT. RESULTS: The basic pathology was reversible hemorrhagic edema of the lung. On HRCT, intraalveolar hemorrhage and edema in the acute stage manifested as ground-glass opacity or air-space consolidation of the whole secondary Iobule. The lesions were predominantly distributed over the dependent posterior lung fields because the specific gravity of Lipiodol is 1.28. Interlobular septal thickening due to edematous fluid collection was also associated. With the elapse of time, the extent and severity of the acute lesions resolved and, sometimes, changed into small nodular opacities. Pulmonary opacity was most severe on the post-embolization 2nd day and completely resolved within 2 weeks. CONCLUSION: Pulmonary embolization of LipiodoI-Adriamycin emulsion causes reversible hemorrhagic edema of the lung and Lipiodol toxicity seems to play a major role. HRCT findings of pulmonary oil embolism are quite different from those of pneumonia and pulmonary metastasis, which suggests the possibility of clincal application.
