A marked low concentration of serum uric acid(0.7-1.2mg/dl) was detected in a 14-year-old boy with recurrent episodes of gross hematuria. The hypouricemia accompanied with a markedly increased urinary clearance of uric acid (32.6-56.0ml/min), which was only minimally changed after both the administration of pyrazinamide, and inhibitor of the renal tubular secretion of uric acid, and the administration of probenecid, and inhibitor of the renal tubular reabsorption of uric acid. Other renal tubular functions were normal. There were no other family members with hypouricemia. Thies is the first case report of isolated renal hypouricemia due to presecretory reabsorption defect of uric acid in the renal proximal tubule in Korea. And renal hypouricemia should be included in the diagnosis of hematuria.

The peritoneal equilibration test(PET) has been recommended in adults as a standardized means of estimating solute transport. However, it appears that norrns for pediatric PD patients may be different. We performed a PET on 16 children aged 3 to 18 years using a dwell volume for 30ml/kg of 2.596 dialysate. Our children transported glucose more rapidly than adults, however, creatinine transport was not significantly different. Age did not correlate with D/13% glucose or drainage volume. There was negative correlation between D/D% glucose and D/P Cr. There was no significant difference between initial and maintenance CAPD patients. There was slight difference in patients with previous peritonitis. In conclusion, there was difference in glucose transport between children and adults.

Two cases of skull metastasis from thyroid carcinoma are presented. The one case shows multiple involvement inthe skull base with sphenoid sinus, frontal bone, and rib. The other case is solitary extensive involvement to theright parietal bone. The follicular caroinoma of the thyroid shows a strong tendency toward vascualr invasion and dissemination through blood stream to the lungs, bones, liver, brain, and other distant sites. The sternum, thevetebrae, and the pelvis are the most common sites in bone metastasis of thyroid carcinoma, followed by the femur,shoulder girdle, skull, and rib. Metastatic caroinoma to the skull base, including sphenoid sinus s extremelyrare. Authors experienced recently 2 cases of metastatic carcinoma of the thyroid to the skull and report withreview of literature.

In order to evaluate the possible roles of secretory proteases in the pathogenesis of amoebic keratitis, we purified and characterized a serine protease secreted by Acanthamoeba lugdunensis KA/E2, isolated from a Korean keratitis patient. The ammonium sulfate-precipitated culture supernatant of the isolate was purified by sequential chromatography on CM-Sepharose, Sephacryl S-200, and mono Q-anion exchange column. The purified 33 kDa protease had a pH optimum of 8.5 and a temperature optimum of 55 degrees C. Phenylmethylsulfonylfluoride and 4- (2- Aminoethyl) -benzenesulfonyl-fluoride, both serine protease specific inhibitors, inhibited almost completely the activity of the 33 kDa protease whereas other classes of inhibitors did not affect its activity. The 33 kDa enzyme degraded various extracellular matrix proteins and serum proteins. Our results strongly suggest that the 33 kDa serine protease secreted from this keratopathogenic Acanthamoeba play important roles in the pathogenesis of amoebic keratitis, such as in corneal tissue invasion, immune evasion and nutrient uptake.

Giant serpentine aneurysm(GSA) is a giant aneurysm containing a serpentine channel which enters and exits at separate sites and is continuous with the parent vessel. This 33-year-old man with a two-year history of frontal headache presented with bitemporal hemianopsia three months before admission. Suprasellar mass of computed tomography revealed a giant(3X2.5X2cm) mixed density, mimicking a brain tumor. Cerebral magnetic resonance images showed a flow void within the mass, and this was consistent with the serpentine vascular channel demonstrated by angiography. The aneurysm was revealed by surgery involving the frontobasal interhemispheric approach, left A1 & A2 were clipped and the mass of the aneurysm was removed in toto an aneurysmorrhaphy was constructed, and to preserve the distal blood flow to the left A2, the patient made an unevenful recovery.
