BACKGROUND AND OBJECTIVES: Protein kinase C (PKC) is known to be related with development of various cells. In the heart, each isoform reacts differentially against agonists and the reaction changes during development. In this study, the roles of PKC isoforms (alpha, beta, gamma, delta, epsilon, zeta) were investigated through the localization of mRNA expression in the developing rat heart with in situ hybridization histochemistry. MATERIALS AND METHOD: The mRNA expression pattern of PKC isoforms (alpha, beta, gamma, delta, epsilon, zeta) was investigated with in situ hybridization histochemistry in developing and adult rat hearts. Whole body parasagittal sections were used for embryonal day 14 (E14), E16, E18 and heart sections were used for just born (P0), postnatal day 7 (P7), P14, P21 and adult rat. RESULTS: The expression of PKC alpha was found from E14, peaked at P7, and gradually decreased to adult level. The expression of PKC beta was observed from P14, peaked at P21, and decreased to adult level. The expression of PKC delta in the heart was observed from E14, peaked at P0, and abruptly disappeared at P14. The expression of PKC epsilon was observed from E14, peaked at P0, after that gradually decreased and disappeared at adult rat heart. The expression of PKC gamma and zeta was not found from any stage of developing rat heart. CONCLUSION: From these results, it is suspected that each PKC isoform may be differentially related with development of heart. The strong expression of PKC alpha, delta, epsilon around perinatal period, rapidly developing stage, suggests that PKC alpha, delta, epsilon may be related with rapid development of rat heart. And the late postnatal expression of PKC beta suggests that PKC beta may be related with maturation of rat heart.

Acromioclavicular joint dislocations are frequently seen with industrial accident, sports activity and traffic accident. Various operative treatment modalities have been suggested. The purpose of this study is to compare the clinical results of two operative methods in young patients with acromioclavicular dislocation. The authors analyzed the clinical and radiological results of 30 patients with acromioclavicular dislocation, in whom 15 patients were treated by acromioclavicular reduction & fixation with K-wire(Phemister procedure) and 15 patients by acromioclavicular reduction & coracoclavicular fixation by cancellous screw with coracoclavicular ligament repair(Bosworth procedure) from March 1989 to September 1993 at Ansan Hospital, Korea University. They were followed up more than 12 months. The results obtained were as follows: 1. The difference of coracoclavicular distance compared with normal side after operation was 2.6mm in Phemister operations and 1.6 mm in Bosworth operations. And these differences were not changed at follow up significantly. 2. The complications were two K-wire migrations, one superficial infection, and one recurred gross deformity after pin removal in Phemister operations and two mild upward migrations of screw after Bosworth operations. 3. We obtained similar good results after Phemister and Bosworth operations. But less complications were visible, and anatomical reduction and early ROM exercise were possible after Bosworth operations.

PURPOSE: Retinoic acid (RA) is well known as a potent teratogenic agent in both deficiency and excess. Cellular retinoic acid binding proteins (CRABPs) are involved in RA. We carried this study in order to determine the possible relations of CRABPs with RA-induced teratogenesis through observation of the expression patterns of CRABP l and ll in developing rats. METHODS: 35S-labeled RNA probes were synthesized using SP6-RNA polymerase in CRABP l and T7-polymerase in CRABP ll. The distribution of CRABP l and ll transcripts analyzed by in situ hybridization of rat embryosections from day 12 to 19, and postnatal brains from day 1 to 14. RESULTS: The CRABP ll transcripts were more widely distributed than CRABPl distribution, however, the relative level of CRABPl transcripts were higher than CRABP ll. The CRABP l mRNA transcripts showed its highest expression on the 16th day of gestation and these distribution correlated well with structures known to be targets of RA-induced teratogenesis. CRABP ll transcripts were expressed in brain vesicle, spinal cord, head, face, tongue and genital tubercle and also found in the structures which are not involved in RA-induced teratogenesis. CONCLUSION: These results suggest the possible involvement of both CRABPs in the RA-induced teratogenesis. However, CRABP l may have more specific roles than CRABP ll which may play a role through a different mechanism.

Transformation and progression of breast cancer are thought to be caused by an accumulation of complex genetic alterations, but little is known about specific changes. In this study, the author has undertaken a genome-wide screening to detect genetic changes in 20 cases of breast cancer among Koreans, including 16 infiltrating ductal carcinomas, 2 medullary carcinomas, 1 invasive lobular carcinoma, and 1 borderline phyllodes tumor. Comparative genomic hybridization (CGH) was used to screen for DNA sequence gains and losses across all human chromosomes. Simultaneous immunohistochemical staining for c-erbB-2 (Her-2/neu), c-myc, cyclin D1, and p53 protein was done to make comparisons with nuclear grade and that with CGH results. Biotin-labeled tumor DNA and digoxigenin-labeled normal DNA were hybridized to normal metaphase cells. The fluorescence signals were captured by fluorescence microscope after detection by avidin-FITC and anti-digoxigenin rhodamine. Then, the ratio of fluorescence was calculated by an image analyzer. The immunohistochemical staining was done in paraffin-embedded tissue with an LSAB kit and avidin-biotin complex (ABC) method. The CGH results showed gains on chromosomes 8q (40%), 1q (30%), 17q (15%), 20q (15%), 18q (15%), 5p (15%), and 13q (15%). Deletions were on chromosomes 17p (45%) and 22q (20%). High-level amplifications (green/red ratio >1.5) were noted on chromosomes 1p31, 1q, 3q25-qter, 5p, 7q31-qter, 8q, 9p22-qter, 10p, 11p, 11q22-qter, 12p, 12q24, 14q21-qter, 15q23-qter, 17q, 18p, 18q12-qter, 20p, and 20q. By comparison with infiltrating ductal carcinoma, the two medullary carcinomas showed high-level amplification on chromosomes 1p31, 1q, 8q, 10p, 11p and 12p. c-erbB-2, c-myc, cyclin D1, and p53 protein expression was immunohistochemically detected in 9 of 20 (45%), 8 of 20 (40%), 10 of 20 (50%), and 13 of 20 (65%), respectively. The results indicate that the amplification on chromosome 8q, 1q and the deletions on chromosomes 17p and 22q are the most frequent genetic alterations in breast cancers among Koreans. The results reveal a different pattern of genetic alteration from previous studies. The CGH results were not correlated with the immunohistochemical profiles. The amplification pattern of medullary carcinomas was quite different from the pattern of infiltrating ductal carcinomas. The CGH was thought to be very useful in the screening of genetic alterations of solid tumors.

After the introduction of antibiotics, empyema is a rare complication of retropharyngeal abscess caused by acute epiglottitis. But once it occurs, it may be a fatal outcome. Retropharyngeal abscess may spread to mediastinum and pleural cavity along the deep cervical fascia, then it can induce pneumonia, mediastinitis, empyema, and sepsis. Because of its fatal complication, early diagnosis and intensive surgical treatment, such as incision and drainage, is necessary. Now we have a experience of empyema caused by retropharyngeal abscess in a 56-year old diabetic patient with nephropathy. He was admitted to our hospital because of hoarseness and sore throat due to acute epiglottitis. Several days after his admission, he complained of swelling of neck, which was diagnosed as retropharyngeal abscess by the computerized tomography. Retropharyngeal abscess was managed with antibiotics, incision and drainage and culture revealed Peptostreptococcus prevotii. Subsequently empyema developed in his right chest, which was managed with closed thoracotomy, though, he expired due to progression of sepsis.

PURPOSE: To measure the magnetization transfer ratio(MTR) of intracranial tumors and to evaluate the difference of the character and the border of tumors on magnetization transfer image. MATERIALS AND METHODS: We prospectively reviewed magnetization transfer imaging(MTI) findings of intracranial tumors of 13 patients. 1.0T MRI machine was used. T1 weighted image(TRITE=700/14) and T1 weighted magnetization transfer image were obtained. The offset and bandwidth of magnetization transfer pulse were 1000 Hz and 250Hz, respectively. Postcontrast images were also obtained. MTR was measured at lesion area. RESULTS: The mean of normal MTR of white matter and gray matter in 7 normal volunteers were 37% and 29%, respectively. The MTR of each tumor was as follows;metastatic tumors(mean 16.3%), meningioma(mean 27.3%), oligodendroglioma(mean 20.2%), glomus jugulare tumor(mean 17.3%), gliomatosis cerebri(mean 27. 0%). The contrast of lesions were better at magnetization transfer image than at T1 weighted image. CONCLUSION: The MTR of the intracranial tumors is lower than normal tissue. Measurement of MTR at muirpie areas within the tumor can be useful for evaluation of indistinct border of the tumor.

Minimally invasive cardiac surgery including robotic technique has become increasingly popular over the last decade. The advantages of such technique include improved cosmesis and healing, and reduced stress response, hospital and intensive care unit stay, and transfusion requirements. Robot-assisted cardiac surgery requires prolonged one-lung ventilation to optimize exposure. Remote-access perfusion requires appropriate positioning of multiple catheters to establish cardiopulmonary bypass. Carbon dioxide insufflation into the thorax can cause hemodynamic instability and carbon dioxide embolism. Limited exposure of the heart may pose difficulties with management of arrhythmia, hemostasis, myocardial protection and de-airing at the end of surgery. Limited access due to robot manipulator would make rapid intervention for cardiopulmonary resuscitation difficult or impossible. This case report describes robot-asssisted atrial septal defect repair and discusses the anesthetic issues associated with minimally invasive cardiac surgery including robotic cardiac surgery.

Pleomorphic adenoma is the most common salivary neoplasm and occurs mainly in major salivary glands, especially the parotid gland. It is characterized by a variable histopathologic appearance, and a high recurrence rate with malignant transformation depending on previous surgical treatment method used. A benign mixed tumor occurring in minor salivary glands is believed to show the same clinicopathologic appearance and a relatively low recurrence rate as in major salivary glands. Pleomorphic adenoma is the most common intraoral salivary gland tumor and characteristically presents as a slow, painless growth. Herein, we report a case of a patient with pleomorphic adenoma that affected the upper lip.
