2.A Clinical Study of Obstructing Subinfundibular Muscular Bundles of the Right Ventricle in Congenital Cardiovascular Anomalies.

During the period of 4 years from May, 1981 to April, 1985, 71 cases of obstructing subinfundibular muscular bundles of the right ventricle in congential cardiovascular anomalies were diagnosed by cariac catheterization and angiography at Hanyang University. The finding of right ventricular angiogram and degree of the right ventricular outflow obstruction, clinical data, electrocardiographic data and hemodynamic data were correlated irrespectively. The summary of this article is as follows. 1) The age of patients was ranged from 1 to 22 years old with a mean age of 8 years old. There were 39 males and 32 females with M:F ratio of 1.2:1. The incidence of obstructing subinfundibular muscular bundles of the right ventricle in congenital cardiovascular anomalies was higher with increasing age. 2) The associated cardic anomalies were as follows: 40 cases (56%) of isolated ventricular septal defect(VSD), 13(18%) of tetralogy of Fallot physiology, 7(10%) of patent ductus arteriosus(PDA), 3(4%) of pulmonary stenosis, 1 aortic stenosis, 1 double outlet of right ventricle(DORV), 1 trilogy, 1 ostium secundum defect, etc. The incidence of VSD with or without other associated cardiovascular anomalies was 56 cases(79%) out of 71 cases. 3) Maximum systolic pressure gradient between proximal and distal chamber of the right ventricle were under 25 mmHg in 32 cases, between 25 and 50 mmHg in 13 and above 50 mmHg in 26. Pressure gradients of all 7 cases with PDA were under 25 mmHg. 4) Correlative assesment of angiographic manifestation(2 indicies:Diameter of right ventricular outflow tract(systolic phase)/diameter of tricuspid valvular annulus(diastolic phase)=OT/TV, Diameter of right ventricular outflow tract(systolic phase)/Length of right ventricular diaphragmatic surface(systolic phase)=OT/RV) according to pressure gradient, OT/Tv and OT/RV values were lower the increasing pressure gradient, between proximal and distal chamber of the right ventricle. These were reverse correlations but coefficients of correlation(r) were-0.49and -0.48. Therefore, the degree of right ventricular outflow obstruction could be predicted, using 2 indices of right ventricular angiogram in individual cases, but could not be calculated accurately. 5) This indicated that pressuer gradient was also affected by technical errors, variable cardic anomalies, development of sinusoid, age and the other factors. 6) We assumed that non-invasive Doppler echocardiography could be useful in making the diagnosis and follow up of the patient with obstructing subinfundibular muscle bundles in right ventricle.

3.Therapeutic Uses of T-lens (Soflens) in Bullous Keratopathy.

21 patients suffering from bullous keratopathy were fitted with T-Iens (Soflens) continuously for average 12 months without adverse effect. The T-Iens (Soflens) produced dramatic relief of ocular pain occurred in 9 patients among the 12 paitents. The T-Iens (Soflens) produced mild clinical improvement of vesicle. visual auity and corneal edema.

4.Influence of a Soft Contact Lens (Soflens) on the Penetration of 24NaCI and 3H Cortisol Solution into the Anterior Chamber.

A soft contact lens is used not only for the correction of refractive errors, but also for therapeutic purpose. Soft contact lens is hydrophilic, it takes up and releases medicaments so that it promotes and prolongs the penetration of drugs through the cornea. It has been also recently report!,!d that a soft lens increases pressure lowering effect in pilocarpine therapy. The purpose of this study is to evaluate the penetration rate of drugs immersed into a soft contact lens (Soflens) through the cornea. In this study the authors used 24NaCl and [3H] cortisol solution on the rabbit cornea with or without Soflens. The results were as follows: 1. During four hours following the administration of 24NaCl the pent ration rate into the anterior chamber when Soflens, immersed in 24NaCl solution for 2 minutes was applied, showed 2.4 times higher than that of topical instillation. In topical instillation, the Soflens group revealed a higher penetration rate (1.6 times) than non-Soflens group. It was revealed that Soflens promoted the penetration of 24NaCl solution into the anterior chamber of rabbits. 2. Intracorneal penetration of 24NaCl solution was 1.5 times higher in topical instillation in the Soflens group than the non-Soflens group. Meanwhile, the highest penetration (2.2 times) was noted in the group of Soflens which were soaked in 24NaCl solution. The highest concentration in the iris (1.5 times) was noted in the soaked Soflens group. 3. In topical instillation of cortisol solution in the group of Soflens without treatment and the group of Soflens immersed in cortisol solution showed much lower penetration rates into the anterior chamber with 0.9 and 0.5 times than those of the topical instillation group. Based on the above results, it is clear that Soflens does not always promote the penetration of drugs into the anterior chamber. The drug-sponging effect is supposed to very according to various factors such as solubility, molecular weight of drug and physico-chemical properties of a soft con tact lens.

6.Prevalence of Tardive Dyskinesia among the Hospitalized Schizophrenic Patients.

OBJECT: This cross-sectional study was performed in order to evaluate the prevalence of tardive dyskinesia among the hospitalized schizophrenic patients. METHODS: Four hundred nineteen hospitalized schizophrenic patients(male=263, female=156) were recruited for this study. They were treated with antipsychotics for more than 3 months. The prevalence of tardive dyskinesia was assessed by the Abnormal Involuntary Movement Scale. RESULTS: The prevalence of tardive dyskinesia was 35.6%(Male=36.9%, Female 33.3%). There were no significant differences in the prevalence of tardive dyskinesia among male and female schizophrenic patients. The prevalence of tardive dyskinesia among the patients over 30years old was much higher than those below 30years old. There were no significant correlations between the prevalence of tardive dyskinesia and the duration of hospitalization, the total amount of antipsychotics. The frequently involved parts of the body in the schizophrenic patients who have tardive dyskinesia were tongue, upper extremity, lips and perioral area, jaw, lower extremity, muscles of facial expression trunk, respectively. CONCLUSIONS: There was significant correlation between the age and the prevalence of tardive dyskinesia in the antipsychotic-treated schizophrenic patients.

7.Postoperative Intubation Time in Patients Undergoing Open Heart Surgery.

BACKGROUND: Cardiac patients undergoing open heart surgery usually require ventilatory support that involves ICU admission in the postoperative period. We tried to find out determinants of postoperative ventilatory support time. METHODS: We reviewed the medical records of 56 open heart surgery patients retrospectively in terms of their disease, preoperative physical status, age, post-bypass arterial oxygen tension/inspired oxygen fraction (PaO2/FIO2) ratio, number of inotropics used and searched the relationship between each factor and postoperative intubation time. RESULTS: None of the factors except the number of inotropics used had an significant influence on the postoperative intubation time. CONCLUSION: In open heart surgical patients their disease, preoperative physical status, age, postbypass PaO2/FIO2 ratio do not affect postoperative intubation time rather than number of inotropics used does.

8.Effects of Lipopolysaccharide on Inflammation within the Bladder Muscle and Muscle Contractility in the Rats.

PURPOSE: The mechanisms, which alter urinary bladder muscle function during infectious cystitis, are poorly understood. The objective of this study was to identify potential resident targets for endotoxin lipopolysaccharide(LPS) within normal bladder smooth muscle and test the hypothesis that LPS induces an inflammatory response within the bladder muscle and that this is associated with a decrease in muscle contractility. MATERIALS AND METHODS: Male rats were studied 24 hours after a single bolus i.p. injection of LPS(15mg/kg). Whole-mount preparations of the bladder muscle were immunohistochemically stained for neutrophils(myeloperoxidase), macrophages(ED2), activated leukocytes(LFA-1) and mast cells(FITC-Avidin). Contractile activity was assessed from muscle strips of the bladder in response to bethanechol(0.3-300microM). Voiding frequency and urine volume for 24 hours were measured using metabolic cage. Cystometry was performed to measure the intravesical bladder pressure. RESULTS: Using the resident macrophage marker ED2, dense network of macrophages were observed within the bladder muscle of control and LPS treated rats. Few neutrophils(myeloperoxidase-positive cells, 2.3 +/- 0.38 cells, x200) were detected in whole-mounts of bladder muscle of control rats, while LPS pretreatment resulted in a significant increase in the number of neutrophils which demonstrate inflammatory response(10.8 +/- 1.70 cells, x200, p<0.001). LFA-1 immunohistochemistry demonstrated an increased presence of LFA-1 positive cells in bladder muscle of LPS treated rats, which had a morphology similar to both neutrophils and resident macrophages. The expression of LFA-1 is known as a marker of cells that are in an activated state. LPS pretreatment resulted in a significant reduction in bladder muscle contractions in response to bethanechol(i.e. control = 0.049 +/- 0.010 vs. LPS= 0.029 +/- 0.003 gr/mm2/sec, 100microM, p<0.05). Voiding frequency of LPS treated rats was significantly increased compared to that of control rats. In LPS treated rats, voiding phase representing bladder contractility in cystometry was observed. CONCLUSIONS: These data demonstrate that a single intraperitoneal injection of LPS initiates an inflammatory response within the bladder muscle that is associated with a decrease in the functional activity of the bladder. We hypothesize that secretions from the resident macrophages and extravasated leukocytes within the muscle cause the observed suppression in bladder muscle activity in vitro.

9.Clinical Analysis of Internal Carotid Artery Aneurysms.

The authors analyzed 60 cases of ruptured internal carotid artery(ICA) aneurysms admitted and operated at Department of Neurosurgery in Kyung Hee University Hospital during the period from July 1983 to June 1988. The results of analysis were summarized as follows; 1) Mean age of patients was 51 years. ICA aneurysms occurred more frequently in female(47) than male(13). 2) Distribution of ICA aneurysms were ophthalmic aneurysm 8 cases(13.3%), posterior communicating aneurysm 40 cases(66.6%), anterior choroidal aneurysm 7 cases(11.6%), bifurcation aneurysm 5 cases(8.3%). 3) Most common direction of aneurysmal dome in ophthalmic, anterior choroidal and posterior communicating aneurysm was posterior-inferior-lateral and in bifurcation aneurysm was posterior-superior. 4) Multiple aneurysm was 23 cases(38%). 5) Incidence of clinical vasospasm was 48.3% and that of rebleeding was 20%. 6) Clipping was done in all cases, 2 cases combined with wrapping and 2 cases with trapping. 7) Mortality rate was 21.6%.

10.Effect of Topical Propolis on Wound Healing Process After Tonsillectomy: Randomized Controlled Study.

OBJECTIVES: The post-tonsillectomy pain and post-tonsillectomy hemorrhage are the two main problems after tonsillectomy. The aim of this study was to investigate the beneficial effects of water soluble ethanol extract propolis on post-tonsillectomy patient. METHODS: One hundred and thirty patients who underwent tonsillectomy or adenotonsillectomy were randomly divided into the control and propolis groups, each including 65 patients. The propolis group was applied with propolis orally immediately after surgery and by gargle. The pain scores were assessed on post-tonsillectomy 0, 1st, 2nd, 3rd, and 7th–10th day using a visual analogue scale score. Postoperative wound healing was evaluated by scoring pinkish membrane of tonsillar fossae on postoperative days 3 and 7–10. The incidence of post-tonsillectomy bleeding was examined in each group. RESULTS: Post-tonsillectomy pain was significantly less in propolis group compared to control group on postoperative days 3 and 7–10. Post-tonsillectomy hemorrhage was significantly less in the propolis group compared to the control group (P < 0.05). The wound healing was significantly better in the propolis group compared to the control group on postoperative day 7–10 (P=0.002). CONCLUSION: Applying the propolis to post-tonsillectomy wound showed beneficial effect of reducing postoperative pain, preventing hemorrhage, and accelerating of wound healing of tonsillar fossae.
